Bifocal right ventricular resynchronization for the failing right ventricle.

The present case illustrates that in patients with right ventricular (RV) failure and right bundle branch block it is possible to resynchronize the RV without further worsening RV or left ventricular (LV) pump function, even in cases with various degrees of atrioventricular block. The acute response to different pacing configurations was analyzed in terms of dP/dt variations. Bifocal RV pacing (His bundle plus RV outflow tract pacing) achieved the best acute results and was chosen for permanent pacing. This pacing configuration was associated to clinical and echocardiographic improvement.

Dystrophica myotonia. Correlative electrocardiographic, electrophysiologic, and conduction system study.

Correlative ECG, electrophysiologic (EPS), and pathologic findings of the conduction system (CS) in dystrophica myotonia has not been documented to our knowledge in the English literature. We present such a correlation in two cases. The first at age 55 had right bundle branch block, left anterior fascicular block, and first-degree AV block. At age 65, ECG demonstrated type 1 AV block, and EPS revealed block proximal to the His bundle. Two years later, he died of pneumonia. The CS showed marked degenerative changes and fatty infiltration in the atrial septum and the approaches to the AV node, w

Comparison between conduction system pacing and cardiac resynchronization therapy in right bundle branch block patients.

A significant number of right bundle branch block (RBBB) patients receive cardiac resynchronization therapy (CRT), despite lack of evidence for benefit in this patient group. His bundle (HBP) and left bundle pacing (LBP) are novel CRT delivery methods, but their effect on RBBB remains understudied. We aim to compare pacing-induced electrical synchrony during conventional CRT, HBP, and LBP in RBBB patients with different conduction disturbances, and to investigate whether alternative ways of delivering LBP improve response to pacing. We simulated ventricular activation on twenty-four four-chamb

Transcatheter electrical ablation of right bundle branch. A method of treating macroreentrant ventricular tachycardia attributed to bundle branch reentry.

The present study describes the clinical and electrophysiological characteristics of sustained bundle branch reentrant ventricular tachycardia treated with electrical ablation of the right bundle branch. Seven patients presented with syncopal episodes, and six of the seven had documented episodes of ventricular tachycardia. All patients had depressed left ventricular ejection fraction with cardiomegaly. Six of the seven had dilated cardiomyopathy in the absence of significant coronary disease. Twelve-lead electrocardiograms in all seven patients during sinus rhythm were remarkably similar; six

Coexistent Wenckebach phenomenon in the distal branches of the specialized conduction system.

A patient in whom Wenckebach phenomenon was observed to coexist in the three distal branches of the specialized ventricular conduction system, with an intermittent trifascicular block producing a second degree A-V block, is described. Surface electrocardiogram demonstrated the presence of this phenomenon in the right bundle branch (RBB) and the anterior division of the left bundle branch (LBB). Evidence of the presence of Wenckebach phenomenon in the remaining fascicle was provided by His bundle recording.

Belhassen Ventricular Tachycardia in a Child.

First reported in 1981, idiopathic left ventricular tachycardia (VT) of the Belhassen type is characterized during electrocardiography (ECG) by a right bundle branch pattern and left axis deviation. We report the case of a 15-year-old Hispanic male who, during a routine evaluation ECG to support sports participation, was found to have nonsustained monomorphic VT. Prior to his exercise treadmill test, his physical examination and echocardiogram were normal. Then, during preparation for the exercise treadmill test, the ECG showed sustained monomorphic VT with a right bundle branch block pattern

Left bundle branch area pacing in congenitally corrected transposition of great arteries - The obvious choice?

Patients with congenitally corrected transposition of the great arteries (CCTGA) are susceptible to acquired atrioventricular conduction disease in early life. Emerging studies propose that conduction system pacing either by His bundle pacing or bundle branch pacing is advantageous in this population. The anatomical abnormality of CCTGA conveniently positions the left bundle branches on the easily accessible right ventricular septal side. We present a case of a young female CCTGA patient with exercise-induced heart block who received left bundle branch area pacing with the ventricular lead imp

Involvement of a nodofascicular connection in supraventricular tachycardia with VA dissociation.

We present the case of a patient with episodes of supraventricular tachycardia and atrial dissociation that were terminated by either adenosine or verapamil. Involvement of an accessory pathway was shown by ventricular extrastimuli, elicited during His-bundle refractoriness, that interrupted the tachycardia or advanced the next His potential. The tachycardia circuit was demonstrated to be confined to the nodofascicular region based on the exclusion of surrounding tissues. Atrial activity, including that in the perinodal region, was totally dissociated during tachycardia. The lowest part of the

[Paroxysmal Hisian tachycardia and familial sarcoidosis].

