Vagal gastric efferent activity and its reflex changes in conscious sheep.

Electromyographical recordings from neck muscles reinnervated by the vagus nerve allowed monitoring of vagal output to the reticulo-rumen in conscious sheep. The vagus nerve was sectioned below the nodose ganglion and its central end anastomosed with the peripheral end of the spinal accessory nerve, which innervates the trapezius and mastoido-humeral muscles. Fifty-three vagal efferent units recorded were identified and classified into seven distinguishable types on the basis of their temporal relationship with gastric movements. With respect to their patterns of discharge during the primary c

The Efficacy of Neuromodulation, Interventional Treatment and Unconventional Therapies in the Treatment of Complex Regional Pain Syndrome: A Systematic Review.

Complex regional pain syndrome (CRPS) is a chronic debilitating multisystem neuropathic pain disorder. It is characterized by continuous pain, usually out of proportion to any known tissue injury, vasomotor changes, sudomotor or edema, and motor or trophic changes. The objective of this study is to assess the efficacy of neuromodulation, interventional, and unconventional treatments for CRPS. The primary focus is pain reduction, assessed through various scales, with secondary outcomes examining effects on autonomic, sensory, motor, and psychological aspects, and quality of life. Impact of single vs. dual local anaesthetic regimen on the efficacy and safety of percutaneous stellate ganglion block for electrical storm: new insights from the STAR study.

AIMS: Percutaneous stellate ganglion block (PSGB) is a promising treatment for refractory electrical storm (ES). However, there is a lack of solid indications regarding the optimal pharmacological regimens and combination of local anaesthetics (LAs). This study, from the multicentre STAR registry, aimed to evaluate the impact of single vs. dual LA regimen on the efficacy and safety of PSGB. METHODS AND RESULTS: We analysed 422 PSGB procedures from 298 patients (30% single LA, 70% dual LAs). Both regimens significantly reduced the number of treated ventricular arrhythmias (VAs) in the 12 h pos

Effect of preoperative ultrasound-guided stellate ganglion block on pain thresholds and postoperative nausea and vomiting in gynecological laparoscopy: a randomised clinical trial.

BACKGROUND AND OBJECTIVES: Despite advancements in laparoscopic techniques, postoperative pain and nausea/vomiting (PONV) remain primary barriers to optimal Enhanced Recovery After Surgery (ERAS) outcomes in gynecology. While stellate ganglion block (SGB) modulates autonomic function, its efficacy in elevating pain thresholds, reducing rescue medication use, and accelerating functional recovery requires robust evidence. This randomized trial evaluated whether preoperative SGB could refine these ERAS metrics. METHODS: This single-center randomized controlled trial included 84 patients undergoin

A novel approach to diabetes-related foot ulcers/disease: First report of bipolar pulsed RF lumbar sympathetic block.

BackgroundDiabetes-related foot ulcers (DFUs), often complicated by peripheral arterial disease (PAD), remain a major therapeutic challenge. In patients unsuitable for revascularization, alternative treatments are essential. This study is the first to evaluate bipolar radiofrequency (RF) ablation as part of lumbar sympathetic block (LSB) for enhancing wound healing in DFU patients with critical limb ischemia.MethodsThis retrospective cohort study included 31 DFU patients with severe PAD who were not candidates for revascularization. Group 1 (n = 18) received LSB with bipolar RF plus medical ca

Sympathetic Blockade for Pain Associated With Nonaxial Bone Lesions in Patients With Cancer: An Uncontrolled Cohort.

BACKGROUND: Cancer-related bone pain remains a prevalent and frequently incapacitating ailment. Although conventional approaches effectively alleviate pain in most individuals, a subset of patients may continue to experience intractable pain. Current recommendations for treating cancer-related bone pain include oral analgesics and multimodal adjuvants, radiation therapy, and, in selected cases, intrathecal therapy. Cancer-related bone pain is mediated by a proliferation of sensory and sympathetic fibers. Thus, we believe that this pain can be successfully managed with minimally invasive sympat

Laparoscopic versus open approach to neurolytic celiac plexus block in inoperable pancreatic cancer.

