Surgical treatment is usually indicated for the management of Neer type IIB fractures of the distal third of the clavicle. These unstable injuries have shown a rate of nonunion that oscillates around 30% to 45% when managed conservatively, and surgical strategies often require a second operation for implant removal. We describe an arthroscopic-assisted technique for the treatment of Neer type IIB unstable distal-third clavicle fractures that overcomes the issues related to open surgery, metal hardware, and implant irritation. This technique increases the load to failure of the construct by mea…

BACKGROUND: Clavicle fractures are common, accounting for 5-12 % of all fractures. Traditionally, displaced middle-third clavicle fractures have been managed non-operatively but the associated displacement often leads to mal-union with shortening, cosmetic deformity and occasionally non-union, with clinicians looking towards alternative operative methods such as intramedullary nailing (IMN). However, such methods have their own complications. In order to ascertain the effectiveness of IMN in the management of middle-third clavicle fractures compared with non-operative treatment, analysis of re…

PURPOSE: Locking plate osteosynthesis has become the preferred method for operative treatment of clavicle fractures. The method offers stable fixation, and would theoretically be associated with a low rate of fracture-related complications and reoperations. However, this remains to be explored in a large cohort, and our purpose was to assess the overall rates of complications and reoperations following locking plate osteosynthesis of mid-shaft clavicle fractures.
METHODS: We identified all locking plate osteosynthesis of mid-shaft clavicle fractures operated upon in our department from Januar…

BACKGROUND: The clavicle hook plate achieves like most other operative techniques, a high percentage of union and a low percentage of complications however concerns about long term complications still exist, particularly the involvement of the acromioclavicular joint.
METHODS: To evaluate the results and long term effects in use of this plate we performed a retrospective analysis with a mean follow up of 65 months (5.4 years) of 28 consecutive patients with acute displaced lateral clavicle fractures, treated with the clavicle hook plate.
RESULTS: Short term functional results in all patients…

PURPOSE: The incidence of operative treatment of dislocated midshaft clavicle fractures (DMCF) is rising due to unsatisfactory results after non-operative treatment. Knowledge of complications is important for selection of the surgical technique and preoperative patient counselling. The aim of this study is to compare complications after plate fixation and elastic stable intramedullary nailing (ESIN) with a titanium elastic nail (TEN) for DMCF.
METHODS: A retrospective analysis of our surgical database was performed. From January 2005 to January 2010, 90 patients with DMCF were treated with p…

BACKGROUND: Recent literature supports surgical intervention for shortened, displaced, mid-shaft clavicle fractures. We present the results of a randomized clinical trial comparing locked intramedullary fixation and plate fixation for short, displaced, mid-shaft clavicle fractures.
MATERIALS AND METHODS: Local ethical approval was obtained and power analysis and sample size calculations were performed prior to commencement. Patients randomized to 2 groups to be treated with either locked intramedullary fixation or plating. Patients regularly followed up to clinical and radiographic union. The…

We evaluated the incidence and clinical significance of postfracture shortening of the clavicle in 85 patients. There were 71 mid-clavicular fractures and 14 of the lateral end of the clavicle. 46 fractures were primarily undisplaced and 39 displaced. All fractures were nonoperatively treated with sling immobilization. All patients were reexamined 5 years after the fracture. 35 clavicles had healed with at least 5 mm shortening. Clavicles with originally displaced fractures were shorter and radiologically more frequently malunited. Mobility, strength and the functional Constant score were simi…

Clavicle fractures are a common orthopedic issue requiring a comprehensive approach to management. When surgery is indicated, postoperative concerns and discussions based on psychosocial measures should take place to help patients obtain the best individualized outcomes. Traditional indications for hardware removal include infection, mechanical failure, or significant pain, but patients can elect to undergo removal for non-clinical purposes such as hardware visibility, clothing interference, or activity limitations. These patient-centered indications for removal are currently underreported in …

INTRODUCTION: The clavipectoral fascial plane block (CPB) can be used in clinical situations where general anesthesia (GA) is contraindicated. CPB has been reported to provide a better hemodynamic profile and lower complications compared to GA. This study aimed to evaluate the patient satisfaction, radiological and functional outcomes of CPB in the surgical management of clavicle fractures.
MATERIALS AND METHODS: A single-arm prospective cohort study was conducted between November 2022 and July 2024. All patients with clavicle fractures meeting the inclusion criteria were operated on with CPB…

