Paragonimiasis is an important re-emerging parasitosis in Japan. Although the lungs and pleural cavity are the principal sites affected with the parasite, ectopic infection can occur in unexpected sites such as skin and brain. This case report describes a patient with active hepatic capsulitis due to Paragonimus westermani infection. The patient was successfully treated with praziquantel at the dose of 75 mg/kg/day for 3 days.

The HIV-1 protease inhibitor (PI) saquinavir is available as a soft gelatin capsule (SGC) formulation. At the recommended dosage of saquinavir SGC (1200mg 3 times daily), this formulation provides around 8-fold greater exposure than the established hard gelatin capsule (HGC) formulation at the recommended dosage of 600mg 3 times daily. As with the HGC formulation, the most common adverse events seen with saquinavir SGC are gastrointestinal symptoms (e.g. diarrhoea, abdominal discomfort and nausea). The specific features of the submicroscopic structure of the causative agents of melioidosis and glanders, grown in culture media and at the initial stages of experimental infection in guinea pigs and golden hamsters, were studied with the use of electronic cytochemistry and immunocytochemistry. P. pseudomallei and P. mallei were found to be capsule-forming bacteria. According to the data obtained with the use of electronic cytochemistry, the basis of the capsular substance P. pseudomallei and P. mallei is formed by polysaccharide biopolymers protecting these bacteria from phagocytosis.

The stretching capability of the zonules was studied in 40 human eyes obtained postmortem from 27 patients. A continuous circular capsulorhexis (CCC) (2.2-6.8 mm) was performed, and the lenses were removed by either phacoemulsification (26 eyes) or planned extracapsular cataract extraction (ECCE) (14 eyes). The capsule and subcapsular epithelium of 18 senile cataractous lenses and 2 normal lenses were studied by scanning and transmission electron microscopy. The structures of the capsule and subcapsular epithelium in most senile cataractous lenses were similar to those in the normal lenses. Low electron density bands composed of tiny granular or fibrillar material deep in the posterior capsule were observed in 4 of the senile cataractous lenses. Degeneration was more marked in the subcapsular epithelial cells in some of the senile cataractous lenses.

A case of multiple Glisson's capsule phleboliths is reported in a 29-year-old-woman who had portal vein obstruction with spontaneous shunting resulting in hepatic siderosis and cirrhosis. The phleboliths were visible as multiple linear calcifications on plain film and computed tomography. Diagnosis was established by surgical liver biopsy. The potential mechanisms of this unique entity is discussed.

The pharmacokinetics and antihypertensive effects of the nifedipine tablet and capsule have been examined in six male patients with diastolic pressure greater than 95 mm Hg despite metoprolol therapy. The presence of capsulate Bacteroides spp. and anaerobic gram-positive cocci was investigated in pus specimens from 182 children with chronic orofacial infections or abscesses and in pharyngeal swabs from 26 children without inflammation.
