We reviewed the records of 73 patients with primary melanoma of the choroid and ciliary body with metastasis treated at M. D. Anderson Hospital and Tumor Institute between 1973 and 1979. At time of diagnosis of primary melanoma 71 of 73 patients had tumor localized to the eye and were treated with enucleation of the affected eye. The interval from resection of primary tumor to detection of systemic metastases in the 71 patients ranged from one to 201 months (median 43.5 months). Two cases (four eyes) of medullary thyroid carcinoma with metastases in the choroid, ciliary body and iris are described for the first time. In one eye the choroidal metastases were destroyed clinically by photo- and cryocoagulation. In another eye coagulation of a tumor may have facilitated transscleral extension into the orbit. The clinical conclusions arising from the pecularities of these tumors are discussed. It is particularly striking that in 2 patients with this rare tumor metastases developed in the anterior uvea in all four eyes.

The authors describe their experience with ten cases of brain tumors in children in whom the onset of symptoms occurred within the first year of life, but who were operated on when they were 2 months to 4 years of age. Isolated pancreatic metastasis from choroidal melanoma is a rare phenomenon. We describe a case of a 38-year-old woman who underwent enucleation surgery for right choroidal melanoma and subsequently developed lesions in the pancreas 10 years after enucleation, which were depicted on Ga-DOTANOC PET/CT and confirmed to be metastasis from choroidal melanoma on surgical histopathology.

RESULTS: A 55-year-old woman complained of visual field loss and decreasing vision. A 50-year-old Japanese housewife became blind owing to bilateral diffuse malignant melanoma of the choroid, ciliary body, and iris. These were histologically proved when enucleation was required because of blind painful eyes with raised intraocular pressures. She presented with reduced visual acuity and night blindness. Cells of human choroidal melanoma were implanted in the cornea of rabbits. A few days later corneal neovascularization onto the tumor can be observed. When the tumor is vascularized, rapid tumor growth follows. Macroscopic, histologic, and angiographic aspects are presented. The role of prostaglandin E2 (PGE2) as a cause of angiogenesis is discussed.

Plaque brachytherapy is the most common procedure for the treatment of small choroidal melanoma, especially in posteriorly located tumors. However, there is only little information on outcome after treatment with stereotactic radiosurgery, for example, CyberKnife radiosurgery. Gamma Knife was designed by Lars Leksell in the early 1950's. It gave rise to a new discipline of medicine--stereotactic radiosurgery. Main use of Gamma Knife in ophthalmology--discussed in the first part of this paper--is non-invasive treatment of uveal melanoma. AIM: To determine the frequency of visually asymptomatic choroidal metastases in patients with disseminated breast and lung carcinomas in order to establish optimal patient management policies.
