Thoracoscopic Thoracic Duct Ligation for Congenital and Acquired Disease.

PURPOSE: Congenital and acquired chylothorax presents a unique management challenge in neonates and infants. A failure of conservative therapy requires surgical ligation to prevent continued fluid and protein losses. This article examines a 15-year experience with thoracoscopic ligation of the thoracic duct. PATIENTS AND METHODS: From June 1999 to December 2013, 21 patients presented with chylothorax refractory to conservative management. Sixteen patients presented following cardiac procedures, 1 after tracheoesophageal fistula repair, 1 after extracorporeal membrane oxygenation cannulation, …

Central Venous Catheter-Related Extravasation Due to Delayed Vascular Erosion.

Central venous catheters (CVCs) are crucial in critical care but may occasionally lead to rare, serious complications such as vascular erosion and extravasation of infused solutions. Leakage of total parenteral nutrition (TPN) into the pleural space can mimic chylothorax, potentially delaying correct diagnosis and appropriate treatment. An 83-year-old male with COVID-19-related acute respiratory distress syndrome (ARDS) received TPN via a left-sided CVC, which remained in a suboptimal position for 18 days. On day 18, he developed sudden respiratory deterioration associated with bilateral milky…

Gorham-Stout disease: a textbook presentation of a rare disease in Pakistan.

Gorham-Stout disease is an exceptionally rare disease which is characterised by massive osteolysis of the bone, oedema, and in severe cases pleural effusion and chylothorax. Its aetiopathology is unknown, and no specific treatment has been modulated thus far. We report the case of a 17-year-old male with osteolysis in the bones of his entire left arm and persistent chylothorax. Due to the late presentation and patient's desire for a better quality of life, amputation was the only choice left for treatment. This case was evaluated and treated at the Orthopaedic Surgery and Trauma department of …

[Mortality in patients with osteolysis of lymphatic origin: a review of the experience with 54 patients and the literature].

INTRODUCTION: Osteolysis of lymphatic origin is a rare disease with a high mortality which is difficult to attribute whether it is due to th … syndrome. All of them suffered severe malnutrition from massive lymphatic loss and 2 had bilateral chylothorax. Among the 51 cases reviewed in the literature between 1954 - 2010, 19 had bilateral chylothorax, 15 had right side involvement and only 2 had an exclusive left chylothorax. Eleven patients had complications, such as respiratory distress and pneumon …

Retrograde thoracic duct embolization in an idiopathic case of chylopericardium.

This is a novel case of idiopathic chylopericardium and chylothorax in a young male who had no significant medical history. He first presented with dyspnea due to idiopathic chylopericardium, which was refractory to medical and surgical treatments, including a medium-chain triglyceride diet, octreotide, and video-assisted pericardial window. The chylopericardium persisted and progressed to concomitant left-sided chylothorax. He subsequently underwent multiple imaging studies, including lymphoscintigraphy and an intranodal lymphangiogram, both of which confirmed leakage in the thoracic duct. Fi…

[Indications of rethoracotomy after esophagectomy for esophageal cancer].

Indications of rethoracotomy after esophagectomy for esophageal cancer are reviewed in this paper. Hemothorax, pneumothorax, pyothorax and chylothorax are the main causes of rethoracotomy. Complications indicating rethoracotomy are summarized as follows: 1) Hemothorax; emergency rethoracotomy is indicated in cases of bleeding through the chest drain over 100ml/hr, which is continuing over 5 hours or in cases when normal blood pressure cannot be maintained without blood transfusion. In many cases the bleeding point is the chest wall, from the branches of the intercostal artery. 2) Pneumothorax;…

Nutrition-related risk factors for prolonged pleural effusion after congenital heart surgery in Chinese infants.

BACKGROUND AND OBJECTIVES: Previous studies on the risk factors for prolonged pleural effusion (PPE) have primarily focused on surgical-related risk factors, with little research exploring the influence of nutritional factors on this delay. This study aimed to identify the nutritional risk factors for PPE in Chinese infants following congenital heart disease (CHD) surgery. METHODS AND STUDY DESIGN: We retrospectively reviewed the medical records of patients under 3 years old with chylothorax following CHD surgery from 2016 to 2020. PPE was defined as pleural effusion lasting over 14 days. Log…

Thoracoscopic Assisted PleuralPortTM Application in Seven Dogs Affected by Chronic Pleural Effusion.

Chronic non-septic pleural effusion is a condition that frequently may occur because of lung or pleural neoplasia, or chylothorax refractory to surgical treatment, in dogs. Effusion management can be performed with multiple pleurocenteses or the application of chest drains. New modified vascular devices have been used for patients with chronic diseases; they offer the advantage of allowing home management and do not require hospitalization. Eight PleuralPortTM devices were applied in seven dogs during thoracoscopic exploration and biopsy procedures; five were affected by mesothelioma; one by l…

Drugs in Focus: Octreotide Use in Children With Gastrointestinal Disorders.

Octreotide, a somatostatin analogue, has been used for more than 20 years in children with gastrointestinal bleeding, chylothorax or chylous ascites, intestinal lymphangiectasia, pancreatitis, intestinal dysmotility, and severe diarrhoea; however, until now, there is a lack of randomised clinical trials evaluating the efficacy of this compound in childhood. Hence, we aimed to review the literature in order to determine the evidence of its use and safety in children, using PubMed from 2000 to 2021 with the search terms "octreotide" and "children" and "bleeding or chylous ascites or chylothorax …

Additional Octreotide Therapy to Sirolimus Achieved a Decrease in Sirolimus-refractory Chylous Effusion Complicated with Lymphangioleiomyomatosis.