BACKGROUND: Neurolytic celiac plexus block (NCPB) is an effective method of palliative pain control. The purpose of this study was to evaluate the feasibility and effectiveness of the laparoscopic NCPB versus open approach. METHODS: Eight patients (Group A) underwent diagnostic laparoscopy which revealed an inoperable pancreatic cancer. Forty millilitres of solution (20 mL of 95% ethanol mixed with 20 mL of xylocaine) was injected into either side of para-aortic soft tissue. The same solution was injected in 10 patients (Group B), with inoperable pancreatic body cancer diagnosed during laparo

Continuous cervical epidural block: Treatment for intractable hiccups.

Intractable hiccups, although rare, may result in severe morbidity, including sleep deprivation, poor food intake, respiratory muscle fatigue, aspiration pneumonia, and death. Despite these potentially fatal complications, the etiology of intractable hiccups and definitive treatment are unknown. This study aimed to evaluate the effectiveness of continuous cervical epidural block in the treatment of intractable hiccups.Records from 28 patients with a history of unsuccessful medical and invasive treatments for hiccups were evaluated. Continuous cervical epidural block was performed with a midlin

Treatment of patients with painful blind eye using stellate ganglion block.

BACKGROUND AND OBJECTIVES: management of pain in painful blind eyes is still a challenge. Corticosteroids and hypotensive agents, as well as evisceration and enucleation, are some of the strategies employed so far that are not always effective and, depending on the strategy, cause a deep emotional shock to the patient. Given these issues, the aim of this case report is to demonstrate a new and viable option for the management of such pain by treating the painful blind eye with the stellate ganglion block technique, a procedure that has never been described in the literature for this purpose.

Trans-facet-joint approach to pulsed radiofrequency ablation of the L5 dorsal root ganglion in a patient with degenerative spondylosis and scoliosis.

Mechanical low back pain secondary to degenerative facet joint changes is a common reason for referral to pain clinics. When these changes cause encroachment into the intervertebral foramen, radicular pain may result. While pulsed radiofrequency of the dorsal root ganglion can be used in this setting, the anatomic deformity may make the transforaminal approach difficult. We report a case where a trans-facet approach was used successfully and describe the technique.

Pharmacodynamic actions of (S)-2-[4,5-dihydro-5-propyl-2-(3H)-furylidene]-1,3-cyclopentanedione (oudenone).

The pharmacodynamic actions of (S)-2-[4,5-dihydro-5-propyl-2(3H)-furylidene]-1,3-cyclopentanedione (oudenone) were studied in both anesthetized animals and isolated organs. Oudenone (10--40 mg/kg i.v.) induced an initial rise in blood pressure followed by a prolonged hypotension in the anesthetized rats. In unanesthetized spontaneously hypertensive rats (SHR), oudenone (5--200 mg/kg p.o.) caused a dose-related decrease in the systolic blood pressure. The initial pressor effect was diminished by pretreatments with phentolamine, guanethidine, hexamethonium and was abolished in the pithed rats. I

Effectiveness of Ultrasound-Guided Stellate Ganglion Block for Breast Cancer-Related Lymphedema Patients Undergoing Lymphovenous Anastomosis: A Prospective, Randomized, and Controlled Pilot Study.

Lymphovenous anastomosis (LVA) and stellate ganglion block (SGB) have been reported as effective treatments for breast cancer-related lymphedema (BCRL). However, no studies have evaluated the efficacy of ultrasound-guided SGB in patients undergoing LVA. This study aimed to assess whether adding SGB to LVA improves outcomes in BCRL patients. Thirty BCRL patients were randomly assigned to either the control (LVA only, n = 15) or SGB group (n = 15). The SGB group received ultrasound-guided SGBs at 2 days, 2 weeks, and 4 weeks postoperatively. Arm circumferences were measured at four points (5 cm
