High-grade L5 spondylolisthesis associated with dural ectasia in neurofibromatosis.

A 15-year-old girl was admitted to our hospital with severe low back pain. She had scoliosis dextra and tight hamstrings. A plain radiograph showed high-grade L5 spondylolisthesis with vertebral scalloping from the fourth lumbar to the first sacral vertebra. L5 wide laminectomy and posterior lumbar interbody fusion by iliac bone graft was performed using the Galveston method of sacral fixation and a pedicle screw system. The rod and hook system was used from T9 to the bilateral iliac wing. We added posterolateral fusion from T10 to S1 by autograft and allograft. The patient became pain free an

[Congenital cystic bile duct dilatation: an incidental intraoperative finding].

In adulthood, choledochal cysts are often surprisingly discovered during cholecystectomy. We report here on a case of an unsuspected congenital type-IA biliary cyst, according to Todani's classification encountered during laparoscopic cholecystectomy in a 30-year-old woman complaining of acute abdominal pain with an unremarkable preoperative workup. The well-known risk of developing cystic cancer, mainly in the adult, means that an excisional operation is indicated to prevent such complications. Cyst excision and Roux-en-Y hepaticojejunostomy is the definitive treatment of choice, performed at

Post-stenotic dilatation: challenge in renal ostial stenting--a simple solution.

Stenting is the treatment of choice for treating stenotic renal ostial lesions. During the stenting of an ostial lesion in a renal artery with post-stenotic dilatation, we were faced with the problems of unavailability of a balloon of appropriate length and diameter, and determining the real reference vessel diameter. The problem was solved by a simple technique.

Measuring esophageal distention by high-frequency intraluminal ultrasound probe.

This investigation used a high-frequency intraluminal ultrasound (HFIUS) probe to study changes in esophageal muscle thickness as a marker of muscle contraction. The aim of this study was to determine whether this technique could accurately measure distention of the esophagus during swallows of liquid bolus and during gastroesophageal reflux (GER) in normal subjects. Despite some limitations, HFIUS appears to be a valid technique for both of these measurements in normal subjects. Future studies in patients with symptomatic GER disease (GERD) should be able to determine whether typical symptoms

New technique to ligate enlarged cystic duct using a clip applier during laparoscopy: report of a case.

We developed a new technique to ligate an enlarged cystic duct under laparoscopy. First, the cystic duct was ligated with a large size Endoclip and cut halfway through with scissors. The rest of it was ligated by the clips and divided. This procedure does not require any special device or skill.

[Varicocele at a prepubertal age. Adolescent varicocele].

Varicocele is a dilatation of the spermatic veins, particularly on the left side, and represents one of the most frequent causes of male infertility. Its frequency in pre-adolescence varies from 10% to 15%. Even today varicocele pathogenesis is not clear. In the period from August 1990 to August 1999, 35 males between 10 and 16 years of age were treated surgically in our Institution via interruption of the internal spermatic vein using Palomo's technique. Patients that reached the age of 18 after surgery have had a spermiogramm. The problem of varicocele therapy in pre-adolescence has not yet

Segmental dilatation of the ileum presenting with acute intestinal bleeding.

The authors report a case of acute intestinal bleeding caused by ulcerated segmental ileal dilatation occurring in a 5-month-old infant. The profuse emission of dark red blood per rectum caused acute anemia (Hb 5.3 g/dL). Because of absence of any other sign and symptom and normal plain x-ray, a bleeding Meckel's diverticulum was suspected. Laparoscopy was performed showing the ileal dilatation, and through an enlarged port incision the bowel was exteriorized and resected. Histology results showed an ulcer caused by heterotopic gastric mucosa. The etiopathogenesis and clinical manifestations o

Prenatal ultrasonographic appearance of type IIId (uncorrectable type with cystic dilatation) biliary atresia.

Although prenatal ultrasonographic (US) diagnosis has been reported in biliary atresia (BA), most cases are type I (correctable with cystic dilatation). We report three prenatal cases of type IIId BA (uncorrectable with cystic dilatation). Routine fetal US at 22 to 24 weeks of gestation showed two communicating cystic lesions 12 to 16 mm in diameter. On color Doppler US, the lesions were separate from the portal vein or hepatic artery. The size did not change during the prenatal period in any case. Choledochal cyst (CC) was considered the most likely diagnosis, although BA with cystic lesions

Polymyositis as a cause of total gut failure.

BACKGROUND: Gastrointestinal manifestations are seen in systemic sclerosis and mixed connective tissue disorders but are rare in pure polymyositis. CASE REPORT: A 44 year old woman with polymyositis who developed total gut failure requiring treatment with total parenteral nutrition is described. RESULTS: The patient's polymyositis is now fully controlled biochemically, but her gastrointestinal symptoms persist.

Mild dilatation of renal pelvis in term neonates with urinary tract infection.

INTRODUCTION: In patients with acute biliary pancreatitis (ABP), cholecystectomy is mandatory to prevent further biliary events, but the precise timing of laparoscopic cholecystectomy (LC) for non-severe disease remain a subject of ongoing debate. AIM: To prove which method - early or delayed LC - is the method of choice in the non-severe disease by examining rates of gallstone-related complications, dissection difficulty, conversion rate, morbidity, mortality, and length of hospital stay. MATERIAL AND METHODS: We retrospectively analysed the data of patients diagnosed with non-severe ABP, w

[Pancreato-biliary maljunctions and congenital cystic dilatation of the bile ducts in adults].

Pancreato-biliary maljunctions (PBM) in adults are defined by the presence of an abnormally long common pancreato-biliary duct (more than 15 mm long) formed outside the duodenal wall and/or by high amylase level in the bile. The high amylase level in the bile is the functional expression of a chronic toxic reflux of pancreatic juices into the biliary tree. The presence of the PBM have two basic consequences: (i) formation of congenital cystic dilatations of the bile duct (CCBD) during embryogenesis and (ii) cancerous degeneration of extrahepatic bile ducts including the gall bladder. CCBD are

[Agenesis of the gallbladder].

The authors report a case of agenesis of gallbladder associated with lithiasis of common bile duct. A review of the literature confirms the rarity of this congenital malformation. In 40-60% of cases it is associated with other gastrointestinal, skeletal, cardiovascular and genitourinary malformations. In 20-50% of cases it can be associated with common bile duct stones. Patients are asymptomatic in the majority of cases. A preoperative diagnosis is difficult despite of high accuracy of the modern diagnostic imaging techniques.

[The so-called megaureter-megacystis syndrome: a case report].

We encountered a patient with megaureter-megacystis syndrome showing a giant bladder and dilated ureters with marked reflux, which is very rare; to our knowledge, only 2 patients have been reported in Japan. The patient was a 4-year-old boy, who showed inborn polyposia and polyuria, and proteinuria at the age of 1 year. He visited the pediatric department in our hospital complaining of cold-like symptoms, stomachache and diarrhea. Urinary infection and kidney dysfunction were observed, and the patient was hospitalized for close examination. Bilateral pyelocaliceal hydronephrosis was detected b
