Multiple-dose activated charcoal: a review of relevant clinical studies.

Although many studies in animals and volunteers have demonstrated that multiple-dose activated charcoal increases drug elimination significantly, this therapy has not been shown in a controlled study in poisoned patients to reduce morbidity and mortality. Further clinical studies are required to establish its role and the optimum dosage regimen of charcoal to be administered. Based on current evidence, multiple-dose activated charcoal should only be considered if a patient has ingested a life-threatening amount of phenobarbital (phenobarbitone), carbamazepine, theophylline, quinine, dapsone or

[Which programmable functions of pacemakers are available, and what is their clinical relevance?].

Microprocessors incorporated into cardiac pacemakers enable a substantial number of programmable functions (Table 1), the clinical relevance of which is the subject of this overview. Stimulation mode: The mode of operation can be chosen as fixed-rate, triggered and inhibited stimulation. Rate: Rate-programmability enables optimal setting with respect to electrophysiologic and hemodynamic considerations. Impulse amplitude and width: With programmable impulse amplitude and width excessive stimulation energy can be avoided and the duration of the aggregate prolonged. In some pacemakers, the outpu…

Defecography in patients with anorectal disorders. Which findings are clinically relevant?

To evaluate the results and clinical impact of defecography in patients with anorectal disorders, 100 results of defecographic examinations … ound between anorectal angle and the other function tests. In conclusion, the anorectal angle lacks clinical relevance. In patients with defecation problems, defecography may be indicated whenever other investigations (physical examination, anal manometry, anal EMG) have excluded local pathology or a spastic pelvic floor syndrome. In these situations, defecography c …

The clinical utility of faecal calprotectin in patients with differentiated and undifferentiated spondyloarthritis: Relevance and clinical implications.

OBJECTIVES: There is cumulative evidence in the literature supporting a potential role of faecal calprotectin (FCP) as a biomarker for gut inflammation in spondyloarthritis (SpA). However its relevance in undifferentiated SpA (USpA) is still uncertain. The aim of the current study is to assess the diagnostic significance of FCP levels in patients with differentiated and undifferentiated SpA. MATERIAL AND METHODS: A total of 52 differentiated SpA, 33 USpA and 50 controls could be included. For all patients, clinical evaluation, routine laboratory investigations, FCP levels, and occult blood in

Vipers of Major clinical relevance in Europe: Taxonomy, venom composition, toxicology and clinical management of human bites.

Snakebites in Europe are mostly due to bites from Viperidae species of the genus Vipera. This represents a neglected public health hazard with poorly defined incidence, morbidity and mortality. In Europe, fourteen species of "true vipers" (subfamily Viperinae) are present, eleven of which belong to the genus Vipera. Amongst these, the main medically relevant species due to their greater diffusion across Europe and the highest number of registered snakebites are six, namely: Vipera ammodytes, V. aspis, V. berus, V. latastei, V. seoanei and V. ursinii. Generally speaking, viper venom composition

Integrating screening and clinical interviews to assess clinically relevant addictive exercise behaviour in athletes.

BACKGROUND AND AIMS: Questionnaire-based tools such as the Exercise Dependence Scale (EDS) are widely used to identify athletes exhibiting dependence-like features, but it remains unclear how these classifications relate to clinically relevant addictive exercise behaviour. This study combined quantitative screening with semi-structured, study-specific clinical interviews to (1) examine the proportion of athletes showing high EDS scores who meet ICD-11 criteria for a disorder due to addictive behaviour (6C5Y), (2) characterize psychological and motivational differences between athletes meeting

Establishing a clinically relevant cutoff to the Dependency Scale from the dimensional clinical personality inventory.

The Clinical Dimensional Personality Inventory (IDCP) is a 163-item self-report tool developed for the assessment of 12 dimensions of personality pathology. One of the scales comprising the instrument-the Dependency scale-is intended to provide psychometric information on traits closely related to the Dependent Personality Disorder (DPD). In the present study, we used both Item Response Theory modeling and Receiver Operating Characteristic curve analysis to establishing a clinically meaningful cutoff for the IDCP Dependency Scale. Participants were 2.481 adults, comprised by outpatients diagno

Clinical T staging accuracy and its prognostic relevance in oral squamous cell carcinoma.