Recently, sirolimus, an inhibitor of mammalian target of rapamycin, was reported to decrease chylous effusion in patients with lymphangioleimyomatosis (LAM). We herein report a case of a 34-year-old woman with LAM who developed refractory chylothorax and chylous ascites during sirolimus therapy. In this case, to reduce chylous effusion, we administered octreotide, which is often used to control postoperative chylous effusion, in addition to the sirolimus therapy. This combination therapy reduced the chylothorax and chylous ascites. For patients with LAM, octreotide therapy in addition to sirol…

A case of a traumatic chyle leak following an acute thoracic spine injury: successful resolution with strict dietary manipulation.

BACKGROUND: Chylothorax is a rare form of pleural effusion that can be associated with both traumatic and non-traumatic causes. Thoracic duct ligation is often the treatment of choice in postsurgical patients; however the optimal treatment of this disease process after traumatic injury remains unclear 1. We present a rare case of a thoracic duct injury secondary to a blunt thoracic spine fracture and subluxation which was successfully treated non-operatively. CASE PRESENTATION: A 51 year old male presented as a tier one trauma code due to an automobile versus bicycle collision. His examinatio…

Severe venous and lymphatic obstruction after single-chamber pacemaker implantation in a patient with chest radiation therapy.

A 73-year-old woman with a history of paroxysmal atrial fibrillation, sinus node dysfunction, bilateral breast cancer, and extensive chest radiation developed progressive edema, dyspnea, and recurrent pleural effusions soon after single-chamber pacemaker implantation. Thoracentesis yielded a diagnosis of chylothorax, and progressive refractory anasarca developed. A computed tomography angiogram suggested obstruction of the superior vena cava and left subclavian vein despite outpatient therapeutic anticoagulation. Autopsy confirmed venous thrombosis, along with mediastinal fibrosis. The presume…

EBUS/EUS-guided lymph nodal gadolinium injection and MRI of the chest in diagnosis of abnormal lymphatic drainage. A case series.

Impaired lymphatic circulation in the mediastinum may result from congenital disorders, leakage secondary to trauma, or infiltration by neoplasia. It manifests as chylothorax, plastic bronchitis, and/or alveolar fatty pneumonitis. Lymphography, involving the injection of contrast medium into peripheral lymph nodes, was previously utilised but posed challenges in execution and image interpretation. We describe two patients admitted to our department with relapsing chylothorax and respiratory failure associated with "fatty" pneumonitis and plastic bronchitis. The final diagnosis was confirmed by…

Nonimmune hydrops fetalis due to generalized lymphatic dysplasia in an infant with Robertsonian trisomy 21.

We report the first case of generalized lymphatic dysplasia and trisomy 21 presenting with nonimmune hydrops fetalis. This infant showed intractable chylothorax, chylous ascites, and periodic bouts of edema. A karyotype analysis revealed Robertsonian trisomy 21: 46,XY,t(14q21q)(q10;q10) +21. This patient died of multiple organ failure at 400 days of life, despite the management of chylous effusions. The lymphoscintigraphy and histopathological findings led to the final diagnosis of generalized lymphatic dysplasia, which might also contribute to the development of hydrops. Refractory chylothora…

Retrograde transvenous thoracic duct embolization for lymphatic leakage after retroperitoneal tumor and lymph node resection: a case report and literature review.

BACKGROUND: Postoperative lymphatic leakage is a complication of ineffective conservative treatment for retroperitoneal mass. Herein, we rep … orta; the latter was reconstructed using a prosthetic graft. Postoperatively, the patient developed chylothorax. No improvement was observed after conservative treatment that included fasting and somatostatin therapy. The leakage site could not be identified using antegrade lymphangiography of the bilateral inguinal lymph nodes, but was detected using retrogr …

A systematic review and meta-analysis of surgical morbidity of primary versus patch repaired congenital diaphragmatic hernia patients.

Large studies comparing the surgical outcome of primary versus patch repair in congenital diaphragmatic hernia (CDH) patients are rare. This study aims to evaluate the incidence of surgical complications in both types of CDH repair. PubMed, EMBASE, Cochrane and Web of Science were searched for peer-reviewed articles. Studies on CDH between 1991 and August 2020 were systematically screened and meta-analyses were performed. Primary outcomes of this review were: haemorrhage, chylothorax, recurrences and small bowel obstruction (SBO). A total of 6436 abstracts were screened, after which 25 publica…

Lipid pleural effusions.

Traditionally, a lipid pleural effusion has been described as milky or turbid appearing. However, lipid effusions may have varied presentations making a diagnosis by appearance problematic. Distinguishing between a chylothorax and a cholesterol effusion, the 2 types of lipid effusions, is essential. A chylothorax develops after injury or obstruction of the thoracic duct, leading to a chyle leak into the pleural space that is characterized by an increased triglyceride concentration and the presence of chylomicrons. In contrast, a cholesterol effusion is a long-standing effusion associated with …

Complications following airway surgery in Noonan syndrome.

Two children with Noonan syndrome underwent airway surgery and both experienced spontaneous chylothorax after surgery. The Noonan syndrome phenotype may include defects that significantly increase the risk of complications associated with surgery. Manifestations of Noonan syndrome may include congenital heart disease; coagulation factor deficiency; pterygium colli; and lymphangiomatosis of the pleura, lungs, and chest wall that can lead to life-threatening chylothorax. In this article, the 2 cases are presented and the relevant literature is reviewed to increase awareness of the potential prob…
