Benign breast disease affects almost all women, although only one out of 11 will eventually develop breast cancer. Fibrocystic disease or benign breast mastopathies have been associated with an increased propensity to progress to malignancy. However, the increased relative risk for women with benign breast disease developing breast cancer appears to be associated with proliferative benign disease in association with atypia. 1,271 patients with breast cancer treated at the Institut Gustave Roussy between 1967 and 1972 and with a minimum follow-up of 10 years, have been studied in order to analyse the risk factors for bilateralization. The case for treating women at high risk of breast cancer with the anti-oestrogen tamoxifen is presented. This is a logical extension of a screening programme and is scientifically based on the effectiveness of tamoxifen in established disease, the relation between free oestradiol (and other hormone-related factors) to the risk of breast cancer, and the lack of side-effects from tamoxifen treatment. Choice of a high-risk group is critical; a specific trial protocol is outlined.

This work is concerned with a morphometric analysis of 23 breast fibroadenomas subdivided into four histological subgroups from purely pericanalicular to purely endocanalicular forms. The volume fractions (Vv) of the lumina and the total volume: epithelial surface ratios (Sv) showed a significant (P less than 0.001) correlation (r = + 0.64). In addition, the Vv and Sv series did not correlate with any of the histological subgroups. The data reported in this work suggest that the ductal epithelium proliferates in fibroadenomas, and that the subtypes are not related to the epithelial growth.

We have analysed the data of biopsy, tumour size, size, side and axillary lymph node status in a group of 866 patients presenting with breast cancer in Southampton from September 1975 to August 1981 and a group of 1424 women presenting with benign disease in the same period. The whole group showed a seasonal variation, with a peak presentation in June. The pre-menopausal age group account for almost all of this seasonality. The cause of the variation is not established and requires further investigation. The control group of benign breast disease cases did not show a seasonal variation.

Fibromatosis of the breast results in aggressive, infiltrative lesions with a propensity for local recurrence. We describe an unusual case of fibromatosis producing a growth resembling carcinoma in a 39-year-old woman. Recognizing this infrequently encountered condition is important to avoid unnecessary radical surgery.

A retrospective study of 134 patients with a clinical diagnosis of fibro-adenoma of the breast indicated that histological confirmation of this diagnosis is made in only 50 per cent. The majority of the others have a diagnosis of benign mammary dysplasia. Eight patients had an unsuspected carcinoma, all but one being above the mean age for the fibro-adenoma group. Two cases of Wegener granulomatosis of the breast are reported. In both cases, this rare disease was detected on mammographic examination and was diagnosed for the first time by a fine-needle aspiration biopsy of the breast. This article discusses the management of the client who presents with a breast lump or abnormality. Approximately 25 percent of women who seek advice in a primary care setting do so because of a breast-related complaint. The practitioner can be instrumental not only in discovering breast abnormalities, but also in facilitating referral for further diagnostic measures, teaching BSE and providing psychological support to the client who has a breast lesion.

The interpretation of ultrasound mammograms is complicated by the heterogeneous tissue structure of the breast. To improve the diagnostic analysis, a method has been developed which enables us to correlate ultrasound images of mastectomized specimen with the cross-sectional anatomy, the computerized tomography, the X-ray photographs of the specimen and the histopathologic study of the corresponding gross serial sections. Some echophenomena considering the propagation properties of ultrasound are discussed.
