[Chronicity and primary care: the role of prison health].

The Prison Primary Health Care Teams in Catalonia have been integrated into the Catalan Health Institute. This integration shall facilitate¹ training and updating, while eliminating the existing differences between the health services belonging to prison institutions and those of the Catalan Health Service. It shall enable team work and coordination between Primary Health Care Teams in the community and the PHCTs in prisons within the same geographical area by sharing ongoing training, multi-sector work teams and territory-based relations, thereby facilitating continuance in care and complete

Facilitators and barriers to provision of hospital in the home: does geographical location matter? A scoping review.

BackgroundThere is an increasing imperative for hospital services delivered in the home due to overburdened hospital systems, ageing populations and rising prevalence of chronic disease. Hospital in the home services have numerous advantages including reduced mortality, reduced hospital readmission and emergency department presentations and improved patient quality of life. Despite anticipated benefits for rural communities, which typically experience high prevalence of chronic disease and high rates of potentially avoidable hospitalisation, there appears to be limited research that focusses s

Multidisciplinary management of multiple sclerosis symptoms.

BACKGROUND: Effective management of multisymptomatic chronic diseases such as multiple sclerosis (MS) requires a multimodal, interdisciplinary approach. At MS clinics, numerous healthcare specialties are coordinated to provide patients with quality clinical care for all aspects of their disease. Settings and resource availability may vary between countries. Four specific specialty services from different EU countries are examined in more detail. SUMMARY: The multidisciplinary neurorehabilitation team in Rennes, France, provides specialized consultations (e.g. spasticity, urodynamic unit, devi

Exploring depression symptoms in chronic care users in Dr Kenneth Kaunda for culturally relevant counselling.

BACKGROUND: South Africa is faced with a mental health burden attributed to a large treatment gap for common mental disorders (CMDs), and a shortage of mental health professionals. Although comorbidity of CMDs with chronic diseases is common, chronic and non-communicable diseases may receive more attention than CMDs highlighting the need for contextually appropriate, culturally relevant counselling to increase access to mental healthcare for CMDs at primary health care (PHC). AIM: To explore the experiences of patients with comorbid chronic medical conditions and depression attending PHC, to

A lifecycle governance and learning health system framework for trustworthy, generalizable, and sustainable human-ai partnership in clinical practice: Lessons from the asthma-guidance and prediction system (A-GPS).

The integration of artificial intelligence (AI) into clinical decision support (CDS) holds promise for proactive, personalized, and precision care. However, current understanding of how to establish trustworthy human-AI partnerships is in its infancy, despite its critical importance for implementing AI in healthcare. We present the Asthma-Guidance and Prediction System (A-GPS) as a case study of a practice-integrated AI platform that demonstrates how trustworthy, generalizable and sustainable AI can be developed, evaluated, and implemented in real-world asthma care. We describe major challenge

Evaluating hospital readmission rates in dialysis facilities; adjusting for hospital effects.

Motivated by the national evaluation of readmission rates among kidney dialysis facilities in the United States, we evaluate the impact of including discharging hospitals on the estimation of facility-level standardized readmission ratios (SRRs). The estimation of SRRs consists of two steps. First, we model the dependence of readmission events on facilities and patient-level characteristics, with or without an adjustment for discharging hospitals. Second, using results from the models, standardization is achieved by computing the ratio of the number of observed events to the number of expected

Predicting Laboratory Test Ordering in Emergency Departments Using Integrated Structured and Unstructured Electronic Health Records: Machine Learning Study.

BACKGROUND: Laboratory testing is a cornerstone of diagnostic decision-making in emergency departments (EDs), yet its overuse contributes substantially to unnecessary health care costs and inefficiencies. Predictive approaches that leverage electronic health record data may help optimize and guide more appropriate test use. OBJECTIVE: This study aims to develop and evaluate machine learning models that predict laboratory test use during ED visits by integrating structured clinical data and unstructured text from electronic health records. METHODS: We analyzed 13,115 adult ED visits from the

The development of health literacy in patients with a long-term health condition: the health literacy pathway model.

BACKGROUND: Inadequate health literacy has been associated with poor management of long-term health conditions and has been identified as a key social determinant of health outcomes. However, little is understood about how health literacy might develop over time or the processes by which people may become more health literate. Our objectives were to describe how patients with a long-term condition practice health literacy in the management of their health and communication with health professionals, how they become more health literate over time and their experience of using health services. Development of GuaB Inhibitors to Treat Mycobacterium tuberculosis Infection.

Mycobacterium tuberculosis (Mtb) is responsible for more deaths per year than any other infectious disease. Resistance of circulating Mtb to the standard of care treatment regimen is an urgent global health crisis for which novel antibiotic treatments are needed. We have developed extremely potent, orally bioavailable, small-molecule inhibitors of inosine 5'-monophosphate dehydrogenase (GuaB) that catalyze the rate-limiting step in de novo guanine biosynthesis. The inhibitors exhibited bactericidal activity against Mtb in extracellular, intracellular, and hypoxic environments that capture feat

Role of the School Nurse in Providing School Health Services: Policy Statement.

School nursing has evolved into an essential field bridging health care and education. School nurses play multifaceted roles, addressing direct care, care coordination, public health, leadership, and quality improvement. They manage communicable diseases, chronic conditions, mental health, and emergencies while promoting safe, equitable learning environments. Collaboration with pediatricians enhances outcomes for students, families, and communities by addressing shared challenges like the rising prevalence of children and youth with special health care needs (CYSHCN), mental health concerns, a

Visit Entropy Associated with Diabetic Control Outcomes.

BACKGROUND: Chronic diseases such as type 2 diabetes place a large burden on the health care system and are associated with increased morbidity and mortality. A team-based multidisciplinary approach that organizes care to improve chronic disease management may actually decrease traditional continuity of care metrics. Visit entropy (VE) provides a novel measure of care organization produced by team-based approaches. Higher VE, reflecting more disorganized care, has been associated with more hospital readmissions. We hypothesized that higher VE was also associated with reduced adherence to the D

Improving support and education of low-income baby boomers diagnosed with chronic hepatitis C virus infection through universal screening.

AIMS AND OBJECTIVES: To identify support needs of low-income baby boomers recently diagnosed with chronic hepatitis C virus infection. BACKGROUND: The U.S. Preventive Services Task Force has endorsed one-time screening of all baby boomers (born 1945-1965) for hepatitis C because 75% of the estimated 2-3 million persons with chronic infection are in this age range. We hypothesised that persons diagnosed by routine screening would have significant psycho-emotional, cognitive and healthcare challenges that need to be met by collaborative care and services from nurses and other healthcare personn

Palliative care: Seduced by wishful thinking or on safe ground?

Global advocacy for early and integrated palliative care (PC) in oncology has intensified over the past two decades, frequently positioning PC as both a clinical imperative and a human right. However, the empirical foundation supporting these claims remains surprisingly fragile relative to the strength of the associated policy rhetoric. This analytic paper critically examines the disjunction between expanding normative expectations and the limited, context-bound evidence base underpinning early PC integration in contemporary cancer care. We argue that prevailing narratives risk overstating the
