Anatomical variant of the liver blood supply.

Vascular variations are significant for liver transplantations, radiological procedures, laparoscopic method of operation and for the healing of penetrating injuries, including the space close to the hepatic area. These variants are very common in the abdominal region, and their description will be useful. During a routine dissection of a 73 year old female cadaver, we found in the subhepatic region that the blood supply of the liver differed from a normal one. The difference was found in the absence of the right liver branch and the cystic artery, which normally arises from the common hepatic

Magnetic resonance imaging of suspected cervicocranial arterial dissections.

The authors propose that the optimal screening protocol for evaluation of suspected cervicocranial arterial dissections is magnetic resonance imaging (MRI) that includes three components: 1) contrast-enhanced three-dimensional time-of-flight magnetic resonance angiography (MRA) through the superior mediastinum, neck, and skull base; 2) three-dimensional multiple overlapping thin-section acquisition MRA of the skull base and Circle of Willis region; and 3) axial non-contrast, non-fat-suppressed T1-weighted, fat-suppressed T1-weighted, and T2-weighted spin-echo MRI from the level of the aortic a

Vascular supply of the subcutaneous pedicle of Nagata's method in microtia reconstruction.

Nagata's method is a two-stage method for total ear reconstruction in patients with microtia. In the first stage of this procedure, mastoid flap and posterior lobule flap are elevated with a subcutaneous pedicle. However, contribution to the vascular supply by this pedicle has been controversial. We investigated the presence or absence of apparent vessels in the subcutaneous pedicle in 14 primary cases of microtia in the first stage operation. In all cases some vessels were included in the pedicle. In lobular and small concha type microtia, the vessels originated from the parotid fascia or apo

Single aortic sinus repair with double annular sutures in acute type A aortic dissection: a single center case series of seven patients.

BACKGROUND: We evaluated a modified surgical technique designed to preserve and restore the aortic root geometry in patients with acute type A aortic dissection (ATAAD) when the intimal tear is limited to a single coronary sinus. METHODS: Between January 2015 and December 2021, 160 patients with ATAAD underwent emergency open replacement of the ascending aorta. Most patients (137, 85%) underwent supracoronary ascending aorta replacement. Thirteen patients underwent aorta root replacement with a valved conduit and three underwent valve-sparing aortic root replacement. The remaining seven patie

[Vascular network of the entrance area of the trigeminal nerve root into the brainstem].

High-tech methods of surgical treatment of neural diseases require detailed data on the microanatomy on the base of the brain. Using an original method of dissection of the posterior cranial fossa and brain removal from the skull, the data on microanatomical peculiarities of the entrance area of the trigeminal nerve root into the brainstem were received at autopsies on 48 human subjects. Three variants of arterial blood supply to the trigeminal nerve root and three variants of venous drainage from the cerebellopontine angle are described. The suggested method of posterior cranial fossa dissect

Myocardial infarction and coronary artery dissection during pregnancy associated with type IV Ehlers-Danlos syndrome.

Ehlers-Danlos syndrome (EDS) is a rare, genetically transmitted connective tissue disorder which occurs with an incidence of 1 in 5000. Nine subtypes of EDS have been identified, with type IV being associated with sudden death due to rupture of the bowel, uterus, or major blood vessels. We describe a case of a patient with type IV EDS who presented at 30 weeks' gestation with preterm labor which was treated with subcutaneous terbutaline followed by oral maintenance therapy. Approximately 72 hours later she suffered a myocardial infarction, which led to coronary artery dissection and death. Thi

Clinical and high-resolution magnetic resonance imaging-based prediction of ischemic stroke in cervical artery dissection.

BACKGROUND: Cervical artery dissection (CeAD) is an important cause of ischemic stroke, yet early risk stratification remains challenging. This study aimed to identify clinical and high-resolution vessel wall magnetic resonance imaging features associated with ischemic stroke and to develop a patient-level model for short-term risk prediction. METHODS: A total of 129 patients with CeAD (148 dissected vessels) were retrospectively included. Baseline clinical data and HRMRI features were collected. At the vessel level, least absolute shrinkage and selection operator (LASSO) regression was used

A Simple Technique for Three-Dimensional Imaging and Segmentation of Brain Vasculature U sing Fast Free-of-Acrylamide Clearing Tissue in Murine.

OBJECTIVE: Fast Free-of-Acrylamide Clearing Tissue (FACT) is a recently developed protocol for the whole tissue three-dimensional (3D) imaging. The FACT protocol clears lipids using sodium dodecyl sulfate (SDS) to increase the penetration of light and reflection of fluorescent signals from the depth of cleared tissue. The aim of the present study was using FACT protocol in combination with imaging of auto-fluorescency of red blood cells in vessels to image the vasculature of a translucent mouse tissues. MATERIALS AND METHODS: In this experimental study, brain and other tissues of adult female

The anatomical basis for gastric mobilization in total oesophagectomy.

Following the injection of suitable media, the arterial tree of 22 post-mortem human stomachs was studied by angiography, or corrosion and micro-dissection. In stomachs infused exclusively by the right gastroepiploic artery, i.e. following ligation of all other gastric vessels on the stomach wall, both methods produced good fundal filling, while demonstrating consistently poor filling of a strip of stomach wall along the lesser curve. These findings do not support the assertion that in operations involving mobilization and transposition of the stomach the viability of the fundus is critically

Incidence, predictors, and outcomes of coronary dissections left untreated after drug-eluting stent implantation.

