Advancing treatment strategies for pathological gambling.

Experiences from treatment trials are described. The use of structured instruments for subject characterization and symptom change assessment is discussed and the challenges of treating specific groups (e.g., those with co-occurring alcohol use disorders) explored. Treatment for pathological gambling might be advanced most rapidly through collaborative multi-center efforts.

Pathological gambling caused by drugs used to treat Parkinson disease.

BACKGROUND: Pathological gambling is a rare potential complication related to treatment of Parkinson disease (PD). However, the etiology of this behavior is poorly understood. OBJECTIVE: To examine the relationship between medical therapy for PD and pathological gambling. METHODS: In our routine movement disorders practice (2002-2004), we encountered 11 patients with idiopathic PD who had recently developed pathological gambling. We assessed the relationship to their medical therapy and compared them with cases identified by systematic review of the existing literature on pathological gambli

Lottery playing amongst youth: implications for prevention and social policy.

Factors associated with lottery ticket purchases, accessibility of lottery products, and lottery playing behaviour amongst 1,072 youth (ages 10-18 years old, mean age 14 years-old) was examined. Playing the lottery was found to be the most popular gambling activity with youth reporting playing all forms of lottery tickets including draws, scratch tickets, and sports lottery tickets. Youth reported beginning to play the lottery at age 12, with scratch ticket participation being amongst the most highly reported type of lottery activity with the youngest age of onset. The vast majority of youth a

An evaluation of contingency strength and response suppression.

Typically, functional analyses of severe problem behavior have been conducted in two ways: (a) The target response is reinforced immediately after it occurs, or (b) the target response is reinforced on some schedule thought to mimic a naturally occurring schedule. We evaluated the effects of contingency strength in reducing levels of problem behavior with 2 participants who had been diagnosed with developmental disabilities. Results showed that under a neutral contingency, one in which the probability of reinforcement for aggression was equal to the probability of reinforcement for the nonoccu

Pharmacological treatment of kleptomania and bulimia nervosa.

We describe three patients with concurrent DSM-III-R kleptomania and bulimia nervosa who all demonstrated a partial or complete response of both bulimic and kleptomanic symptoms to fluoxetine, trazodone, or tranylcypromine. These observations raise the possibility that pharmacological treatment may benefit kleptomania, and call for further study of the relationship between kleptomania, bulimia nervosa, and major depression.

Pathological gamblers are neither impulsive nor sensation-seekers.

Ten pathological gamblers, ten alcoholics, ten heroin addicts and twenty five non-patients were compared using Zuckerman's Sensation Seeking Scale (SSS) and Barratt's Impulsivity Scale (BIS). The pathological gamblers did not differ from the non-patient group on either measure. Drug addicts scored significantly higher and alcoholics significantly lower than the pathological gamblers and the non-patient group on the SSS, while drug addicts scored significantly higher than the other groups on the BIS. The difficulties in defining impulsivity are discussed. It is suggested that the classification

Gambling behavior of student-athletes and a student cohort: what are the odds?

This study investigated the prevalence of gambling, gambling related NCAA violations, and disordered gambling in student-athletes (n = 736) with a comparison cohort of students (n = 1,071) at four universities. Student-athletes reported similar rates of gambling frequency, use of a bookmaker, and disordered gambling as students. After accounting for demographic differences, student-athletes were less likely to engage in sports wagering than students. Several risk factors for disordered gambling were identified, including being male and reporting at least one parent with a history of gambling p

Executive dysfunction as a risk marker for substance abuse: the role of impulsive personality traits.

The relationships between family history of substance use, executive functions, impaired decision making, and current substance dependence are the focus of this study. Thirty-eight substance-dependent inpatients were compared with 30 community controls on performance on the Wisconsin Card Sorting Test, Trail-Making Test-B, the Stroop Color Word Test, the Wais-II Digit Span, and the Iowa Gambling Task. Recent alcohol use, depressive symptoms, and impulsivity were also assessed. As hypothesized, individuals with substance dependence exhibited poorer executive functioning. Family history status w

[Is the developmental model accurate for all psychiatric disorders?].

In the 1980's a neurodevelopmental hypothesis of schizophrenia was proposed. Since that time, the role of early developmental age as a risk period for action of etiological factors in psychosis was confirmed. Additionally, many disturbances of pre-schizophrenic persons in the developmental age were demonstrated. Recently published longitudinal cohort studies indicate that majority of the psychiatric disorders other than schizophrenia are preceded by occurrence of psychiatric disorders in childhood and early adolescence. Moreover, in numerous studies of adult anxiety and mood disorders, the rol

Impulse control disorders in Parkinson disease and RBD: A longitudinal study of severity.

OBJECTIVE: To describe the prevalence, natural history, and risk factors for impulse control behaviors (ICBs) among people with Parkinson disease (PD), those with REM sleep behavior disorder (RBD), and controls. METHODS: Participants with early PD (within 3.5 years of diagnosis), those with RBD, and controls were clinically phenotyped and screened for ICBs longitudinally (with the Questionnaire for Impulsivity in Parkinson's Disease). ICB-positive individuals were invited for a semistructured interview, repeated 1 year later. The severity of the ICB was assessed with the Parkinson's Impulse C

Anhedonia in Parkinson's disease patients with and without pathological gambling: a case-control study.

