[Flexible flatfoot in children: variation within normal range or need for treatment?].

Flexible flatfoot in childhood is a common cause for repetitive consultations and the diagnosis is verified by the clinical examination. In most cases the findings are age-dependent variants of the norm and if asymptomatic there is no need for treatment. In the first decade of life symptomatic flexible flatfoot should initially be treated with shoe inserts. Further diagnostic steps are required once conservative treatment is unsuccessful or a rigid structural deformity is found in the clinical examination. The underlying reasons may be neuropathic or structural anatomical in origin. Lateral co

Corrective-elongation osteotomy without bone graft for old ankle fracture with residual diastasis.

The main principle for treatment of ankle fractures is anatomic reduction until bony union is achieved. Old fractures of the ankle with residual diastasis, however, may cause persistent pain, joint effusion, and range-of-motion limitation, and make eventual ankle arthrodesis inevitable. Restoration of the integrity of the ankle mortise is the determining factor for successful repair of this type of ankle fracture. Old ankle fracture, where malunion has already occurred, is a great challenge for the orthopedic surgeon. Twelve such patients were treated by means of reconstructive corrective-elon

Biomechanical consequences of lateral column lengthening of the calcaneus: Part I. Long plantar ligament strain.

Lateral column lengthening of the calcaneus has been a powerful tool used to correct peritalar subluxation in symptomatic flat feet. The mechanical basis and limits for correction with this procedure are not well understood. A flatfoot model was created on 8 fresh-frozen cadaver feet by sectioning the deltoid ligament, talonavicular capsule, and spring ligament. Strain-gauge analysis of the long plantar ligament was performed (on 6 specimens) as the lateral column was sequentially lengthened from 4 to 12 mm in 2-mm increments. Results showed that only the lateral most one-third of the long pla

[Surgical approach in plastic of shortened lower limb].

OBJECTIVE: To introduce a new surgical approach to rectify the shortened lower limbs. METHODS: From March 1985 to October 2000, 288 cases of shortened lower limbs were treated and reviewed. Closed fracture at the metaphysis was made by a self-made "needle saw", and then the "multiple-plane and double-track elongation instrument" was adopted to elongate the fractured bone. There were totally 161 cases of male and 127 cases of female included, with average age 21.3 years old, ranging from 12 to 29 years old, among which there were 268 cases elongated at the proximal metaphysis of the tibia, 16

Limb lengthening in children with achondroplasia. Differences based on gender.

Twelve achondroplastic dwarfs underwent serial limb lengthening during childhood. All had repeated bilateral simultaneous elongation of their lower limbs. The height of these patients was plotted on growth curves for children with achondroplasia. The end results were evaluated after all patients reached skeletal maturity. Growth acceleration was seen in male patients with achondroplasia, but no such effect was seen in females. The female patients lost several centimeters as calculated from their predicted final height based on their preoperative growth curve and the centimeters gained by mecha

Treatment of malunion of the lower extremities by the Ilizarov technique.

From June 1989 to February 1992, 22 patients with post-traumatic malunion of the lower limbs were surgically treated by the Ilizarov technique in our hospital. Their average age was 23.0 years (range, 10.4-38.7 years). The average follow-up time was 35.7 months (range, 18-50 months). They were divided into group A (14 cases): those with post-traumatic shortening only (average shortening: 4.0 cm for the femur, 4.0 cm for the tibia) and group B (eight cases): those with combined post-traumatic shortening and angular deformity (average shortening, 4.6 cm for the femur, 2.7 cm for the tibia; avera

Joint reaction forces during femoral lengthening.

The purpose of this study was to observe the reactive force across the hip and knee joint in a cadaver study during simulated femoral lengthening. Load cells were placed in the hip and knee joints while force measurements were taken after every 1 mm of lengthening. The lengthening was performed on one side with a distal osteotomy and on the other with a proximal osteotomy to determine if the level of the osteotomy had any affect on the forces across the proximal and distal joints. The study showed that the forces at the hip and knee joints increased during lengthening. The proximal osteotomy a

Claudication in an adolescent with a hypoplastic femur.

The most common cause of claudication is atherosclerosis obliterans. However, when it presents in adolescence, other causes should be considered. We describe the case of a 15-year-old girl who had severe intermittent claudication 8 years after a limb-lengthening procedure for a hypoplastic femur. The lesion responsible was an isolated fibromuscular dysplastic segment of the distal superficial femoral artery and proximal popliteal artery. The etiology, treatment, and histopathology are discussed.

Early experience with leg lengthening by callotasis and chondrodiastasis.

Over a four year period, 20 segments in 16 patients were treated by lengthening by callotasis or chondrodiastasis. The indications for treatment were leg length discrepancy in 13 patients and short stature in three. Patients with leg length discrepancy were all treated by femoral lengthening; the mean length gained was 4.4 cm (range 2.5 to 6.Ocm). For patients with short stature, the mean femoral length gain was 10.0 cm (9.0 to 10.5 cm), and the mean tibial length gain was 8.0 cm (6.5 to 9.0 cm). The commonest problem was pin tract infection, but this always settled with antibiotic therapy. Mo

Amputation stump lengthening with the Ilizarov technique. A case report.

