With the growth of spinal surgery, in terms of numbers of operations, as well as the techniques and implants used, surgeons and spine physicians are turning more to imaging for clinical management decisions and surgical planning for spinal disorders. Medicolegal implications also make it mandatory in all cases. We discuss the importance of imaging in modern spinal surgical practice, with an emphasis on decision making in clinics, preoperative surgical work-up, and operative use, as well as in a postoperative setting when complications occur. The increasing use of endoscopic techniques has made

A key part of neurosurgery is decision-making; not only by the neurosurgeons but also by the patients. The patient needs to decide: shall I do the operation? Now or later? With which doctor? At which medical center? In which country? But what if the exact condition that needs an operation causes you problems with making decisions, such as a frontal lobe lesion? Your decision-making capacity may need to go to others [1]. But what if those others may not act in your best interest? What if your decisions carry weight not just for you, but for someone else who depends on you, e.g. as a caregiver,

BACKGROUND: There is an increased tendency towards adopting minimally invasive interventions in dentistry, supported by advancement in materials and techniques. However, the decision-making process in choosing conservative or invasive treatments is influenced by several factors, particularly in permanent teeth with irreversible pulpitis.
OBJECTIVES: The objective of the study was to systematically review the literature regarding factors that influence decision-making for vital pulp therapy (VPT) as definitive treatment option in permanent mature teeth diagnosed with irreversible pulpitis.

Background: Congenital cardiac care involves multiple stakeholders including patients and their families, surgeons, cardiologists, anaesthetists, the wider multidisciplinary team, healthcare providers, and manufacturers, all of whom are involved in the decision-making process to some degree. Game theory utilises human behaviour to address the dynamics involved in a decision and what the best payoff is depending on the decision of other players. Aim: By presenting these interactions as a strategic game, this paper aims to provide a descriptive analysis on the utility and effectiveness of game t

In most patients with suspected or confirmed pituitary adenomas (PAs), MRI, performed using T1- (with or without gadolinium enhancement) and T2-weighted sequences, provides sufficient information to guide effective clinical decision making. In other patients, additional MR sequences (e.g., gradient recalled echo, fluid-attenuation inversion recovery, MR elastography, or MR angiography) may be deployed to improve adenoma detection, assess tumoral consistency, or aid distinction from other sellar/parasellar lesions (e.g., aneurysm, meningioma). However, there remains a small but important subgro

The Ex-Utero Intrapartum Treatment (EXIT) is a method of delivery utilized in the setting of fetal anomalies that can lead to respiratory or cardiovascular compromise with the transition to extrauterine life. With multidisciplinary collaboration, delivery occurs with uterine relaxation to preserve placental function, allowing for appropriate intervention while the maternofetal interface is maintained. Multiple types of EXIT procedures are described in the literature that differ based on fetal indication and specific clinical goals. These include EXIT-to-airway, EXIT-to-resection, EXIT-to-extra

Hallux valgus deformity is the most common surgical indication in the foot. The achievable result is highly dependent on preoperative planning and the choice of surgical procedure. Consideration of anatomic specifications, angulation at the MTP and intermetatarsal 1/2 (IMT), and joint status at the metatarsophalangeal and TMT joints provide the basis for this. Minor, moderate, and severe deformities can be assigned to different osteotomy heights and, within these, a surgical procedure. Each method, if applied appropriately, is suitable and its correct use allows simple operations with maximum

OBJECTIVES: This study aimed to explore the design and outcomes of preference-based value clarification methods (Pb-VCMs) used in the context of supporting patients' clinical decision making to understand whether guidance is used in Pb-VCM development and which Pb-VCM is most effective.
METHODS: In April of 2023, PubMed, Scopus, and Web of Science were searched for studies reporting the development and/or evaluation of Pb-VCMs without restrictions on publication date. Two reviewers independently extracted data including context and clinical decisions, study methods, sample, Pb-VCM design and

This prospective multinational substudy of the AMESI project provides critical insights into managing acute SMA occlusion. By comparing endovascular versus surgical revascularization, the authors demonstrate that baseline illness severity, reflected by elevated lactate levels and mechanical ventilation requirements, dominates prognostic outcomes, with adjusted analyses confirming no independent mortality effect from treatment modality (surgery OR 1.59, 95% CI 0.57-4.37). Notably, unadjusted mortality rates varied substantially (endovascular-effective: 2.9% vs. surgical: 45.8%), primarily attri

