The main indications for internal fixation of intracapsular fractures are undisplaced and minimally displaced fractures and displaced intracapsular fractures in those aged less than about 70 years. For displaced fractures, closed reduction is to be preferred to open reduction. Numerous different implants may have to be used; with current practice favouring two or three parallel cannulated cancellous screws. These may be inserted either percutaneously or with minimal surgical exposure. Attention to surgical details of fracture reduction and implant positioning will minimise the risk of fracture

Superior dislocation of the patella without patellar ligament injury is an extremely rare condition. A review of the English-language literature found only 23 reported cases. In addition, the primary factor for dislocation in most of these cases was considered to be osteophytes in the inferior pole of the patella and the anterior surface of the femoral condyle; only 1 case had no osteophytes. We treated a 19-year-old woman who sustained a painful locking of the left knee after colliding with a friend. Plain radiography and computed tomography showed superior-lateral dislocation of the patella

A 43-year-old female fell backward outside her home while carrying a watering can. She sustained an ipsilateral fracture of the proximal ulna and distal radius (Colles fracture). The ulnar fracture was plated, and the Colles fracture was managed by closed reduction and percutaneous Kirschner wiring. The patient recovered without incident. This association of forearm fractures is unusual. Both fractures can be caused by a fall on the hand with the wrist in mid-extension and the elbow flexed, and they probably occurred in quick succession.

OBJECTIVE: Reverse oblique (RO) and transverse intertrochanteric fracture patterns constitute a challenge for the operating surgeon. Current … cted with the DCS.
CONCLUSION: The most crucial disadvantage of DCS was the high rate of nonunion. Closed fracture reduction in PFN seems to be the most critical parameter to prevent severe complications. The open reduction using DCS showed no advantages over closed reduction and PFN fixation in providing a more anatomical alignment in AO/Orthopaedic Trauma Association (O …

The aim of this study was to investigate the effect of Gotfried positive reduction (GPR) on repair of femoral neck fracture in rabbits and its underlying mechanisms. Male New Zealand white rabbits were employed to establish the model of femoral neck fracture. All the rabbits were randomly divided into four groups: control, open accurate reduction (OR), closed Gotfried negative support reduction (CR-N) and closed Gotfried positive support reduction (CR-P). At the 8th and 12th week after surgery, the anteroposterior and lateral radiographs of their hip joints were taken by X-ray, and local hemod

OBJECTIVE: Monteggia fracture-dislocation variants have been well documented in adults, but most of the literature in the pediatric population is in the form of case reports. These injuries present differently in children due to the presence of immature radiocapitellar epiphyses and the flexibility of the joint that is more prone to subluxation, contributing to occult presentations and/or misdiagnoses. The purpose of this study is to investigate the outcomes and complications of true Monteggia fracture-dislocations compared with their variants in the pediatric population.
METHODS: A retrospec

OBJECTIVE: To investigate the causes and the measures for prevention and management of complications of supracondylar fracture of the humerus.
METHODS: In 203 cases of supracondylar fracture of the humerus treated from July 2002 to July 2008, 49 cases had complication, whose data were retrospectively analyzed. There were 32 males and 17 females with an average age of 9 years (range 2-14 years). Fracture was caused by tumble in 38 cases, by falling from height in 4 cases, and by traffic accident in 7 cases. All cases were closed fractures. According to Gartland typing for supracondylar fractur

BACKGROUND: Distal forearm fractures are common pediatric injuries. While Closed Reduction and Percutaneous Pinning (CRPP) is a standard treatment with excellent outcomes, subsequent poliomyelitis is a less commonly reported but serious complication. This case highlights the diagnostic and therapeutic challenges of managing Methicillin-resistant Staphylococcus aureus (MRSA) fracture-related infection (FRI), particularly in the presence of subtle initial signs. CASE PRESENTATION: A 9-year-old male underwent CRPP for a right distal forearm fracture (Fig. 1). Intraoperative radiographs confirmed

BACKGROUND: Monteggia equivalent lesion represents a series of combined elbow and forearm injuries that resemble typical Monteggia fracture either in presentation or mechanism. The term has gradually evolved since its introduction, as sporadic case reports continued to complement it. The aim of this study was to present a furthermore type of that lesion which no previous study had reported and arouse pediatric orthopedists' additional awareness of it.
CASE PRESENTATION: A 11-year-old girl, whose injury pattern initially appeared to be a mild radial neck fracture with undisplaced proximal ulna

