Intraoperative ultrasonography of the biliary tract using saline as a contrast agent: a fast and accurate technique to identify complex biliary anatomy.

BACKGROUND: Intraoperative assessment of biliary tract anatomy is relevant for a number of benign and malignant hepatobiliary diseases. During biliary reconstruction, drainage of all relevant bile ducts is imperative to prevent atrophy of undrained segment, cholangitis and secondary biliary cirrhosis. Intraoperative cholangiography, though widely used for intraoperative imaging of the biliary tract, involves heavy equipment use, radiation risk and has a limited role in the evaluation of isolated segmental bile ducts. METHODS: We evaluated the use of a novel technique of intraoperative ultraso…

[Prevention, diagnosis and surgical treatment of injuries of the biliary tract during laparoscopic cholecystectomy. Treatment of papilla injuries caused by invasive endoscopy. Part 2. Surgical treatment of biliary tract injuries].

INTRODUCTION: Introduction of endoscopic invasive procedures in the 70th and 80th years leaded to decrease reoperations on biliary tree. latrogenic injury of the biliary tract have increased in incidence in the first decade with the introduction of laparoscopic cholecystectomy. Athough a number of factors have been identified with a high risk of injury (and number of technical steps have been emphasized to avoid these injury, the incidence of the bile duct injury has reached at least double the rate observed with open cholecystectomy. Most patients that sustained a bile duct injury are recogni…

Pharmaco-genomic characterization of pancreatic and biliary tract cancer tumoroids for drug response.

Pancreatic ductal adenocarcinoma (PDAC) and biliary tract cancer (BTC) are highly aggressive malignancies with limited therapeutic options. In this study, we established eleven tumoroids with paired patient-derived xenograft (PDX) models (five PDAC and six BTC), enabling scalable in vitro drug screening and corresponding in vivo validation. These tumoroids retained the histological and genetic characteristics of their original tumors and exhibited varied responses to chemotherapeutic agents. Drug screening identified PI3Kα inhibitors as promising candidates for both PDAC and BTC tumoroids, whi…

Facility Type Predicts Completeness of Oncologic Resection and Survival in Biliary Tract Cancers.

PURPOSE: Complete Oncologic Resection (COR) for biliary tract cancers (BTC) includes negative margin status and adequate lymphadenectomy, defined as retrieval of ≥ 6 lymph nodes. Whether these standards and their association with survival vary across facility types and BTC subtypes remains unclear. METHODS: Using the National Cancer Database (2004–2022), we identified adults undergoing curative-intent resection for Stage I-III BTC, excluding T1a gallbladder cancer. Facility types were compared for COR using nested logistic regression, adjusting for demographic and tumor factors. Overall surviv…

Differential bioenergetic reprogramming driven by sorafenib reveals therapeutic vulnerabilities in biliary tract cancers.

Biliary tract cancers (BTCs) comprise biologically heterogeneous subtypes with limited therapeutic options and variable responses to sorafenib. The biological basis underlying this variability remains unclear. In this study, we compared intrahepatic cholangiocarcinoma (iCCA) and gallbladder cancer (GBC) models under defined sorafenib exposure conditions using transcriptomic profiling, Seahorse-based mitochondrial stress assays, pharmacologic combination analysis, and patient-derived organoid models. In representative iCCA models, 1.25 μM sorafenib increased mitochondrial respiration and ATP pr…

Circular RNA-Based Molecular Computation Enhances Plasma Biomarker Detection in Biliary Tract Cancer.

Molecular diagnosis of biliary tract cancer (BTC) remains a significant clinical challenge due to the lack of sensitive and specific diagnostic tools. Although large panels based on multi-biomarkers have demonstrated potential in enhancing diagnostic accuracy, their clinical application is hindered by complexity and high costs. To overcome these limitations, we have developed a DNA-based molecular computation system that integrates the detection of plasma circular RNAs (circRNAs) with molecular computation, enabling intelligent and rapid diagnosis of BTC. By identifying and validating a specif…

Association of candidate surrogate endpoints with overall survival in advanced biliary tract cancer.

BACKGROUND & AIMS: Surrogate endpoints are increasingly used in biliary tract cancer (BTC) trials. While this may expedite drug approval and decrease costs, surrogate endpoints may not always correlate with an overall survival (OS) advantage. We aimed to explore the association of progression-free survival (PFS), objective response rate (ORR) and disease control rate (DCR) with OS at the trial- and patient-level. METHODS: For the trial-level analysis, we performed a systematic review of Pubmed/MEDLINE, Embase, Cochrane, clinicaltrials.gov and conference proceedings for phase II-III trials in …

Prognostic roles nutritional index in patients with resectable and advanced biliary tract cancers.

BACKGROUND: Biliary tract cancer (BTC) is a rare, aggressive malignancy with increasing incidence and poor prognosis. Identifying preoperative prognostic factors is crucial for effective risk-benefit assessments and patient stratification. The prognostic nutritional index (PNI), which reflects immune-inflammatory and nutritional status, has shown prognostic value in various cancers, but its significance in BTC remains unclear. AIM: To assess the prognostic value of the preoperative PNI in BTC patients, with a focus on overall survival (OS) and disease-free survival (DFS). METHODS: Comprehens…

Survival outcome of patient with pT1N0 biliary tract cancer treated with surgery alone.

