PURPOSE OF REVIEW: While primary headaches like migraines or cluster headaches are prevalent and often debilitating, it's the secondary headaches-those resulting from underlying pathologies-that can be particularly ominous. This article delves into the sinister causes of headaches, underscoring the importance of a meticulous clinical approach, especially when presented with red flags.
RECENT FINDINGS: Headaches, one of the most common complaints in clinical practice, span a spectrum from benign tension-type episodes to harbingers of life-threatening conditions. For the seasoned physician, dif…

Orexins/hypocretins are neuropeptides formed by proteolytic cleavage of a precursor peptide, which are produced by neurons found in the lateral hypothalamus. The G protein-coupled receptors (GPCRs) for these ligands, the OX₁ and OX₂ orexin receptors, are more widely expressed throughout the central nervous system. The orexin/hypocretin system has been implicated in many pathways, and its dysregulation is under investigation in a number of diseases. Disorders in which orexinergic mechanisms are being investigated include narcolepsy, idiopathic sleep disorders, cluster headache and migraine. Hum…

We have reviewed the diagnoses of 654 children aged from 7 to 14 years who attended a neurologist for headache evaluation. Headaches beginning between the age of 7 and 14 represented a higher percentage (18.3%) than the proportion of preadolescent children in our health area (12.9%). Headaches were more common in girls; although cluster, posttraumatic, benign exertional headaches, and the only case of brainstem glioma were restricted to boys. Despite the female predominance, the proportion of males with migraine was significantly higher in the preadolescents than in the over 15 age group. Migr

Establishing an open and honest physician-patient relationship is essential for the proper evaluation and management of headache disorders. Obtaining a complete headache and medical history is the most important part of the initial diagnostic evaluation. This history should include information about headache onset, pain intensity, character of the pain, presence of aura, associated autonomic symptoms, and trigger factors. Special attention must be paid to the frequency of analgesic use, both prescription and over-the-counter, to identify analgesic rebound headache. A thorough neurologic examin

A Hungarian patient with short-lasting, unilateral, neuralgiform headache attacks with conjunctival injection and tearing (SUNCT) is presented in this paper. This male patient was first diagnosed as having first division trigeminal neuralgia. The location and duration of the attacks and the prominent accompanying autonomic features on the symptomatic side, such as conjunctival injection, lacrimation, nasal stuffiness, and the inefficacy of drugs, led to a reconsideration of the diagnosis. The pain paroxysms occurred frequently during a 3- to 4-month period, followed by a longer remission phase

The term "atypical facial pain" describes chronic pain syndrome involving the face. The syndrome consists of continuous mostly burning bouts of pain, localized in one half of the face. The authors review the literature of the topic, discuss also the historical aspects of terminology. The clinical symptoms and signs, course, and therapeutic attempts are also discussed.

The traditional approach to classifying headache based on symptoms and assumed mechanisms is criticized as having limited utility when applied to tension-type headache and migraine. The study reported here was designed to explore an alternative method of conceptualizing chronic headaches based on functional characteristics or controlling variables. One hundred and ninety nine chronic headache sufferers completed questionnaires which enquired about the antecedents and consequences of their headaches. An attempt to build a categorical model driven by functional characteristics using cluster anal

The Sphenopalatine neuralgia of the ganglion is characterized for a throbbing pain mostly unilateral encircling the orbit and occasionally extended to the neighborhood. A vegetative component hydrorrhea, sneezing crisis and abnormal lacrimation is associated. The ideal therapy, giving excellent results, is the iontophoresis of the ganglion, which technic was published years ago by F. Barceló.

BACKGROUND: Both clinical experience and clinical studies suggest a relationship between tinnitus and headache. Here, we aimed to investigat … airment in patients with left-sided and bilateral headaches as well as in patients with migraine or cluster headache.
CONCLUSION: The observed increased impairment in tinnitus patients with comorbid headache can be explained as an additive effect of both disorders on health-related quality of life. The more frequent occurrence of further comorbidities suggests a …

Despite the fact that neurophysiological evaluation is not useful for primary headache diagnosis, the nociceptive system exploration through reflexes and evoked potentials procedures may give an aid in understanding the pathophysiological mechanism subtending pain. Neuropathic pain is caused by a lesion or disease of the somatosensory nervous system, which is supported by clinical evaluation and instrumental assessment by trigeminal and nociceptive reflexes and laser evoked potentials. The same methods, applied to migraine and cluster headache, together with evidences coming from structural an…

Furthermore, we recommend that invasive neuromodulation be debated by multidisciplinary committees, and that the pro …

Background The use of complementary and alternative medicines (CAM) is common among patients with primary headaches. In parallel, CAM resear … y of these interventions for migraine, though some explored their role in tension-type headache and cluster headache. The quality of the evidence in this area is generally poor. Conclusions CAM is becoming more commonplace in the headache world. Several interventions show promise, but caution needs to be exercised in using these agents given limited safety and eff …

The greater occipital nerve (GON) is primarily responsible for the sensory innervation of the posterior region of the scalp. GON block has been described as a safe and effective therapeutic option for various types of headaches, including occipital neuralgia, migraine, cervicogenic headache, and cluster headache. Despite its established role in headache management, there is a paucity of evidence in the literature regarding its use in the perioperative setting, particularly in surgeries involving the occipital region of the scalp. In this context, we present a case series of three patients unde…

Paroxysmal hemicrania (PH) is an underreported and underdiagnosed primary headache disorder. It usually begins in the third or fourth decade of life. The recent observations indicate that it is equally prevalent in both males and females. PH is characterized by severe, strictly unilateral head pain attacks that occur in association with ipsilateral autonomic features. The attacks in PH are shorter and more frequent compared with cluster headache (CH) but otherwise PH and CH have similar clinical features. The hallmark of PH is the absolute cessation of the headache with indomethacin. However, …