Treatment options in the past for distal femoral physeal fractures have varied from closed reduction to open reduction with internal fixation to balanced skeletal traction. In this study, ten patients with distal femoral physeal fractures treated with closed reduction and casting or skeletal traction are reviewed. Seven fractures lost position in comparison with original reduction films. Nine patients developed subsequent deformity. No consensus exists regarding the use of open versus closed treatment with internal fixation. This review of closed treatment yielded a high rate of unacceptable r

BACKGROUND: Fractures of the proximal third of the ulna and radius with associated anterior radial head dislocation are uncommon in children. Early recognition and appropriate management are essential to prevent long-term consequences of loss of forearm rotation, cubitus valgus, elbow instability and chronic pain.
CASE PRESENTATION: We present the case of a 3-year-old Caucasian boy who attended the emergency department following an un-witnessed fall, resulting in right elbow and forearm pain, swelling and deformity. Clinical and radiological examination revealed a Monteggia type IV fracture-d

The aim of distal humerus fracture treatment is articular surface reduction and stable fixation for early mobilisation and rehabilitation. This is usually performed by open reduction and internal fixation with plates. In the elderly osteoporotic patient this treatment is difficult to achieve due to fixation failure in fragile bone. We present our experience with treatment by closed reduction and external fixation with a non-bridging ring fixator in distal humerus fractures in elderly patients. There were ten females, aged 70-89 (average 78.4). Fracture types (AO/ASIF) included three supracondy

Although subtalar dislocation is not common in foot trauma, fracture of the entire posterior process of the talus is even rarer. The concomitant injury of a subtalar dislocation associated with a fracture of the posterior process of the talus has not yet been reported in the medical literature. A 52-year-old woman involved in a traffic accident had her right foot twisted into an inversion deformity. An X-ray revealed a medial subtalar dislocation associated with a fracture of the entire posterior process of the talus. Tomography demonstrated that the posterior process was incongruent to the an

We retrospectively reviewed 39 distal metaphyseal radius fractures in children. This study compares the results of closed reduction with or without percutaneous pinning. Twenty-four fractures were treated by closed reduction and above-elbow cast immobilisation. Re-displacement was noted after one week in three patients, for which they were revised with reduction and pinning. Fifteen patients were initially treated by closed reduction and pinning. In these patients we saw no redisplacement after six weeks immobilisation in a forearm cast. We recommend closed reduction and pinning as a predictab

We report the unusual case of a 16-year-old young man who sustained a rare association of a Hawkins' type-II talar neck fracture with a complete medial subtalar dislocation (Hawkins type-IIB) that occurred as an isolated injury after indirect trauma during a soccer game. Following closed reduction of the subtalar dislocation, standard radiographs and computed tomography (CT) demonstrated a comminuted fracture of the talus involving the base of the talar neck. Open reduction was performed and the fracture was stabilized by ORIF. At 1-year follow-up, functional and radiographic outcomes were gra

Fracture of the lateral portion of the distal end of the tibia in adults is known as the fracture of Tillaux (Tillaux, 1872). The fracture line extends vertically from the distal articular surface of the tibia upwards to the lateral cortex. The fragment is roughly quadrangular in shape. The analogous epiphyseal injury is less common and two such cases are presented here, both were fixed internally. There now appears to be a consensus of opinion that for fractures of the lateral portion of the distal tibial epiphysis open reduction should be performed, and that closed reduction should be reserv

OBJECTIVE: To appraise the value of clinical treatment of percutaneous reduction and fixation of osteoporotic fractures for the proximal humerus in a geriatric population.
METHODS: Postoperation complication, mortality in hospital and within the first three months postoperation, operation time, blood transfusion requirement, the functional outcome of the shoulder had been analysed and observed in 37 cases for open and closed fixation.
RESULTS: Closed reduction provided the benefit of obtaining and holding adequate reduction without the soft-tissue dissection of open reduction and internal fi

Simultaneous dislocation of the distal radioulnar joint and the radial head without fracture is an uncommon injury. Prior reports have described closed treatment of this injury. We report a case of paired dislocation without fracture that required open reduction of both articulations in the acute setting. Potential mechanisms responsible for this pattern and recommendations for management of similar patients are described.

An uncommon case has been reported of bilateral transscaphoid-perilunate dislocation fracture following trauma. A 35-year-old male with bilateral transscapholunate dislocation was treated by closed reduction and internal fixation. The case was examined in detail, and compared to the findings in the literature; observations regarding fracture prognosis were also made. Two years post-surgery, the patient remained asymptomatic.
