Filling in the gaps: A geospatial approach to optimizing placement of methadone clinics in Massachusetts.

INTRODUCTION: Methadone treatment dispensed at opioid treatment programs (OTPs) reduces the risk of fatal opioid-related overdose. Access to OTPs is limited in many regions. We utilized location-allocation models to identify gaps in OTP services across Massachusetts and generated descriptive statistics to further prioritize areas for expansion of services for socially vulnerable communities. METHODS: We used a geographic information system to generate eight location-allocation models, tools to target the placement of new sites based on the distribution of existing sites and demand for service

Impact of Living in a Food Desert on Complications After Fracture Surgery.

BACKGROUND: Food deserts-communities with limited access to healthy food-have been linked with poor surgical outcomes; however, their impact on orthopaedic trauma outcomes remains unknown. The aims of this study were to determine the prevalence of food desert residency among orthopaedic trauma patients and to investigate the impact of food desert residency on the rate of unplanned reoperation with use of a large, high-quality, prospectively collected dataset with adjudicated outcomes. We hypothesized that orthopaedic trauma patients would reside in food deserts at a higher rate than the genera

Under-five mortality during the war in Tigray: A community-based study.

BACKGROUND: Child mortality is one of the key indicators of the Sustainable development goals. The Ethiopian healthcare system in general and Tigray's healthcare system in particular has shown a remarkable progress in terms of reducing maternal, neonatal, and under-five mortality in the last couple of decades. However, the war erupted in November 2020 caused the healthcare system to collapse and little is known about the status of child mortality in Tigray. Thus, this study aimed to examine the magnitude and causes of under-five child mortality in the embattled Tigray region was conducted from

Socioeconomic status is not associated with unfavorable outcomes in patients with acute limb ischemia.

OBJECTIVE: Whether socioeconomic status (SES) is associated with health outcomes in patients with acute limb ischemia (ALI) is largely unknown. We aimed to determine whether SES is associated with worse presentations and outcomes for patients with ALI. METHODS: We performed a retrospective medical record review of patients who presented with ALI between April 2016 and October 2020 at a single tertiary care center. SES was quantified using individual variables (median household income, level of education, and employment) and a composite endpoint, the neighborhood deprivation index (NDI). The N

Short-term compliance with peak flow monitoring: results from a study of inner city children with asthma.

The objective of the study was to assess the feasibility of initiating daily peak flow monitoring in a research study of asthma in inner city children. We performed a descriptive study of patterns of peak flow monitoring in children randomized to receive a simple mini-Wright (SM) or an electronic recording meter (ERM). The ERM served as a "covert" meter, providing objective documentation of actual peak flow use. Sixty-five Hispanic or African-American children, ages 5-9 years, with a history of physician-diagnosed asthma participated in the study. All children resided in census tracts with 40%

Acute Ocular Hypertension Diminishes Flicker-Induced Enhancement in Retinal Blood Flow and Full-Field Electroretinogram in a Mouse Model.

IMPORTANCE: Family socioeconomic status has been associated with autism spectrum disorder (ASD) diagnoses. Less is known regarding the role of neighborhood disadvantage in the United States, particularly when children have similar access to health insurance. OBJECTIVE: To evaluate the association between neighborhood disadvantage and the diagnosis of ASD and potential effect modification by maternal and child demographic characteristics. DESIGN, SETTING, AND PARTICIPANTS: This cohort study examined a retrospective birth cohort from Kaiser Permanente Southern California (KPSC), an integrated

Neighborhood Characteristics and Elevated Blood Pressure in Older Adults.

IMPORTANCE: The local environment remains an understudied contributor to elevated blood pressure among older adults. Untargeted approaches can identify neighborhood conditions interrelated with racial segregation that drive hypertension disparities. OBJECTIVE: To evaluate independent associations of sociodemographic, economic, and housing neighborhood factors with elevated blood pressure. DESIGN, SETTING, AND PARTICIPANTS: In this cohort study, the sample included Health and Retirement Study participants who had between 1 and 3 sets of biennial sphygmomanometer readings from 2006 to 2014 or

Intersectionality of individual and neighborhood-level adverse social determinants of health in early pregnancy.

