Surgery is the recommended treatment for primary hyperparathyroidism. For exceptional medical treatment, the main drugs available are cinacalcet to normalize plasma calcium, and bisphosphonates to prevent bone loss. The patients eligible for medical therapy are those who cannot be operated or whose condition persists despite optimal surgical treatment. However, PHPT can cause other complications that need special attention regarding prevention and treatment during follow-up.

Osteonecrosis of the jaw is an uncommon consequence of biphosphonate therapy. This has most commonly been a bone complication with little if any soft tissue involvement. An unusual case of necrotizing fasciitis with extensive soft tissue infection stemming from a prolonged case of osteonecrosis of the jaw presented. The management of this patient (aggressive surgical debridement and prolonged wound care) is reviewed as well as the review of the underlying processes.

Osteonecrosis of the jaw (ONJ) is a well-known devastating side effect of parenteral bisphosphonate therapy for cancer. Several ONJ cases have been reported in patients taking oral bisphosphonates for osteoporosis or Paget's disease. Even if the number of cases of ONJ in patients taking oral bisphosphonates are still rare compared to the total exposure, rheumatologists treating bone diseases with bisphosphonates must be aware of this new complication, allowing for prevention and early diagnosis. The patients must be informed on the benefit/risk of bisphosphonate therapy and, when necessary and

Skeletal-related events resulting from bone metastases or osteoporosis can significantly contribute to morbidity and mortality in cancer patients. Expert opinion on the effectiveness of bisphosphonates in this setting is evolving. Here we review current evidence on the risks and benefits of bisphosphonate therapy for a wide variety of cancers, as well as clinical management of its adverse effects. A MEDLINE search of English-language literature (1966 through May 2006) was conducted using the terms bisphosphonate, cancer, multiple myeloma, malignancy, and randomized controlled clinical studies.

Bisphosphonates (BPs) have been in clinical use for performing bone imaging, treating different metabolic bone diseases, and managing osteolysis and the hypercalcemia associated with some cancers since the late 1960s. Although these agents are extremely effective at treating these conditions, recent publications have noted a possible sequela of treatment with BPs is bisphosphonate-related osteonecrosis (BRON) of the jaws. In the past few years, a great deal of attention has been paid to recognition and management of different issues surrounding patients being treated with bisphosphonates by ma

Current treatments for bisphosphonate-associated osteonecrosis of the jaw (ONJ) in multiple myeloma patients have limited efficacy. The biological effects of ozone indicate it may be therapeutic for ONJ. We, here, report the efficacy of a 15-day course of antibiotics, surgery and ozone therapy in 12 MM patients with ONJ.

PURPOSE: This study was done to evaluate the use of multidetector computed tomography (MDCT) in the evaluation of jaw osteonecrosis, a recently described medical entity in patients receiving long-term intravenous or oral bisphosphonates, and to help radiologists recognise it to enable early diagnosis and appropriate management.
MATERIAL AND METHODS: Thirty-eight patients with a history of neoplasm (breast, prostate, multiple myeloma, pancreas) or osteoporosis and bisphosphonate use and recent oral surgery who presented with pain in the mouth and temporomandibular joints, soft tissue infection

Bisphosphonates are widely used in the treatment of cancer patients with hypercalcemia and bone metastases or in osteoporosis therapy. Current reports have focused on therapy-resistant osteonecrosis of the jaws as a possible side effect of bisphosphonates. Official German drug committees have recently warned about the possibility of these side effects. So far we have experience with 12 patients showing therapy-resistant osteonecrosis of the mandible under bisphosphonate medication, three of whom received oral bisphosphonates for osteoporosis treatment. Presentation of these three cases provide

Bisphosphonates are the most clinically important class of antiresorptive agents available to treat diseases characterized by osteoclast-mediated bone resorption. Currently, seven bisphosphonates have the approval of the United States Food and Drug Administration. The most common adult diseases treated with bisphosphonates include osteoporosis, Paget disease, and metastatic bone disease. The treatment of pediatric disorders such as osteogenesis imperfecta and fibrous dysplasia with bisphosphonates has gained momentum, and initial investigations have demonstrated an acceptable safety profile. Biophosphonates are analogues of pyrophosphate. The mechanism of action of bisphosphates is the inhibition of osteoclast activation (antiresorptive mechanism). The oral bioavailability of biophosphonates is only about 1-2%, and because of gastrointestinal side effects (mainly esophageal irritation), oral agents are less useful in oncology. Biophosphonates are used for the treatment of Paget s disease of bone, the prevention of osteoporosis, and in another clinical scenarios as the prevention of bone disease after organ transplantation. In clinical oncology biophosphonates are used for the trea

INTRODUCTION: Hypercalcaemia is a relatively common clinical problem with a wide spectrum of presentations. This review provides an overview of the diagnosis and management of hypercalcaemia, and recent developments in drug therapy.
METHODS: A non-systematic review of the English-language literature on various aspects in the management of hypercalcaemia was conducted.
RESULTS: Increasingly, more patients with asymptomatic or minimally symptomatic disease are being detected. Appropriate evaluation of hypercalcaemia requires a careful elucidation of the underlying causes, the commonest being p

The use of bisphosphonates is well established for the treatment of bone metastases to reduce skeletal complications in patients with breast and other cancers, to provide a better quality of life. More recently, however, there is evidence that these agents may also have antitumor effects. Therefore, the use of bisphosphonates is now being examined in improving overall survival, by potentially reducing metastatic bone disease in particular. To date, three randomized trials have been conducted in over 1,500 women with breast cancer using 1,600 mg clodronate as adjuvant medication. This article e

Bisphosphonates, analogs of pyrophosphate, bind to bone at sites of active bone remodeling. In clinical settings of rapid bone turnover and/or excessive osteolytic activity, they have been shown to have beneficial clinical effects. These settings include Paget's disease of bone, osteoporosis from a variety of clinical causes, and malignant bone disease. Bisphosphonate inhibition of osteolysis in cancer has been shown to be effective therapy for malignancy-associated hypercalcemia and as adjunctive therapy for the delay or prevention of cancer-related skeletal morbidity, including bone pain, pa

The problem of osteoporosis is world-wide in people above 50 years of age. As this period is also a risk period for the development of multiple myeloma or other malignant processes, comprehensive differential diagnosis of malignant and benign osteoporosis is essential. By retrospective analysis of a 12-year group of 270 patients treated by chemotherapy on account of multiple myeloma the authors selected a group of 151 patients treated in addition to chemotherapy and immunomodulating drugs (mixture of proteolytic enzymes-Wobe Mugos) for 2-3 years, also with biphosphonates. At the time ofdiagnos

The response to the bisphosphonate, pamidronate, is reported in a child with osteogenesis imperfecta who had recurrent symptomatic hypercalcaemia after immobilisation following fractures. Oral clodronate was effective in the prevention of immobilisation hypercalcaemia in the same child. The bisphosphonates may have other roles in osteogenesis imperfecta by decreasing bone turnover.

The bisphosphonates comprise a new class of drugs, and are increasingly being used to treat bone diseases characterised by increased osteoclastic bone resorption. These compounds are generally well tolerated, but toxicity may vary considerably from one compound to another. Dosages of etidronic acid above 800 mg/day impair the normal skeletal mineralisation and this may be associated with the appearance of fractures, but at the doses used for the treatment of osteoporosis, none of the bisphosphonates induce clinical or histological signs of impaired mineralisation. The skeletal half-life of bis