Anhedonia is present in Parkinson's Disease (PD) as well as in addictive behaviors. Pathological Gambling (PG) and other Impulse Control Disorders (ICDs) have emerged as iatrogenic complications associated with dopamine replacement therapy. We studied 154 PD patients, divided into three groups: 11 with PG, 23 with other ICDs (compulsive buying, hypersexuality, binge eating), 120 without ICDs. All patients underwent a thorough clinical, neuropsychological and psychiatric evaluation. The PG-group, compared to the ICDs-group and PD-controls, reported a significantly higher incidence of anhedonia

Iowa Gambling Task Performance in Parkinson Disease Patients with Impulse Control Disorders.

OBJECTIVE: A subgroup of patients with Parkinson disease (PD) develops impulse control disorders (ICD) associated with their dopamine replacement therapy. Patients and their families may be reluctant to report ICD symptoms or unaware these symptoms are related to PD medication, which can make detecting an ICD difficult for clinicians. Ideally, a behavioral measure that is sensitive to ICD could be employed to ensure that patients with these behaviors are identified and treated. The Iowa Gambling Task (IGT), a standardized decision-making task, has proven sensitive in other populations with imp

Pathological gambling subtypes.

Although pathological gambling (PG) is regarded in the 4th edition of the Diagnostic and Statistical Manual of Mental Disorders (American Psychiatric Association, 1994) as a unitary diagnostic construct, it is likely composed of distinct subtypes. In the current report, the authors used cluster analyses of personality traits with a non-treatment-seeking community sample of gamblers and identified 3 PG subtypes. Gamblers partitioned into a simple PG cluster, characterized by low rates of comorbid psychopathology and trait scores near the normative mean; a hedonic PG cluster, characterized by mo

Trichotillomania comorbidity in a sample enriched for familial obsessive-compulsive disorder.

BACKGROUND: This study addresses the strength of associations between trichotillomania (TTM) and other DSM-IV Axis I conditions in a large sample (n = 2606) enriched for familial obsessive-compulsive disorder (OCD), to inform TTM classification. METHODS: We identified participants with TTM in the Johns Hopkins OCD Family Study (153 families) and the OCD Collaborative Genetics Study, a six-site genetic linkage study of OCD (487 families). We used logistic regression (with generalized estimating equations) to assess the strength of associations between TTM and other DSM-IV disorders. RESULTS:

Obsessive-compulsive or addiction? Categorical diagnostic analysis of excoriation disorder compared to obsessive-compulsive disorder and gambling disorder.

Excoriation disorder (ED) is currently classified in the 5th edition of the Diagnostic and Statistical Manual of Mental Disorders' Obsessive-compulsive and Related Disorders section (OCRD). However, there remain debates regarding whether ED is related to obsessive-compulsive disorder (OCD) or whether it is better conceptualized as a behavioral addiction. The present research compared the diagnostic overlap and psychiatric comorbidities of 121 individuals seeking treatment for ED (n = 40), OCD (n = 41) and gambling disorder (GD) (n = 40). ED was more likely to overlap with OCD (n = 14) than GD

Sexual addictions.

UNLABELLED: The potential adverse consequences, personal distress, shame and guilt presented by patients who suffer from sexual addiction require a more in-depth understanding of the phenomenology and psychobiology of this disorder. METHODS: A bibliographic review was conducted using MEDLINE and EBSCO databases with the following keywords: "sexual addiction," "hypersexuality," "compulsive sexual behavior," "behavioural addiction," "treatment," and "addiction." RESULTS: Several conceptualizations of excessive nonparaphilic sexual disorder have been proposed based on the models of, respectivel

The Youth Psychopathic traits Inventory: psychometric properties and its relation to substance use and interpersonal style in a Dutch sample of non-referred adolescents.

This study presents evidence on the reliability and construct validity of the Dutch version of the Youth Psychopathic traits Inventory (YPI), a self-report instrument for psychopathic traits in adolescent boys and girls. In a sample of 776 Dutch non-referred adolescents, the YPI was found to have good internal consistency. Furthermore, exploratory Principal Components Analysis (PCA) indicated a three-factor structure for the YPI, including a Grandiose-manipulative, Callous-unemotional, and Impulsive-irresponsible dimension. Good internal consistency was demonstrated for all dimensions in boys

Examination of gender in pathologic grooming behaviors.

Trichotillomania and pathologic skin picking are pathologic versions of grooming behaviors. Although mentioned in the psychiatric literature for decades, little is known about how gender influences clinical presentation of these behaviors. Seventy-seven adult subjects (12 men) with trichotillomania or pathologic skin picking were examined on a variety of clinical measures including symptom severity, functioning, and comorbidity. There were more similarities than differences between men and women with these behaviors. Some significant differences, however, were that men with grooming disorders