INTRODUCTION: Individual- and neighborhood-level social determinants of health (SDOH) have been assessed separately in pregnancy, but their relationship to one another remains uncertain. We investigated the intersectionality of three neighborhood-level SDOH measures with three individual-level SDOH measures. This was done to examine the concomitant experiences of multiple SDOH in pregnancy. METHODS: A secondary analysis of data from the Nulliparous Pregnancy Outcomes Study: Monitoring Mothers-To-Be. We assessed three neighborhood-level SDOH measures using geocoded participant home addresses i

Predictive factors of long-term follow-up attendance in very long-term childhood cancer survivors.

BACKGROUND: Long-term follow-up (LTFU) clinics have been developed but only some childhood cancer survivors (CCS) attend long-term follow-up (LTFU). OBJECTIVE: To identify factors that influence LTFU attendance. METHODS: Five-year CCS treated for a solid tumor or lymphoma in Gustave Roussy before 2000, included in the FCCSS cohort (French Childhood Cancer Survivor Study), aged >18 years and alive at the date of the LTFU Clinic opening (January 2012) were invited to a LTFU visit. Factors associated with attendance at the LTFU clinic between 2012 and 2020 were estimated using logistic regressi

Utilizing Residential History to Examine Heterogeneous Exposure Trajectories: A Latent Class Mixed Modeling Approach Applied to Mesothelioma Patients.

BACKGROUND: Life-course exposure assessment, as opposed to a one-time snapshot assessment based on the address at cancer diagnosis, has become increasingly possible with available cancer patients' residential history data. To demonstrate a novel application of residential history data, we examined the heterogeneous trajectories of the nonasbestos air toxic exposures among mesothelioma patients, and compared the patients' residential locations with the spatiotemporal clusters estimated from the National Air Toxic Assessment (NATA) data. METHODS: Patients' residential histories were obtained by

Typhoid Fever and its association with environmental factors in the Dhaka Metropolitan Area of Bangladesh: a spatial and time-series approach.

Typhoid fever is a major cause of death worldwide with a major part of the disease burden in developing regions such as the Indian sub-continent. Bangladesh is part of this highly endemic region, yet little is known about the spatial and temporal distribution of the disease at a regional scale. This research used a Geographic Information System to explore, spatially and temporally, the prevalence of typhoid in Dhaka Metropolitan Area (DMA) of Bangladesh over the period 2005-9. This paper provides the first study of the spatio-temporal epidemiology of typhoid for this region. The aims of the st

Aging under scarcity and insecurity: chronic disease, functional limitation, and basic need material insecurity among older adults in Haiti.

BACKGROUND: Older adults in low-income and fragile states often age with intersecting burdens of chronic disease, functional decline, and material deprivation, yet they remain largely invisible in research and policy. In Haiti, recurrent political crises, violence, and weak health and social protection systems may amplify late-life inequities, but national data are scarce. We examined the distribution and social determinants of health, functioning, and material insecurity among older Haitian adults. METHODS: We analyzed data from adults aged ≥ 60 years (n = 1,408) drawn from a 2024 nationally

Long term effects of traffic noise on mortality in the city of Barcelona, 2004-2007.

Numerous studies showing statistically significant associations between environmental noise and adverse health effects already exist for short-term (over one day at most) and long-term (over a year or more) noise exposure, both for morbidity and (albeit to a lesser extent) mortality. Recently, several studies have shown this association to be independent from confounders, mainly those of air pollutants. However, what has not been addressed is the problem of misalignment (i.e. the exposure data locations and health outcomes have different spatial locations). Without any explicit control of such

Recent, Nearby Gun Violence and Recurrent Psychiatric Exacerbation in Children and Adolescents.

IMPORTANCE: Gun violence and youth mental illness are urgent public health concerns. Although prior studies have linked community violence with pediatric mental health outcomes, little is known about the connection between exposure to hyperlocal gun violence and the short-term risk of a psychiatric emergency at the individual level. OBJECTIVE: To estimate the impact of recent, nearby gun violence exposure on pediatric recurrent psychiatric exacerbation risk following an index psychiatric hospital admission or emergency department (ED) visit. DESIGN: Retrospective longitudinal cohort study us