BACKGROUND: Glenohumeral internal rotation deficit (GIRD) is a well-recognized component of shoulder dysfunction and has been extensively described in overhead athletes within the framework of scapular dyskinesis. However, whether a similar or distinct pattern of GIRD exists after mid-shaft clavicle fractures remains insufficiently understood. This study aimed to evaluate the prevalence and features of GIRD in a post-traumatic population and to explore their potential relationship with scapular positioning. METHODS: This cross-sectional comparative study included 99 patients with healed mid-sh…

BACKGROUNDS: Clavicle fractures are a common injury, with lesions of the middle third accounting for 80% of all clavicle fractures. The most suitable approach to address these fractures remains debated, especially at long term. The aim of this study was to compare surgical and conservative treatments of displaced midshaft clavicle fractures at long-term follow-up.
MATERIALS AND METHODS: A total of 123 patients (≥18 years) with a displaced midshaft clavicle fracture, treated surgically with plate fixation (68 patients) or conservatively (55 patients) and evaluated at a mean follow-up 6.7±2.6 y…

BACKGROUND: Clavicle fractures are common injuries, with proximal clavicle fractures being relatively rare. Previous studies have suggested that proximal clavicle fractures are often associated with high-energy trauma and carry a high risk of complications. However, no studies have comprehensively described their clinical characteristics and severity in Japan. This study aimed to evaluate the clinical characteristics of proximal clavicle fractures and their association with injury severity compared to non-proximal clavicle fractures in Japan.
METHODS: This retrospective cohort study was condu…

Displaced midshaft clavicle fractures in adults have traditionally been managed non-operatively; however, recent literature has challenged this approach, particularly in active populations. This systematic review aims to compare functional outcomes, union rates, complication profiles, and patient satisfaction between operative and non-operative management strategies. A comprehensive search of PubMed, Embase, and the Cochrane Library was conducted in February 2025, identifying 10 eligible studies published from 2015 onwards: eight randomized controlled trials and two prospective cohort studies.…

Thoracic outlet syndrome (TOS) is a rare but significant complication that may occur following clavicular fractures, especially after surgical fixation. We present the case of a 32-year-old woman who developed partial thrombosis of the subclavian and axillary veins as a delayed complication after open reduction and internal fixation (ORIF) of a clavicle fracture with an anatomical plate. Approximately 1 month after surgery, the patient began experiencing pain in the axillary region and mid-arm, symptoms that raised suspicion for TOS. Doppler ultrasonography confirmed partial thrombosis of the …

INTRODUCTION: Lateral clavicle fractures often need to be stabilized by a plate osteosynthesis and due to the acting high forces a cut-out of the screws of the lateral fracture fragment can occur. New plates enable to place anterior screws in addition to the screws placed from the top of the clavicle. This experimental in-vitro study will determine whether they have a substantial biomechanical effect.
MATERIALS AND METHODS: In synthetic bones, lateral clavicle fractures were created and stabilized in 4 different groups: In Group A, a lateral clavicle plate was fixed with 3 screws in the later…

INTRODUCTION: Displaced lateral end clavicle fractures are challenging injuries with a high risk of non-union and functional impairment when treated conservatively. Open reduction and endobutton fixation is a novel surgical technique that aims to provide stable fixation while preserving shoulder biomechanics, potentially improving functional and radiological outcomes compared to traditional methods.
MATERIALS AND METHODS: This prospective study included 20 male patients (mean age: 35.2 years, range: 18-56) with displaced lateral clavicle fractures (Neer type IIB and type V). All patients unde…

BACKGROUND: Clavicle fractures account for over one-third of shoulder injuries and up to 3.3% of all fractures in adults. While the majority of these fractures can be managed non-surgically, there are instances where surgical intervention is performed. Regional anesthesia (RA) can be a preferred alternative to general anesthesia (GA) to avoid complications and high costs in this surgery. Moreover, the identification of the most optimal approach for RA remains challenging.
OBJECTIVES: This study aimed to compare the efficacy of interscalene block (ISB) with and without superficial cervical ple…

BACKGROUND: A recent database study found that 15.2% of clavicle fractures underwent surgical treatment. Recent evidence accentuates the role of smoking in predicting nonunion. The purpose of this study was to further elucidate the effect of smoking on the 30-day postoperative outcomes after surgical treatment of clavicle fractures.
METHODS: The authors queried the American College of Surgeons National Surgical Quality Improvement Program database for all patients who underwent open reduction and internal fixation of clavicle fracture between 2015 and 2020. Multivariate logistic regression, a…
