Premature ovarian insufficiency: addressing the most controversial issues.

It was a true honor to be invited as President to deliver the Pieter Van Keep oration at the 2024 International Menopause Society (IMS) Congress in Melbourne, Australia. There was never any doubt that the topic of my lecture would be premature ovarian insufficiency (POI). POI is a condition which has been neglected for far too long given its greater global prevalence than originally estimated and its implications to the sufferers, both in terms of quality of life and long-term well-being. The hope is that we are now entering a new era of awareness given the recent update of the European Societ

The impact of ovarian aging on muscle strength and life quality in various reproductive aging stages.

OBJECTIVE: Ovarian aging significantly impacts women's overall aging, affecting various systems including the musculoskeletal system. This study investigates the correlation between ovarian function and handgrip strength (HGS) across reproductive aging stages and their relation to health-related quality of life (HRQoL). METHOD: A cross-sectional study was conducted with female participants spanning all stages of ovarian function. HGS was measured using a dynamometer, and ovarian function was assessed via hormone levels (estradiol [E2], follicle stimulating hormone [FSH], luteinizing hormone [

Follicle stimulating hormone, luteinizing hormone and adiposity trajectories in postmenopausal women.

OBJECTIVE: Postmenopause is characterized by changes in reproductive hormones and body composition. Preclinical evidence suggests that follicle stimulating hormone (FSH) may increase adiposity, but epidemiologic research is limited. This study examined whether postmenopausal FSH and luteinizing hormone (LH) are related to adiposity changes. METHOD: The sample included 675 postmenopausal women enrolled in the Women's Health Initiative (WHI) Buffalo Osteoporosis and Periodontal Disease (OsteoPerio) study with dual-energy X-ray absorptiometry measures. Adiposity measures included visceral adipos

Osteo-sarcopenic obesity in midlife and older women: a current worldwide public health challenge.

Sarcopenic obesity, defined as the coexistence of excess adiposity and sarcopenia, represents a high-risk clinical condition that amplifies the adverse effects of each disorder. When accompanied by bone loss, the entity progresses to osteo-sarcopenic obesity (OSO), which further compromises physical function, metabolic health and overall prognosis. Affected individuals face an increased risk of falls, fractures, functional disability, hospitalization, cardiometabolic complications and premature mortality. In women transitioning through menopause, declining estrogen levels accelerate fat accumu

Impact of vaginal estriol on serum hormone levels: a systematic review.

The genitourinary syndrome of menopause (GSM) affects up to 84% of postmenopausal women and may significantly reduce the quality of life in some. For symptom relief, there are several non-hormonal and hormonal vaginal products available. In Europe, vaginal estriol (E3) is the most frequently chosen estrogen for GSM treatment. The aim of this systematic review was to assess the impact of vaginal E3 on serum sex hormone levels, an outcome that has been previously used to assess safety in similar products. In our review, we did not find any alterations in serum estrone, estradiol, testosterone, p

Vaginal dryness: a review of current understanding and management strategies.

The issue of vaginal dryness in genitourinary syndrome of menopause (GSM) and its pervasive impact on women's quality of life is often overlooked. Extensive surveys conducted worldwide reveal limited understanding of vaginal dryness among public and health-care providers. Physician knowledge on menopause medicine varies globally, highlighting the need for standardized training. Effective communication between physicians and patients plays a crucial role in diagnosing and treating GSM symptoms. There are multiple treatment options to improve vaginal lubrication, including hormonal and non-hormo

Cardiovascular risk assessment in women: which women are suited for menopausal hormone therapy?

Individual risk assessment for atherosclerotic cardiovascular disease is important for safe menopausal hormone prescription. Besides the traditional risk factors, female-specific risk variables related to pregnancy and gynecologic conditions importantly contribute to a more tailored risk assessment in women at middle age. Of these, prior pre-eclampsia/HELLP (hemolysis, elevated liver enzymes and low platelets) syndrome and early spontaneous menopause (<40 years) seem to be the strongest adverse risk variables. Concomitant inflammatory disorders should also be taken into account. Adding a coron

Pregnancy-associated risk factors for future cardiovascular disease - early prevention strategies warranted.

We summarize convincing evidence that future cardiovascular disease (CVD) risk increases one-fold to four-fold for women with a history of pregnancy complicated by hypertensive disorders, gestational diabetes, fetal growth restriction, placental abruption and preterm birth. A concomitant occurrence of two or more complications in the same pregnancy further potentiates the risk. These women should be informed of their future CVD risks during the postpartum check-up taking place after delivery, and also, if needed, treated, for example, for persisting high blood pressure. In these women with hig

POESIT recommendations on management of body-identical hormones in menopausal symptoms.

Many women seek treatment to improve menopausal vasomotor symptoms (VMS). The selection of women most likely to benefit from menopause hormone therapy (MHT) is crucial in clinical practice. There is general agreement that women younger than 60 years or who initiate MHT within the first 10 years of menopause, with no contraindications, have greater benefits considering symptomatic relief and additional advantages. This group may have the advantage of protection from osteoporosis and from other chronic diseases that affect postmenopausal women, namely cardiovascular disease (CVD). Cumulating evi
