Alcohol and Apoptosis: Friends or Foes?

Alcohol abuse causes 79,000 deaths stemming from severe organ damage in the United States every year. Clinical manifestations of long-term alcohol abuse on the cardiac muscle include defective contractility with the development of dilated cardiomyopathy and low-output heart failure; which has poor prognosis with less than 25% survival for more than three years. In contrast, low alcohol consumption has been associated with reduced risk of cardiovascular disease, however the mechanism of this phenomenon remains elusive. The aim of this study was to determine the significance of apoptosis as a me

Influence of Fitness on Exercise Blood Pressure and Cardiac Structure: Data from the Exercise Stress Test Collaboration (EXERTION).

PURPOSE: An abnormally high exercise systolic blood pressure (SBP) is associated with adverse left ventricular (LV) remodelling, a hallmark of hypertensive-heart disease. Fitness may modify this relationship through its effects on BP and cardiac structure, influencing the interpretation of exercise test results. This study explored the influence of fitness on the exercise SBP-cardiac structure relationship in individuals undergoing exercise stress echocardiography. METHODS: Cardiovascular structure and function (LV mass indexed to body surface area (LVMIBSA), relative wall thickness (RWT), ca

Transcatheter Edge-to-Edge Mitral Valve Repair for Severe Regurgitation in Cardiogenic Shock: A Comprehensive Review.

Cardiogenic shock is a critical pathological state marked by end-organ hypoperfusion due to severe cardiac dysfunction and is associated with high mortality. A substantial portion of patients with cardiogenic shock have concomitant severe mitral regurgitation (MR), which exacerbates hemodynamic instability by reducing forward cardiac output and contributes to pulmonary edema and respiratory failure through regurgitant backflow. In this high-risk setting, mitral transcatheter edge-to-edge repair (M-TEER) offers a minimally invasive treatment that can lead to hemodynamic and symptomatic improvem

Tension pneumopericardium in a polytrauma patient.

INTRODUCTION: Tension pneumopericardium is a life-threatening condition, manifesting most commonly as hemodynamic instability caused by cardiac tamponade. Reduced cardiac output and blood pressure can lead to difficulties in the detection of arterial bleeding from associated injuries while the increased venous pressure can increase the rate of bleeding. CASE REPORT: This is the case of a patient after a car accident, with bilateral serial fractures, bilateral pulmonary contusion, bilateral pneumothorax, emphysema of the neck and chest, pneumomediastinum and pneumopericardium, and other injuri

Endovascular repair of arteriovenous fistula after microendoscopic discectomy and lamino-foraminotomy.

The last two decades have seen increasing adoption of minimally invasive approaches to lumbar disc herniation management. As with many new advances in surgery, the risk profile of these contemporary approaches has yet to be well defined. We present the case of a 32-year-old man who presented with decreasing exercise tolerance over a 6-month period after microendoscopic lumbar discectomy and lamino-foraminotomy. Subsequent work-up revealed a large fistula between his right common iliac artery and inferior vena cava, resulting in high-output cardiac failure. This was managed well with an endovas

Neuroendovascular therapies in pediatric interventional radiology.

Transarterial embolization of head and neck tumors can be done for a variety of indications, including tumoral epistaxis and high-output cardiac failure, but is mainly done before surgical resection. This technique offers higher surgical cure rates and faster recovery, while decreasing surgical morbidity by curtailing blood loss and reducing the duration of surgery. Patients with epistaxis are considered for endovascular management when they have failed conservative management, including topical vasoconstrictors, cautery, and nasal packing (anterior and posterior). Sudden arterial bleeding in

Diffuse infantile haemangiomatosis: clinicopathological features and management problems in five fatal cases.

The clinicopathological features of five fatal cases of diffuse haemangiomatosis presenting in neonatal life or early infancy are presented. The infants all had multiple skin haemangiomas as well as deep-seated lesions in many different tissues that caused protean clinical manifestations and management problems. Because the outlook may be improved by early diagnosis and application of new modes of treatment, any infant with multiple cutaneous haemangiomas should be closely assessed for possible visceral involvement. Development of hepatomegaly, high-output cardiac failure, unexplained anaemia

Determination of optimal positive end-expiratory pressure by means of conjunctival oximetry.

A method for determining the optimal level of positive end-expiratory pressure (PEEP) by means of noninvasive conjunctival oxygen (PcjO2) monitoring and arterial blood gas analysis was developed from the pattern of changes in PcjO2 tension, invasive hemodynamic parameters, and oxygen transport variables during PEEP titration in a series of patients with adult respiratory distress syndrome. All patients had radial and pulmonary artery (PA) catheters inserted and blood volume was measured with 125I serum albumin before each study. During progressive increases in the level of PEEP, PcjO2 tensions

Levosimendan use in patients with preoperative low ejection fraction undergoing cardiac surgery: A systematic review with meta-analysis and trial sequential analysis.

OBJECTIVES: Patients with preoperative low left ventricular ejection fraction (LVEF) are known to be associated with high morbidities and mortality in cardiac surgery. The primary aim of this review was to examine the clinical outcomes of levosimendan versus placebo in patients with preoperative low LVEF ≤ 50% undergoing cardiac surgery. DATA SOURCES: MEDLINE, EMBASE, PubMed and CENTRAL were searched systematically from their inception until June 2018. REVIEW METHODS: All the randomised clinical trials (RCTs) were included. RESULTS: Twelve trials were eligible (n = 1867) for inclusion in th

Fast in vivo detection of myocardial norepinephrine levels in the beating porcine heart.