We report a His-bundle tachycardia in a 6 year-old patient without evidence of heart disease. Diagnosis and follow-up were assessed by exercise stress testing and repeated long-term electrocardiographic recording. As sarcoidosis was present in the mother and the aunt, this condition was discussed in this child also. However, since there is no absolute proof for this disease, the hypothesis of an antenatal injury of the bundle of His was discussed: this could be secondary to the transplacental transfer of abnormal maternal antibodies, as frequently observed in women with clinical or biologic ev…

Fascicular Ventricular Tachycardia: An Electrophysiological Paradox.

We present a case of fascicular ventricular tachycardia (VT) in a 27-year-old man. Fascicular VT arises within the fascicles of the left bundle of His and is associated with right bundle branch block and axis deviation on ECG. Fascicular VT can often be mislabelled as a narrow-complex tachycardia due to the proximity of the arrhythmic origin to the normal conduction system, creating a widened, but not classically "broad," QRS complex, as occurred in our case. Our patient was resistant to adenosine but sensitive to verapamil, as is characteristic of fascicular VT owing to its origin in a macros…

Is left bundle branch area pacing the optimal pacing strategy for bradyarrhythmia with preserved ejection fraction? A network meta-analysis.

BACKGROUND: Cardiac physiological pacing (CPP), including biventricular pacing (BiVP), left bundle branch area pacing (LBBAP), and His-bundle pacing (HBP), has become the preferred strategy over right ventricular pacing (RVP) owing to the lower risk of pacing-induced cardiomyopathy. However, the evidence supporting CPP use in bradyarrhythmia with preserved left ventricular ejection fraction (LVEF) remains limited. OBJECTIVE: Our network meta-analysis aimed to explore outcomes of CPP in bradyarrhythmia with preserved LVEF. METHOD: A literature review was conducted from the inception of PubMed

Catheter ablation combined with left bundle optimized cardiac resynchronization therapy for persistent atrial fibrillation complicated with malignant arrhythmia: A case report.

RATIONALE: Atrial fibrillation (AF) is characterized by rapid and disordered atrial electrical activity. Rhythm and rate control are the primary strategies applied in AF management. In clinical practice, the patient's symptoms, comorbidities, drug tolerance, cardioversion effectiveness, and risk of disease must be comprehensively evaluated to reasonably recommend strategies for rhythm and rate control. PATIENT CONCERNS: A 65-year-old Asian man experienced persistent AF complicated by malignant arrhythmia. He was poorly treated with medications, which led to heart failure and repeated wide QRS

Intra-Hisian Wenckebach after surgical closure of conoventricular ventricular septal defect.

We report a 5-year-old girl with transient complete atrioventricular (AV) block following surgical closure of a symptomatic conoventricular ventricular septal defect (VSD) which recovered on post-operative day 9. She later presented with exertional dizziness and fatigue. While congenital cardiac defect repairs are occasionally complicated by complete heart block, this patient was found to have intra-Hisian Wenckebach which is rare in the pediatric population and can be very difficult to discern from surface electrocardiograms and by Holter monitoring. Mechanisms of post-surgical AV block, incl

Left Bundle Branch Pacing: How I Do It?

Search for the combined use of LV lead with HBP yielded 2 clinical outcome case studies and one mechanistic study in peer-reviewed journals. One case series reported implantation and follow-up of outcomes in patients who had an inadequate response to HBP with subsequent additional LV lead. Search for the combined use of LV lead with LBBAP yielded two case series studies in non-peer reviewed publications were found. The His-Optimized CRT (HOT-CRT) is a term introduced describing combining HBP with LV pacing. The LB-Optimized CRT (LOT-CRT) is a term recently used to describe combining LBBAP with

Quantitative assessment of the fast pathway in atrioventricular nodal reentrant tachycardia.

BACKGROUND: Mathematical modelling has allowed calculation of the length of the slow and fast pathways in typical atrioventricular nodal reentrant tachycardia (AVNRT). The length of the slow pathway has been correlated with the measured length of the right inferior extension in human histologic specimens, but no histology data exist about the fast pathway. METHODS: In preparations of cadaveric human hearts, the AV node was identified, and the site of the fast pathway was projected according to both existing evidence and results of our electroanatomic mapping. This permitted measurement of the