INTRODUCTION: Adjuvant chemotherapy (AC) with S-1 or capecitabine monotherapy is now the standard of care for resected biliary tract cancer (BTC) according to the Adjuvant S-1 for Cholangiocarcinoma Trial (ASCOT) and the BILCAP study. Patients selection criteria, especially regarding pT1N0 BTC, differed in both trials. We aimed to clarify the survival outcomes regarding resected pT1N0 BTC without AC. METHODS: Among patients with macroscopically complete resection for BTC treated without AC between September 1992 and December 2020, the survival outcomes of those with pT1N0 BTC, except for intr…

Treatment resistance in pancreatic and biliary tract cancer: molecular and clinical pharmacology perspectives.

INTRODUCTION: Treatment resistance poses a significant obstacle in oncology, especially in biliary tract cancer (BTC) and pancreatic cancer (PC). Current therapeutic options include chemotherapy, targeted therapy, and immunotherapy. Resistance to these treatments may arise due to diverse molecular mechanisms, such as genetic and epigenetic modifications, altered drug metabolism and efflux, and changes in the tumor microenvironment. Identifying and overcoming these mechanisms is a major focus of research: strategies being explored include combination therapies, modulation of the tumor microenvi…

Advances in omics-based biomarker discovery for biliary tract malignancy Diagnosis:A narrative review.

Biliary tract neoplasms, which originate from the intrahepatic or extrahepatic biliary epithelium, are relatively rare but diagnostically challenging types of tumours, and their morbidity and mortality have increased in recent years. Due to ineffective early diagnostic methods, once detected, patients are in an advanced stage with a poor prognosis and few treatment options. With the development of omics technologies, the associations between microorganisms, bile acid and salts, noncoding RNAs and biliary tract malignancies have been gradually revealed, providing new methods for the discovery o…

Evolving therapeutic landscape of advanced biliary tract cancer: from chemotherapy to molecular targets.

Biliary tract cancer, the second most common type of liver cancer, remains a therapeutic challenge due to its late diagnosis and poor prognosis. In recent years, it has become evident that classical chemotherapy might not be the optimal treatment for patients with biliary tract cancer, especially after failure of first-line therapy. Finding new treatment options and strategies to improve the survival of these patients is therefore crucial. With the rise and increasing availability of genetic testing in patients with tumor, novel treatment approaches targeting specific genetic alterations have …

[18F]FDG PET/CT in Benign and Malignant Diseases Involving the Biliary Tract.

Role of FDG PET/CT in evaluation of biliary tract diseases remains relatively unexplored. PET/CT with FDG helps in evaluation of both infective / inflammatory as well as neoplastic diseases as increased glucose utilization is observed in both the conditions. In this article, we describe the spectrum of FDG PET/CT findings in various diseases affecting the biliary tract. Role of FDG PET/CT in neoplastic diseases involving the biliary duct has been described at the time of staging and response evaluation; in characterization of the intrahepatic mass (abscess v/s cholangiocarcinoma). In addition,…

Clinical outcomes and risk stratification in unresectable biliary tract cancers undergoing radiation therapy.

BACKGROUND: Biliary tract cancers (BTC) are rare and aggressive malignancies originating from intrahepatic and extrahepatic bile ducts and the gallbladder. Surgery is the only curative option, but due to late-stage diagnosis, is frequently not feasible, leaving chemotherapy as the primary treatment. Radiotherapy (RT) can be an effective alternative for patients with unresectable, non-metastatic BTC despite the generally poor prognosis and significant variability. To help manage patients with unresectable BTC who receive RT, we aimed to identify prognostic markers that could aid in predicting o…

Exploring Molecular Mechanisms in the Second-Line Treatment of Biliary Tract Malignant Tumors.

Biliary tract malignant tumors account for about 3% of gastrointestinal malignancies. Based on anatomical location, biliary tract malignant tumors can be divided into gallbladder carcinoma, intrahepatic cholangiocarcinoma (ICC), hilar cholangiocarcinoma, and distal cholangiocarcinoma. Surgical treatment is the main treatment for early-stage biliary malignant tumors, the insidious nature of the disease often leads to late diagnoses, causing many patients missing the window for surgical intervention. Gemcitabine combined with cisplatin serves as a first-line treatment for patients with advanced …

Anatomic patterns of recurrence in biliary tract cancers: does primary tumor site matter?

BACKGROUND: Recommendations for postoperative surveillance and adjuvant therapy following curative-intent resection for biliary tract cancers-including intrahepatic and extrahepatic cholangiocarcinoma (IHCCA and EHCCA) and primary gallbladder cancer (GBC)-are uniform across primary tumor site. However, these tumors may have distinct patterns of recurrence. METHODS: A retrospective observational cohort study was performed at a specialty cancer center. Patients undergoing resection of IHCCA, EHCCA, and GBC were identified (2005-2020). Recurrence-free survival (RFS) was estimated using Kaplan-Me…