It is often difficult to create a functional stump from a traumatic amputation, especially in a child. A case is described of a traumatic, high, below-knee amputation in a five-year-old child. The resulting stump was too short to allow a conventional below-knee prosthesis, decreasing gait efficiency. The Ilizarov technique was used to increase tibial length. Bony lengthening was very successful, but several problems were encountered with the soft-tissue reconstruction. This technique of stump reconstruction holds promise only if the soft-tissue problems are anticipated. The suggestions for fut

[Supracondylar corrective osteotomy in post-traumatic cubitus varus. 3 cases in 2 patients].

Supracondylar fractures of the humerus in childhood frequently result in cubitus varus deformity. Pain, functional impairment and cosmetic problems can request surgical correction. This is a report on an adult and on a mature child with such deformities. The boy had sustained three injuries with following bilateral cubitus varus and lengthening of one lower leg. Planning, surgical technique and outcome of the osteotomies are described. We should recommend the resection of a full wedge with two oblique osteotomies and internal fixation with two plates on either side of the humerus.

New experiences in treating postburn talipes equinovarus associated with bone and joint pathologic changes.

This article aims to solve the problem of postburn talipes equinovarus associated with bone and joint pathologic changes by simplified and modified techniques. In addition to lengthening the shortened Achilles tendon and the contracted scar above it, we performed triple arthrodesis directly on the densely scarred, deformed foot without replacing the scar tissue with normal tissue tube or flap. Only the proximal scar-periosteum wound edge of the curved incision, 2 cm distal to the conventional one, was elevated to expose the bones to be excised. The two excised wedge-shaped bone blocks, one on

Spontaneous Reduction of Radial Head Dislocation by Gradual Ulnar Lengthening in Patients With Ulnar Growth Disturbance.

PURPOSE: Radial head dislocation associated with a relatively short ulna is the most severe deformity of the forearm. Surgical reduction of the radial head is challenging and often yields unpredictable results. This study investigated whether, in pediatric patients, a dislocated radial head could be spontaneously reduced through gradual ulnar lengthening. METHODS: In this retrospective study, we reviewed patients who underwent ulnar lengthening by distraction osteogenesis from June 2008 to March 2024. Patients with short ulnas accompanied by dislocated radial heads were included. The radial h

Passive Eversion Assessment for Progressive Collapsing Foot Deformity After Lateral Column Lengthening: A Cadaveric Biomechanical Study.

BACKGROUND: Reduced hindfoot eversion motion has been proposed as a cause of increased lateral foot pressure following lateral column lengthening (LCL) for progressive collapsing foot deformity (PCFD). A subjective intraoperative assessment of passive eversion has been suggested to help evaluate correction; however, it is unclear how passive eversion correlates with objective measurements of foot stiffness. Our objectives were to quantify the relationship between the maximum passive eversion in hindfoot joints following LCL with plantar pressure during stance and to determine the influence of

Impairment and restoration of the blood-nerve barrier and its correlation with pain following gradual nerve elongation of the rat sciatic nerve.

Purpose: This study aimed to evaluate the time course of impairment and restoration of the blood-nerve barrier (BNB) following gradual elongation of the sciatic nerve and to clarify its association with nociception.Materials and Methods: The right femur was lengthened at a rate of 1.5 mm/day for 10 days. Von Frey tests were performed until 50 days after lengthening. Compound muscle action potentials (CMAPs) were measured to assess gross dysfunction of the elongated nerve. Evans blue-albumin tracing and immunohistochemistry for endothelial barrier antigen (EBA), rat endothelial cell antigen-1 (

Morscher Osteotomy Through Surgical Dislocation Approach for True Femoral Neck Lengthening with Greater Trochanter Transposition.

BACKGROUND: Young adults presenting with hip pain can be affected by proximal femoral growth disturbances as seen in Legg-Calvé-Perthes disease (LCPD) or as a complication of surgical treatment of developmental dysplasia of the hip (DDH). In 1988, Morscher proposed a novel femoral neck lengthening osteotomy to address these issues. The purpose of this study was to evaluate the effectiveness and safety of the Morscher osteotomy as a procedure to complement the well-documented surgical hip dislocation, to increase femoral offset, to distalize the greater trochanter, and to increase the overall l

Hip stability during lengthening in children with congenital femoral deficiency.

BACKGROUND: Congenital femoral deficiency (CFD) is one of the most challenging and complex conditions for limb lengthening. We focused on the problem of hip instability during femoral lengthening because subluxation and dislocation are potentially catastrophic for hip function. METHODS: We assessed for hip stability in 69 children (91 femoral lengthenings) who had CFD Paley type 1a (43 children) and 1b (26 children). The mean age at first lengthening was 6.4 years. RESULTS: Hip subluxation/dislocation occurred during 14 (15 %) of 91 lengthenings. Thirty-three pelvic osteotomies were performe

Comparison of the calcaneo-cuboid-cuneiform osteotomies and the calcaneal lengthening osteotomy in the surgical treatment of symptomatic flexible flatfoot.

BACKGROUND: Surgery is indicated in symptomatic flatfoot when conservative treatment fails to relieve the symptoms. Osteotomies appear to be the best choice for these painful feet. The purpose of this study was to compare the clinical and radiographic outcome of the calcaneo-cuboid-cuneiform osteotomies (triple C) and the calcaneal-lengthening osteotomy in the treatment of children with symptomatic flexible flatfoot. METHODS: The surgeries were performed by senior surgeons who preferred either triple C or calcaneal lengthening. The results were graded by an orthopaedic surgeon uninvolved with
