Characteristics and causes in patients presenting with convulsions in the Danish prehospital setting.

BACKGROUND: Convulsing patients are one of the most common reasons for dispatch in the prehospital system. Determining the underlying cause of symptoms is paramount for both patient safety and determining the correct patient course. This study aims to describe the population of patients presenting with convulsions at first contact with an anesthesiologist-staffed mobile emergency care unit, to uncover the underlying reason for the convulsions, and to establish the outcome of the patient and correlate this to patient age. METHODS: This retrospective cohort study was conducted through a manual

Anemia, inflammation, circadian rhythm and season effects in febrile convulsions.

OBJECTIVE: Our aim in the study is to investigate circadian rhythm, season, age, gender, and hemogram parameters in children diagnosed with febrile convulsions. METHODS: The data of a total of 478 children, consisting of 160 patients with febrile convulsions (90 male/70 female) and 318 control groups (healthy, convulsiyon free- febrile), were compared. RESULTS: The average age of all patients was 25.7±14.7 (minimum 6-maximum 60) months, and the most common convulsion age was 13-18 months. Circadian rhythm was similar, application seasons were similar. The male/female ratio was 1.3. MPV in wi

Termination of convulsion seizures by destabilizing and perturbing seizure memory engrams.

Epileptogenesis, arising from alterations in synaptic strength, shares mechanistic and phenotypic parallels with memory formation. However, direct evidence supporting the existence of seizure memory remains scarce. Leveraging a conditioned seizure memory (CSM) paradigm, we found that CSM enabled the environmental cue to trigger seizure repetitively, and activating cue-responding engram cells could generate CSM artificially. Moreover, cue exposure initiated an analogous process of memory reconsolidation driven by mammalian target of rapamycin-brain-derived neurotrophic factor signaling. Pharmac

Serum and Cerebrospinal Fluid Lactate Dehydrogenase in Children with Febrile Convulsions.

OBJECTIVE: Tissue damage caused by febrile convulsion has not still been proved or refuted completely. Given the fact that lactate dehydrogenase as an intracellular enzyme can be increased due to tissue damage, we decided to evaluate serum and cerebrospinal fluid lactate dehydrogenase in children with febrile convulsion. MATERIALS & METHODS: This is a cross-sectional study on 166 children aged 6-24 month, in three groups of simple febrile convulsion (n=56), complex febrile convulsion (n=27) with 3 different subgroups (recurrence in 24 hours, duration >15 minutes, and with focal components), a

Non-convulsive status epilepticus in the setting of cannabidiol adjunctive therapy.

Anti-seizure medications (ASMs) can cause non-convulsive status epilepticus (NCSE), but account for less than 5% of all NCSE cases. We present a 63-year-old, right-handed male with a history of intractable focal epilepsy since age seven years old, whose bouts of NCSE were triggered by cannabidiol (CBD) adjunctive therapy. His most common seizure types included focal myoclonic or tonic seizures with vocalization, usually with awakening, which occurred on a monthly basis despite the combination of tiagabine, perampanel, levetiracetam, lacosamide and clonazepam. After CBD was initiated, he began

Primary intestinal lymphangiectasia presenting as limb convulsions: A case report.

BACKGROUND: Primary intestinal lymphangiectasia (PIL) is a rare protein-losing enteropathy characterized by abnormally dilated lymphatic structures, resulting in leakage of lymph (rich in protein, lymphocytes, and fat) from the intestinal mucosal and submucosal layers and thus hypoproteinemia, lymphopenia, hypolipidemia, and pleural effusion. CASE SUMMARY: A 19-year-old Chinese male patient complained of recurrent limb convulsions for the last 1 year. Laboratory investigations revealed low levels of calcium and magnesium along with hypoproteinemia and high parathyroid hormone levels, whereas

Ketamine-induced generalized convulsive seizure during procedural sedation.

Ketamine is used widely in emergency departments for a variety of purposes, including procedural sedation for facial laceration in pediatric patients. The major benefits are its rapid onset of effects, relatively short half-life, and lack of respiratory depression. The known side effects of ketamine are hallucinations, dizziness, nausea, and vomiting. Seizure is not a known side effect of ketamine in patients without a seizure history. Here, we present the case of a patient in whom ketamine likely induced a generalized tonic-clonic seizure when used as a single agent in procedural sedation for

Cough syncope and hyperventilation-induced convulsion in Chiari 1.5 malformation.

Chiari malformation type I (CM1) is defined as cerebellar tonsillar herniation below the level of the foramen magnum. Syncope, especially cough syncope, is a rare but important symptom of CM1 patients. Here, we report a CM1 patient, in combination with brainstem herniation (CM1.5), presenting with repetitive syncope who was successfully treated by decompressive surgery. A 43-year-old right-handed male, with 5-year history of repeated episodes of loss of consciousness in association with cough, was investigated. Neurological examination revealed slight muscle weakness, clumsiness, and sensory d
