The Lazarus sign is a well-documented spinal reflex observed in brain-dead patients, characterized by involuntary upper limb movements that can resemble voluntary gestures. Initially described during the 1980s, this phenomenon has contributed to ongoing discussions about the physiological basis of spinal reflexes and their role in assessing brain death. The Lazarus sign was once considered a diagnostic challenge, but recent research reports confirm that it originates solely from the spinal cord, independent of brainstem or cortical activity. However, recent studies also suggest that spinal ref…

Since Organ Transplant Act was legislated and enforced in 1997, there reported to be 86 cases of organ donation from brain-dead donors, and 70 cases for heart transplantation, 67 for lung transplantation, 67 for liver transplantation, 64 for pancreas transplantation and 103 for kidney transplantation as of the end of May, 2010. The organ donation from brain-dead persons in Japan required the documented will of the deceased to accept brain death and to remove his/her organs for the purpose of transplantation, and the written consent of his/her family member to brain death diagnosis and organ do…

Neuropathologic findings in stillbirths oftentimes provide insight into the specific mechanisms leading to death. Examination of the brains of stillborn infants may also identify pathophysiologic processes that result in prenatal brain injury in liveborn as well as stillborn infants and that lead to neurologic disorders in liveborn infants, such as cerebral palsy or the sudden infant death syndrome (SIDS). A variety of abnormalities are found in the brains of stillborns, the most common including cerebral white matter necrosis (periventricular leukomalacia) or gliosis, germinal matrix or intra

OBJECTIVE: Acquired epilepsy is associated with increased risk of death, but whether this extends to epilepsy after brain infections is unknown. Our objective was to estimate mortality and causes of death in patients with epilepsy after four types of brain infections.
METHODS: A population-wide Swedish cohort was created from the National Patient Registry, including all patients over 18 years of age who received inpatient care in 2000-2010 for herpes encephalitis, brain abscesses, other encephalitis, or bacterial meningitis. Cox proportional hazards modelling was used to compare the hazard ra

Septic brain injury is a severe disease of the central nervous system and involves alteration of the cell death pathway due to sepsis. Increasing evidence indicates that Toll-like receptor 4 (TLR4) plays a key role in the development of sepsis. Cell death is also thought to contribute to septic brain injury. However, the mechanism that regulates cell death pathways in response to TLR4 in neurons exposed to septic brain injury is unclear. Here, we established a rat model of septic brain injury to evaluate the effect of TLR4 inhibition on cell death pathways. Furthermore, primary neurons with an

There is a critical mismatch between available organs for transplant and acutely or critically ill patients with end-stage organ disease. Patients who may benefit from organ transplantation far outnumber available organs. The causes for this imbalance are multiple. One cause is family refusal to donate. A second cause is nonrecognition or delay in determination of brain death. A third cause is donor loss due to profound cardiopulmonary and metabolic instability consequent to brain-stem herniation and brain death. Family refusal may be addressed by education, public awareness, as well as close …

It seems that increasing the knowledge of people about brain death and organizing strategies to confirm brain death for families are necessary to meet the organ shortage.

Successful organ transplantation offers patients with end stage organ failure the chance of a normal life. The recognition of brain death allowed the use of beating heart donors and this has enabled multiple organ procurement from a single donor. Suitable patients with severe brain injury resulting in brain death, who may be potential organ donors, are to be found on both neurosurgical and general intensive care units. The pathophysiological results of brain death are similar, irrespective of the underlying cause. Severe brain injury may result in the loss of temperature regulation, and the de…

Organ donation is often the only treatment option for patients with severe, life-threatening organ insufficiencies. The precondition for organ donation in Germany is the irreversible loss of brain function (ILB). When ILB occure, multiple changes in the organism take place, including the endocrinological system. Organ-protective therapy includes all essential organ systems and, in addition to differentiated volume and catecholamine therapy, lung-protective ventilation and the maintenance of homeostasis with regard to the acid-base balance, electrolytes and also the endocrinological system. INTRODUCTION: The suicide Implicit Association Test (S-IAT) captures the strength of the implicit identification between self and death and is one of the few suicide-specific behavioral tasks that uniquely predicts future suicide risk. Thus, identifying brain regions associated with the S-IAT provides insights into the neural mechanisms underlying suicidality.
METHODS: This study measured brain activation during the S-IAT with concurrent fMRI in a post-9/11 trauma-exposed veteran sample. In total, 37 post-9/11 veterans at low risk for suicide participated in this study as part of an ongoing l

OBJECTIVES: Brain death is defined as the permanent absence of all cortical and brain stem reflexes. A wide range of spontaneous or reflex movements that are considered medullary reflexes are observed in heart beating cases that appear brain dead, which may create uncertainty about the diagnosis of brain death and cause delays in deceased-donor organ donation process. We determined the frequency and type of medullary reflexes and factors affecting their occurrence in brain dead cases.
MATERIALS AND METHODS: During 1 year, 122 cases who fulfilled the criteria for brain death were admitted to t…

INTRODUCTION: There has been a great improvement in transplantation medicine in Brazil in the last 2 decades. However, there remain several barriers regarding notification of brain and cardiac death as well as completion of the donation process.
METHODS: This retrospective study was performed between January 2008 and December 2010. We reviewed all deaths in a University Hospital, observing the causes of non-notification to the State Transplantation Authority and non-donations.
RESULTS: There were 41 notifications of brain death resulting in donation in only 19.5% of those cases. Cardiac deat…

In two patients, men aged 23 and 42 years, a condition that mimicked brain death was observed as a consequence of rapidly progressive complete peripheral paralyses, which included the intrinsic and extrinsic eye muscles. However, the EEG revealed a waking pattern. Maximal supportive therapy was provided, which included haemodialysis for the first patient and artificial ventilation for both patients. A slow recovery was seen after four weeks. The first patient was paralyzed following the ingestion of a large quantity of ethylene glycol and the second by botulism due to the consumption of injudi…

The presence of pregnancy in a brain-dead woman is a rare circumstance. We present a case of a 31-year-old woman who was 22 weeks pregnant at the time of diagnosis of brain death after intracranial and subarachnoid hemorrhage. After a multidisciplinary approach, the decision was made to continue somatic support to maintain the pregnancy until optimal fetus viability. Cesarean section was performed after 11 weeks (33 weeks gestational age) of brain-death diagnosis with a successful delivery of a live infant. Management of brain-death complications during pregnancy is described.

OBJECTIVE: To analyze the role that sympathetic nervous system hyperactivity immediately following brain death plays in the cardiovascular dysfunction of an organ donor.
MATERIAL AND METHOD: Fifteen New Zealand white rabbits were placed in three groups: a control group, a brain-death group without propranolol and a brain-death group with propranolol. Brain death was caused by severe intracranial hypertension; in the third group propranolol was first perfused intravenously. We recorded hemodynamic (heart rate, number of extrasystoles, central venous pressure, systemic and pulmonary arterial pr…
