Multiple Brain Abscesses Due to Aspergillus Fumigatus in a Patient With Liver Cirrhosis: A Case Report.

Invasive cerebral aspergillosis always developed in immunocompromised host. Early diagnosis may save life in this critical condition; however, it is difficult to reach. Herein, we presented an unusual case of invasive cerebral aspergillosis in a cirrhotic patient. A 47-year-old man presented with progressive deterioration of consciousness for three days. The patient had a history of alcoholic liver cirrhosis, Child-Pugh class C. Magnetic resonance imaging (MRI) of brain showed multi-focal parenchymal lesions, which was consistent with multiple brain abscesses. The diagnosis of invasive cerebra…

Anesthesia for a Rare Case of Uncorrected Pentalogy of Fallot Undergoing Craniotomy and Drainage of Brain Abscess.

Children with uncorrected cyanotic congenital heart diseases can present for non cardiac surgeries. They pose several challenges to the Anaesthesiologist, especially when they are posted for emergency surgery, due to the complex haemodynamic changes secondary to the heart disease. Pentalogy of Fallot (POF) is a rare form of congenital heart disease characterized by the association of Tetralogy of Fallot (TOF) with an atrial septal defect (ASD). TOF is the leading cause of intracardiac right to left shunt and is the commonest type of cyanotic congenital heart disease to cause a brain abscess. C…

Treatment of a brain abscess caused by Scedosporium apiospermum and Phaeoacremonium parasiticum in a renal transplant recipient.

Cerebral mycosis is a significant cause of morbidity among immunocompromised populations. We present here a case of cerebral infection with Scedosporium apiospermum and Phaeoacremonium parasiticum in a 49-year-old renal transplant recipient. Fourteen years after renal transplantation, the patient presented with invasive pulmonary aspergillosis treated with intravenous liposomal amphotericin B. The patient had clinical and radiographic improvement. However, 6 weeks later, the patient presented with cerebral infection. Magnetic resonance imaging revealed multiple rim enhancing brain abscesses. B…

Extensive pneumocephalus extending into the lateral ventricles from a brain abscess: an intracranial complication of cholesteatoma.

OBJECTIVE: We report the case of a patient with extensive pneumocephalus extending into the lateral ventricles from a brain abscess arising from a cholesteatoma-induced defect in the skull base. CASE REPORT: A 70-year-old man with cholesteatoma presented with right-sided otalgia, otorrhoea and progressive headaches. Computed tomography showed a tegmental defect (approximately 2 × 2 cm) at the right mastoid antrum. A T1-weighted, gadolinium-enhanced magnetic resonance imaging scan showed pneumocephalus in both lateral ventricles, which was directly connected to the mastoid cavity via a brain a…

Fatal cerebritis and brain abscesses following a nontraumatic subdural hematoma in a chronic hemodialyzed patient.

Staphylococcus aureus is the leading cause of bacteremia in hemodialysis-dependent patients that can lead to metastatic abscesses with poor outcome. We report a case of a 65-year-old chronic hemodialyzed male patient who developed cerebritis and brain abscesses complicating a spontaneous subdural hematoma, following Staphylococcus aureus bacteremia related to infected arteriovenous fistula. In spite of adequate antibiotherapy and several surgical brain drainages, our patient did not survive. Prevention of S. aureus is highly important in hemodialysis patients.

Successful treatment of aspergillus brain abscess in a child with acute lymphoblastic leukemia and liver failure.

Invasive fungal infection continues to pose a significant threat to immunocompromised patients, with cerebral aspergillosis being among the most feared ones. The authors describe an adolescent girl with acute lymphoblastic leukemia (ALL) with subsequent acute liver failure, who developed an aspergillus brain abscess. The patient was treated with combined antifungal therapy using amphotericin B local instillation, prolonged systemic amphotericin B colloidal dispersion along with vinca alkaloids-containing chemotherapy, followed by neurosurgical débridement and oral voriconazole in the setting o…

Brain Abscesses of Ear, Nose, and Throat Origin: Comparison between Otogenic and Sinogenic Etiologies.

This retrospective study analyzed 29 cerebral abscesses of ear, nose, and throat (ENT) origin. The mean follow-up of patients was 37 months. … inages (p = 0.02). Bacteriologic abscesses samples were positive in 90% of cases. Bacteria found in brain abscesses were different from the ones found in ENT samples in 62% of cases. Thus, the results of ENT bacteriologic samples were not helpful for choosing adequate antibiotic agents in case of negative brain abscess samples. Although mortality was not signif …

Streptococcus Brain Abscess in a Healthy 30-Year-Old: A Case Report and Brief Review of Non-Neoplastic Mass-like Brain Lesions.

The reported case surrounds a previously healthy 30-year-old male who initially presented to primary care with acute vision changes. Further workup and imaging revealed an occipital mass lesion which appeared to enlarge after initiation of steroids. Debridement and specimen collection for culturing revealed an abscess growing Streptococcus intermedius. The consideration of a broad differential diagnosis for both neoplastic and non-neoplastic causes of tumor-like brain lesions is a critical component in accurate diagnosis and management in these rare cases.

Remarkable recovery from a Cladophialophora bantiana fungal brain abscess in an immunocompromised patient: a case report.

BACKGROUND: Cladophialophora bantiana is a globally distributed dematiaceous mold which acts as an uncommon source of human infection. With … ity and return to pre-infection baselines. CONCLUSION: This is a rare case of a C. bantiana fungal brain abscess in an immunocompromised patient. Complete surgical resection was not possible; however, this case provides additional data to show the benefits of a dual antifungal treatment approach without additional surgical intervention, and the dosages used to …

Brain Abscess Likely Secondary to Infective Endocarditis Mimicking High-Grade Glioma in a Patient with Pyelonephritis.

Brain abscesses are serious central nervous system infections that often present with nonspecific infectious and neurologic symptoms and may mimic intracranial neoplasms on imaging. They can develop rapidly or insidiously, and diagnosis is often delayed when symptoms overlap with concurrent systemic infections. We report the case of a 66-year-old woman admitted for pyelonephritis who initially improved with intravenous antibiotics but later developed persistent fever, worsening occipital headache, and acute neurologic deterioration. Initial imaging demonstrated an irregular ring-enhancing lesi…

Nocardia farcinica Brain Abscess Mimicking Acute Ischemic Stroke in an Immunocompetent Elderly Patient: A Case Report.

Central nervous system (CNS) infections caused by Nocardia species are rare but potentially life-threatening. Even more unusual is Nocardia infection in immunocompetent patients. Most patients initially present with a primary respiratory or cutaneous infection that may progress to disseminated disease involving the CNS; however, primary CNS infection is rare. Nocardia farcinica is frequently associated with brain abscesses. We report the case of an 81-year-old immunocompetent man who presented with acute focal neurological deficits and seizures, initially managed as an ischemic stroke. Subsequ…

Case report: Laboratory diagnosis methods on brain abscess pathogens caused by anaerobic species and Streptococci constellatus.

Brain abscess (BA) represents a severe, intracranial infection associated with high morbidity and mortality. Multiple pathogens may contribute to BA development, some of which are difficult to detect using conventional diagnostic methods alone. With the wide utilization of metagenomic next-generation sequencing (mNGS), more experiences are concluded in authenticity practice except for being regarded as a comprehensive method for detecting all pathogens. Here, we report a severe case of BA in a patient diagnosed by CT imaging, who subsequently underwent surgical removal of the abscess. To ident…