High-quality decision making in radiation oncology requires the careful consideration of multiple factors. In addition to the evidence-based indications for curative or palliative radiotherapy, this article explores how, in routine clinical practice, we also need to account for many other factors when making high-quality decisions. Foremost are patient-related factors, including preference, and the complex interplay between age, frailty and comorbidities, especially with an ageing cancer population. Whilst clinical practice guidelines inform our decisions, we need to account for their applicab

Decisions to initiate long-term ventilation (LTV) in children with severe neurologic impairment have recently been subject to candidacy determinations by home ventilation teams that exclude patients based on their neurologic status alone. Determinations of whether decisions are inappropriate require careful analysis of specific clinical circumstances and attention to the family's values. In this Ethics Rounds, we present a case of a previously healthy child who sustained an acute severe anoxic brain injury and was assessed by the medical team to have a high likelihood of remaining minimally co

In this commentary paper, we discuss the use of the electrocardiogram to help clinicians make diagnostic and patient referral decisions in acute care settings. The paper discusses the factors that are likely to contribute to the variability and noise in the clinical decision making process for catheterization lab activation. These factors include the variable competence in reading ECGs, the intra/inter rater reliability, the lack of standard ECG training, the various ECG machine and filter settings, cognitive biases (such as automation bias which is the tendency to agree with the computer-aide

BACKGROUND: Shared decision-making (SDM) is an approach to clinical decision-making that includes patients' values and preferences during health-related decisions. Previous research suggests that SDM may be beneficial in the treatment of substance use disorders; however, the impact of SDM in the treatment of opioid use disorder (OUD) remains unclear.
OBJECTIVES: To identify relevant peer-reviewed literature related to SDM in the treatment of adults with OUD, and to summarize the main findings according to patient outcomes.
METHODS: The research team conducted a scoping review. The team searc…

BACKGROUND: Youth with chronic illnesses and their parents make complex medical decisions and also need to develop medical decision-making skills for transition of care to adult care. The use of inclusive (e.g. 'we decided …') and exclusive (e.g. 'they decided …') language in youth and parents' medical decision-making experiences provides insights into perceptions of engagement. This study assessed youth and parents' perceptions of engagement in medical decision-making about biologic therapy.

BACKGROUND: Orthodontics is a common treatment for malocclusion and is essential for improving the oral health and aesthetics of patients. Currently, patients often rely on the clinical expertise and professional knowledge of doctors to select orthodontic programs. However, they lack their own objective and systematic evaluation methods to quantitatively compare different programs. Therefore, there is a need for a more comprehensive and quantitative approach to selecting orthodontic treatment plans, aiming to enhance their scientific validity and effectiveness.

INTRODUCTION: Although various methods exist for osteoporosis prevention, most patients fail to receive optimal treatment due to information asymmetry between physicians and patients, as well as limited consultation time. Current literatures suggest that decision aids can support clinical decision-making by improving patients' risk perception and treatment acceptance.
OBJECTIVE: This scoping review described the use and effectiveness of decision aids in clinical decision-making among individuals with osteoporosis.

INTRODUCTION AND HYPOTHESIS: Women undergoing surgery for apical pelvic organ prolapse have several medically sound options for specific surgical approaches.
METHODS: We review the principles of shared decision-making as they pertain to surgery for prolapse. We review the literature supporting the superior sacrocolpopexy as a durable treatment for prolapse and the factors that may differentiate it from other repairs in risk and benefit.
RESULTS: We emphasize the importance of collaboration between patients and surgeons in surgical decision-making.
CONCLUSION: All medically appropriate patie

Therapeutic inertia is the result of a complex interaction between multiple factors. In the recent years, some authors suggested that the burden of responsibility for therapeutic inertia is shared, with 50% due to physician-related factors, 30% to patient-related factors, and the remaining 20% to healthcare system barriers. This view is misleading because it fails to capture the deep interconnection between the various elements and how they feed into each other in a vicious circle that influences therapeutic action. The physician's decision-making is influenced by cognitive biases, mental shor