The sympathetic nervous system modulates cardiac function by controlling key parameters such as chronotropy and inotropy. Sympathetic control of ventricular function occurs through extrinsic innervation arising from the stellate ganglia and thoracic sympathetic chain. In the healthy heart, sympathetic release of norepinephrine (NE) results in positive modulation of chronotropy, inotropy, and dromotropy, significantly increasing cardiac output. However, in the setting of myocardial infarction or injury, sympathetic activation persists, contributing to heart failure and increasing the risk of ar

Acute overdosage with thioridazine: a review of the available clinical exposure.

Two hundred and twenty-three cases of acute overdosage associated with thioridazine were reviewed. The most frequent feature was impairment of consciousness which was linearly dose-related and occasionally resulted in life-threatening complications. Arrhythmia was the most frequently reported serious toxic effect. Patients presenting with anoxia were at risk for arrhythmia, as were patients ingesting a high dose. Arrhythmia may, by decreasing cardiac output, predispose to the occurrence of all other observed complications (ie, pulmonary edema, severe hypotension and renal failure). Therefore,

Overview of Best Practices and Complications in Hemodialysis Access.

End-stage renal disease and the number of patients relying on hemodialysis are increasing. Hemodialysis access, such as autogenous arteriovenous (AV) fistulas, prosthetic AV grafts, and tunneled catheters, is essential yet also a major source of morbidity. This focused narrative review summarizes the best practices for access planning. Complications after AV fistulas are reviewed, including stenosis, thrombosis, infection, central venous stenosis, steal syndrome, ischemic monomelic neuropathy, high-output cardiac failure, and aneurysmal degeneration. The Silent Shunt: Traumatic Splenic AV Fistula Revealed and Treated Endovascularly.

BACKGROUND: Splenic arteriovenous fistula (AVF) is a rare vascular anomaly that can arise from trauma, aneurysmal rupture, or congenital conditions. Prompt diagnosis and management are crucial to preventing complications such as portal hypertension and high-output cardiac failure. CASE PRESENTATION: We present the case of a 62-year-old male with a traumatic splenic AVF resulting from a road traffic accident (RTA), associated with a Grade IV splenic injury and multiple pseudoaneurysms. The patient underwent successful endovascular coil embolization and experienced an asymptomatic follow-up. C

Deciphering the neural signature of human cardiovascular regulation.

Cardiovascular regulation is integral to life. Animal studies have identified both neural and endocrine pathways, by which the central nervous system adjusts cardiac output and peripheral vascular resistance to changing physiological demands. The outflow of these pathways is coordinated by various central nervous regions based on afferent information from baroreceptors, chemoreceptors, nociceptors, and circulating hormones, and is modulated by physiologic and behavioural state. In humans, however, knowledge on central cardiovascular regulation below the cortical level is scarce. Here, we show

Mechanical ventilation strategy following Glenn and Fontan surgeries: On going challenge!

The Glenn and Fontan operations put the pulmonary and systemic circulations in series. It has been shown that positive pressure ventilation (PPV) decreases pulmonary blood flow (PBF) and cardiac output (CO), and negative pressure ventilation (NPV) significantly improves PBF and CO. If early extubation is not achievable, the postoperative ventilator management strategy should aim at promoting PBF and CO by lowering pulmonary vascular resistance (PVR) and intrathoracic pressure. Multiple ventilator strategies have been evaluated to optimize this physiology, including high frequency ventilation,

Hypoplastic left heart syndrome in the second trimester.

Hypoplastic left heart syndrome is a relatively common congenital anomaly with a high mortality even after palliative postnatal surgery. The case presented had a normal cardiac cavity and great artery dimensions at 19 weeks of gestation but bright left ventricular myocardial echoes, impaired left ventricular shortening, and no detectable forward flow in the left ventricular outflow tract. Autopsy showed left ventricular subendocardial calcification. This demonstrates a likely early stage in the evolution of hypoplastic left heart syndrome, which has a variable time course. The abnormal left ve

Pulmonary stenosis and impaired myocardial function.

Two young Black female patients with pulmonary valve stenosis and intact ventricular septa are presented in protracted congestive cardiac failure with severe tricuspid insufficiency and, in one, atrial fibrillation. Right ventricular systolic dysfunction was manifested by peak systolic pressures below systemic level, raised end-diastolic pressures and low cardiac output, but without right-to-left shunt. These findings are in strong contrast to those found in most patients with pulmonary stenosis of long standing, where persistent impairment of right ventricular function is diastolic with a hig

Gas exchange in disease: asthma, chronic obstructive pulmonary disease, cystic fibrosis, and interstitial lung disease.

Ventilation-perfusion (VA/Q) inequality is the underlying abnormality determining hypoxemia and hypercapnia in lung diseases. Hypoxemia in asthma is characterized by the presence of low VA/Q units, which persist despite improvement in airway function after an attack. This hypoxemia is generally attenuated by compensatory redistribution of blood flow mediated by hypoxic vasoconstriction and changes in cardiac output, however, mediator release and bronchodilator therapy may cause deterioration. Patients with chronic obstructive pulmonary disease have more complex patterns of VA/Q inequality, whi
