Haemorrhagic cystitis: a review of management strategies and emerging treatments.

Haemorrhagic cystitis (HC) is characterised by persistent haematuria and lower urinary tract symptoms following radiotherapy or chemotherapy. Its pathogenesis is poorly understood but thought to be related to acrolein toxicity following chemotherapy or fibrosis/vascular remodelling after radiotherapy. There is no standard of care for patients with HC, although existing strategies including fulguration, hyperbaric oxygen therapy, botulinum toxin A, and other intravesical therapies have demonstrated short-term efficacy in cohort studies. Novel agents including liposomal tacrolimus are promising …

A rare case of emphysematous cystitis due to Candida glabrata.

Emphysematous cystitis (EC) is a rare condition characterized by gas within the bladder wall or lumen. EC is most commonly seen in elderly women with poorly controlled diabetes mellitus (DM). Escherichia coli and Klebsiella pneumoniae are most commonly implicated. We present a 68-year-old woman with poorly controlled DM who presented with altered mental state with growth of Candida glabrata in urine and blood cultures. CT abdomen and pelvis revealed air in the bladder lumen and the extraperitoneal space. Bladder rupture was suspected and bladder decompression was managed conservatively with a …

General Overview of Artificial Intelligence for Interstitial Cystitis in Urology.

Our understanding of interstitial cystitis/bladder pain syndrome (IC/BPS) has evolved over time. The diagnosis of IC/BPS is primarily based on symptoms such as urgency, frequency, and bladder or pelvic pain. While the exact causes of IC/BPS remain unclear, it is thought to involve several factors, including abnormalities in the bladder's urothelium, mast cell degranulation within the bladder, inflammation of the bladder, and altered innervation of the bladder. Treatment options include patient education, dietary and lifestyle modifications, medications, intravesical therapy, and surgical inter…

A Rare Case of Recurrent Cystitis in a Primary Care Setting.

Uncomplicated cystitis is common in women and typically presents with symptoms such as increased urinary frequency, dysuria, suprapubic pain, and urgency. Escherichia coli is the most frequently identified pathogen in these cases. Colovesical fistulas constitute an uncommon etiology of recurrent urinary tract infections, and they are even rarer in women due to the protective barrier provided by the uterus. Faecaluria and pneumaturia are the pathognomonic symptoms of these types of fistulas that help differentiate them from recurrent cystitis. While the gold standard imaging is the abdominopelv…

[Onto-phylogenetic prerequisites for development of chronic cystitis in women].

INTRODUCTION: Urinary tract infections (UTIs) are among the most common bacterial infections. At the request "cystitis", there are 12,067 publications in the RSCI system (e.library) as of 10/08/2023 and 16,332 articles were screened in the Pubmed. This is evidence that the problem of cystitis is far from being resolved. MATERIAL AND METHODS: A total of 425 patients with bacterial vaginosis and 77 women with chronic recurrent cystitis were included in the study. In all patients, the vaginal biocenosis was assessed through molecular genetic testing. The examination included filling out the Russ…

Gangrenous Cystitis Secondary to a Spontaneous Hematoma of the Lesser Pelvis.

Gangrenous cystitis is considered a life-threatening but rather rare clinical entity due to the widespread use of antibiotics. We herein report a case of a 78-year-old female with gangrenous cystitis secondary to a spontaneous expanded hematoma of the lesser pelvis who underwent partial cystectomy followed by bilateral ureterostomies with no favorable outcome.

Pneumomediastinum as a rare complication in an immunosuppressed patient with emphysematous cystitis.

The extravesical spread of gas into the extraperitoneal space is an unusual complication of emphysematous cystitis and rarely, can reach remote areas of the body. Herein, we present the case of an immunosuppressed woman with emphysematous cystitis and extensive spread of extraperitoneal free gas up to the mediastinum.

Obesity influence on bladder inflammation and cancer: a cystitis model.

BACKGROUND: Recently, the role of subclinical inflammation in obesity has gained prominence. An association between obesity and chronic inflammation has been observed in several studies that show a relationship between increased morbidity and high Body Mass Index (BMI). This study aims to compare inflammatory pathways in obese (by high-fat diet) and non-obese mice after exposure to an intravesical carcinogen in a cystitis model. METHODS: We divided 16 female, 7 week old mice into two groups: 1) CONTROL: standard diet, and 2) OBESE: high fat diet for 8 weeks. Both groups underwent a protocol f…

Comparison of Physical and Digital Treatment and Documentation of Uncomplicated Cystitis.

Introduction Symptomatic criteria have a diagnostic specificity of approximately 90% for uncomplicated cystitis. Today there are triage bots that can collect patient history and document simultaneously. Acute uncomplicated cystitis could potentially be managed digitally, due to the symptom-based approach to diagnosis, but no studies have yet validated this approach. Aim We determined the extent of criteria documentation and evaluated adherence to antibiotic recommendations in order to compare physical and digital patient consultations for uncomplicated cystitis. Materials and methods This cros…

Epidemiology of antibiotic treatment for uncomplicated cystitis in adults in Japan.

INTRODUCTION: A large number of antibiotics are used for the treatment of uncomplicated cystitis owing to its high morbidity. As the administration of antibiotics for uncomplicated cystitis may be considered an example of inappropriate use, outpatient antimicrobial stewardship for this condition is important. We evaluated the current pharmacoepidemiology trends for the treatment of uncomplicated cystitis in Japan to predict stewardship strategies. METHODS: This descriptive observational study analyzed data from an anonymized claims database of employees and their family members covered by the…

Uroprotective effect of ambroxol in cyclophosphamide-induced cystitis in mice.

PURPOSE: Hemorrhagic cystitis (HC) is defined as any types of acute or chronic inflammation of urinary bladder with several reasons. One of the most common causes of HC is cyclophosphamide (CYP), an effective antineoplastic agent, due to its urotoxic potential. Ambroxol (AMB) is a mucoactive drug that has been used for numerous respiratory diseases. Besides its mucolytic activity, AMB is a potent antioxidant and antiinflammatory agent that is becoming more attractive for the treatment of several oxidative/inflammatory disorders. The aim of this study was to evaluate the uroprotective potential…
