Coronary Blood Flow Measurement in Conventional Coronary Angiograms by a New Method Based on Contrast Density Detection. A Physiological Insight.

BACKGROUND: TIMI flow grade and corrected TIMI frame count (CTFC) are widely used methods to evaluate angiographic coronary blood flow. Measurement of coronary blood flow (CBF) on standard coronary angiography (CAG) has aroused great interest recently, trying to combine the CTFC concept with new methods for post-angioplasty and for cardiac syndrome X assessment. Additionally, coronary slow flow it is now considered a major criterion for microvascular angina. OBJECTIVE: Explore a new approach of quantitative angiographic measurement of CBF based on densitometric contrast detection in CAG off-l

Observations on the external morphology and vasculature of a fetal heart of the bowhead whale, Balaena mysticetus.

BACKGROUND: Specialized demands within the aquatic environment for over some 60 million years have shaped unique morphological expressions in the whales, dolphins, and porpoises (Cetacea). Detailed consideration of these features, particularly in the great whales, has often been constrained by difficulties in securing adequate specimens for study. We had the opportunity to examine external heart morphology in a rarely obtained and prepared specimen from the bowhead whale, Balaena mysticetus. METHODS: The external morphology and in situ relations of a formalin-perfused heart were examined gros

Regional coronary blood flow in canine hemorrhagic shock.

This study examined circumferential, apex-to-base, and transmural distribution of coronary blood flow at rest and in hemorrhagic shock in a canine model. In addition, animals subjected to identical anesthesia and instrumentation were not hemorrhaged and served as controls. Regional coronary blood flow was measured by the radioactive microsphere technique, using 15-microns diameter microspheres in both control and shocked dogs. Circumferential variation in flow occurred at rest with the left ventricular free wall blood flow higher than septal flow and the anterior region better perfused than th

Scintigraphic quantification of asynchronous myocardial motion during the left ventricular isovolumic relaxation period: a study in the dog during acute ischemia.

Asynchronous motion of left ventricular myocardium during the period of left ventricular isovolumic relaxation has often been observed in patients with coronary artery disease. Detection and quantitation of this abnormality with noninvasive nuclear tracer methods, however, have not yet been reported. Thus, functional images of regional left ventricular time to minimum counts (or volume), computed from gated blood pool image sequences, were analyzed to detect and quantitate myocardial asynchrony during this interval. The method was tested by comparing regional with global time to minimum counts

Platelet aggregation secondary to coronary obstruction.

From many observations made at autopsy it is apparent that thrombosis in a coronary artery is usually, if not always, associated with rupture of an atheromatous plaque. The sequelae of such rupture include hemorrhage into the plaque with further narrowing of the lumen, formation of an occlusive thrombus or of a non-occlusive thrombus. A developing thrombus in an artery undergoes fragmentation with showering of the distal microcirculation by aggregates of platelets possibly with some admixture of fibrin. In many cases of sudden cardiac death associated with severe atherosclerotic stenosis of th

[Can isoflurane be advised for controlled hypotension?].

Since 1983, many papers tell of the usefulness of isoflurane for induced hypotension. It can induce and maintain stable arterial hypotension during neurosurgery, or any other surgical procedure requiring induced hypotension. Its use has proved to be simple. Although other hypotensive techniques are possible, especially if only moderate hypotension is required, the mechanism of action of isoflurane is very appealing: it reduces arterial pressure by reducing the peripheral resistances, without reducing the output, unlike halothane or trinitrin. Moreover, as it is anaesthetic, it reduces the over

Impact of Coronary Dominance on Atrioventricular Node Ablation Approach.

Persistent atrial fibrillation often requires atrioventricular node (AVN) ablation when conventional rhythm - or rate-control strategies are unsucessful, but procedural outcomes may be influenced by anatomic variability. We hypothesized that coronary artery dominance affects AVN positioning and the preferred ablation approach. In this retrospective study, 132 patients who underwent AVN ablation and coronary CTA were analyzed. Right coronary dominance was present in 78.8% of patients, left in 13.6% and co-dominant in 7.6%. Patients with left or co-dominant circulation had significantly higher o

Invasive evaluation of coronary microvascular dysfunction.

Coronary microvascular dysfunction (CMD) is a prevalent cause of ischemic heart disease and is associated with poorer quality of life and worse patient outcomes. Both functional and structural abnormalities of the microcirculation can generate ischemia in the absence of epicardial stenosis or worsen concomitant obstructive coronary artery disease (CAD). The invasive assessment of CMD allows for the evaluation of the entirety of the coronary vascular tree, from the large epicardial vessels to the microcirculation, and enables the study of vasomotor function through vasoreactivity testing. The s

Type IV dual left anterior descending artery associated with anomalous origin of the left circumflex coronary artery from the right coronary artery: a case report.

Dual left anterior descending coronary artery (LAD) originating from the left main stem and the right coronary artery (RCA) (Type IV dual LAD) is a very rare coronary artery anomaly. Association of this anomaly with the anomalous origin of the circumflex artery from the RCA is a very rare occurrence. In this report, we describe a patient presenting with acute lateral wall myocardial infarction who subsequently was found to have this coronary anomaly.

[Diffuse atherosclerotic disease unmasked by invasive physiologic assessment of coronary flow].

It is known that coronary atherosclerosis is a diffuse process, very little visible at angiography. This article describes a stable angina patient, three months after acute myocardial infarction (AMI), and a severe lesion in anterior descending artery (ADA), evinced by coronariography. Myocardial fractional flow reserve (FFR), obtained through intracoronary pressure measurements, was 0.37 during maximum hyperemia, clearly showing the presence of ischemia. A stent was implanted in ADA and, despite the excellent angiographic result, post-stent FFR was only 0.75, the minimum limit, below which th

Right pulmonary artery agenesis and coronary-to-bronchial artery aneurysm.

Isolated unilateral pulmonary artery agenesis is a rare congenital anomaly that may be complicated with hemoptysis, recurrent pulmonary infections or pulmonary hypertension. To our knowledge the occurrence of a coronary syndrome associated with a coronary-to-bronchial artery saccular aneurysmal collateralization has never been described before. A 44-year-old female presented a congenital right pulmonary artery agenesis associated with a hypotrophic and multicystic right lung complicated with recurrent bronchitis. This patient had a coronary syndrome for which the coronary artery imaging showed

Anomalous left coronary artery from pulmonary artery with mitral stenosis.

The usual presentation of anomalous left coronary artery from pulmonary artery is severe left-sided heart failure and mitral valve insufficiency presenting during the first months of life. The manifestations of left heart failure may be masked if pulmonary artery pressure remains high. We believe this is a rarest of rare case of anomalous left coronary artery from pulmonary artery with severe mitral stenosis and pulmonary hypertension in which pulmonary hypertension, along with good collateral circulation helped to preserve left ventricular function.