Accurate clinical T-staging is essential for guiding surgical management in oral squamous cell carcinoma (OSCC), particularly when determining treatment escalation or de-escalation. This study evaluated its diagnostic accuracy and impact on potential over- and undertreatment. This retrospective single-center study included patients with histologically confirmed OSCC who underwent primary surgical resection with negative margins between 2012 and 2022. Clinical T-staging based on preoperative contrast-enhanced computed tomography and clinical examination was compared with pathological staging. T

Plasma cell leukemia: genomic features and their potential relevance for exploring clinical actionability.

Plasma cell leukemia (PCL) is rare and aggressive. Plasma cells from patients with PCL were examined using next-generation sequencing (NGS). We compared NGS data from 18 patients with PCL (peripheral blood, n = 10; bone marrow, n = 8) and 1742 multiple myeloma (MM) samples. Mutations of TP53, CCND1, and KRAS were commonly observed in both diseases. Alterations in DIS3 (17% vs 1%), CCND2 (22% vs 15%), PIK3R1 (6% vs 0%), and MAP3K1 (6% vs 2%) were more common in PCL than in MM (P< .05). Translocations occurred in 11 (61.1%) patients with PCL; IGH::CCND1 and IGH::MYC were more frequent in PCL tha

Mechanism and clinical relevance of metabolic-immune cross-talk in pancreatic tumor microenvironment.

Metabolic stress-induced immunosuppression begins as early as the precancerous stage in pancreatic ductal adenocarcinoma (PDAC), increases with disease progression, and is characterized by the abundant suppressive myeloid and lymphoid cells. Metabolic adaptation and reprogramming are critical for the survival and function of tumor-infiltrating immune cells. In particular, cytotoxic T cells and natural killer (NK) cells, the stalwarts of cell-mediated immunity, often fail to adapt to the metabolically distinct pancreatic tumor microenvironment (TME) and thus perform antitumor activities poorly,

The Role of Vitamin D in Neuropathic Pain: Biological Mechanisms and Clinical Relevance.

Neuropathic pain remains a major clinical challenge due to its complex pathophysiology and limited treatment efficacy. Recent evidence suggests that vitamin D, beyond its classical role in bone and mineral metabolism, exerts neuroprotective and immunomodulatory effects that may influence pain perception. This review synthesizes current findings on the relationship between vitamin D status and neuropathic pain, highlighting potential mechanisms such as modulation of neuroinflammation, regulation of neuronal excitability, and influence on neurotransmitter pathways. Observational studies frequent

Point-of-Care Ultrasound in Blunt Chest Trauma: Diagnostic Performance and Clinical Relevance.

Chest trauma is a leading cause of death and illness among trauma patients, accounting for a significant proportion of emergency department admissions. Therefore, the early identification of intrathoracic injuries is crucial to ensure effective treatment and reduce the risk of complications, particularly those affecting the heart and lungs. Complementary diagnostic methods, such as ultrasound, due to their non-invasive nature and ability to provide real-time imaging, are therefore essential for making informed decisions based on correctly identifying these complications. In this context, this

Circulating exosomal miRNA in acute coronary syndrome: mechanisms, clinical relevance and future directions.

Despite significant advances in diagnosis and treatment, Acute Coronary Syndrome (ACS) remains one of the leading causes of morbidity and mortality globally. It comprises ST-elevation myocardial infarction, non-ST-elevation myocardial infarction, and unstable angina, caused by complex mechanisms such as plaque rupture, thrombosis, erosion, and microvascular dysfunction. The development of high-sensitivity cardiac troponins has greatly improved the early detection of ACS. However, concerns remain about specificity, timing, and prognostic accuracy. Exosomal miRNAs (ExomiRs) show promise as minim