AIMS: Coronary dissections left untreated after percutaneous coronary intervention are associated with unfavourable outcomes. However, their role after drug-eluting stent (DES) implantation is still undescribed. We assessed incidence, predictors, and outcomes of residual dissections in DES-treated lesions. METHODS AND RESULTS: Consecutive patients undergoing DES implantation were enrolled in four Italian centres, with baseline, procedural, and outcome data entered into a dedicated database. Residual dissections were classified according to the National Heart Lung and Blood Institute criteria.

Laparoscopic esophagogastric devascularization in bleeding varices.

BACKGROUND: Bleeding from esophageal varices is the major cause of death in patients with portal hypertension. The ideal surgical procedure should effectively control bleeding and maintain liver function with low rates of encephalopathy. Based on this objective, laparoscopic devascularization of the lower esophagus and upper stomach was studied. METHODS: Eighteen patients were studied prospectively who underwent a laparoscopic esophagogastric devascularization procedure for variceal hemorrhage. The diaphragmatic hiatus and esophagus are dissected. The lower 7 or 8 cm of esophagus is devascula

Case Report: Anomalous Origin of the Right Coronary Artery From the Left Sinus of Valsalva With Aortic Dissection: New Myocardial Ischemia Mechanism.

The aortic anomaly of the right coronary artery (AAORCA) originating from the left aortic sinus (LCS) is a rare malformation that may result in sudden cardiac death (SCD), which may be due to the dilated aorta-pulmonary artery affecting the blood supply of the coronary artery. However, there are still some disputes about the treatment of the AAORCA. Herein, we present a rare case of AAORCA from the LCS with aortic dissection (AD). Considering the risk of dissection rupture and SCD, an emergency surgery of aortic replacement and coronary anomaly correction was performed successfully for the pat

Microvascular Doppler Assisted Microsurgical Denervation of the Spermatic Cord in Treating Chronic Orchialgia.

Chronic orchialgia is a common disease in department of urology and andrology. The etiology is complex, and the treatment is difficult. In severe cases, orchiectomy is even necessary. In recent years, microsurgical denervation of the spermatic cord (MDSC) is a minimally invasive and effective surgical method for the treatment of chronic orchialgia. Its greatest advantage is to preserve the testis and epididymis, avoid the possible organ resection. The key of the operation is to dissect all the fibrous tissues in the spermatic cord, while protecting the arteries (especially the testicular arter

Left Ventricular Failure Despite Patent Coronary Arteries in Acute Type A Aortic Dissection.

Severe myocardial failure after acute type A aortic dissection is usually attributed to structural coronary malperfusion. We report catastrophic global left ventricular failure despite preserved epicardial coronary anatomy, successfully bridged with durable mechanical circulatory support. A 57-year-old man presented with acute type A dissection and cardiogenic shock with extensive precordial ST-segment elevation. Emergency total arch replacement with a frozen elephant trunk was performed. Intraoperative inspection and postoperative coronary angiography confirmed patent coronary ostia and arter

Robot-assisted donor hysterectomy in uterus transplantation - a modality to increase reproducibility.

Uterus transplantation is a non-lifesaving vascularized composite allotransplantation procedure requiring immunosuppression until removal of the graft. The focus of uterus transplantation is changing regarding refining individual treatment procedures included in this complex treatment of absolute uterine factor infertility, such as robot-assisted donor hysterectomy. The inferior hypogastric nerve plexus should be preserved during robotic dissection of the ureter and uterine vessels to prevent postoperative complications such as urine and fecal evacuation disturbances and sexual disorders. As m

Holographic image-guided thoracoscopic surgery: possibility of usefulness for esophageal cancer patients with abnormal artery.

BACKGROUND: With the recent improvement of medical image analysis technology, three-dimensional (3D) holograms technology is beginning to be used as intraoperative image support. CASE PRESENTATION: We used a wearable holographic computer during thoracoscopic esophagectomy in a 70-year-old man with esophageal cancer. During lymph node dissection around the right recurrent laryngeal nerve, abnormal blood vessels were observed beside the right subclavian artery (RSA). As a result of confirming the anatomical variation of the right vertebral artery (RVA) using the 3D holograms, it was possible to

Ocular manifestations of internal carotid artery dissection.

Internal carotid artery dissection (ICAD) results from disruption of the intima of the arterial wall, and can lead to intrusion of blood into the arterial wall and form an intramural hematoma. The hematoma can compress the true lumen of the vessel, causing functional stenosis or occlusion. The classic triad signs of ICAD include pain in the ipsilateral neck, head and orbital regions; a (partial) Horner syndrome; and cerebral or retinal ischemia. However, not all ICAD patients present with this classic signs. In some cases, ocular manifestations are the initial (and sometimes the only) findings

Antecedent Bile duct Cutting in the Glissonean pedicle technique (ABC technique) for extremely advanced perihilar cholangiocarcinoma.

BACKGROUND: In an increasing number of patients undergoing radical surgery for perihilar cholangiocarcinoma [1-3], the intrahepatic bile duct is conventionally transected after the vessels to be preserved or reconstructed are confirmed [3,4]. In patients with extremely advanced perihilar cholangiocarcinoma having massive vascular involvement, it is sometimes difficult to confirm the vessels for reconstruction because of restricted working space and/or anatomical variants, even after liver parenchymal dissection [4]. When the vessels cannot be confirmed, the tumor is usually unresectable [4].

Entrapment of the Subscapular Artery between the Radial Nerve and the Posterior Wall of the Axilla: An Anatomical Variation with Clinical Significance.

OBJECTIVE: The subscapular artery vascularizes a substantial region of the thoracic wall, and the significance of its distribution is well depicted in the diversity of reconstructive procedures that rely on its blood supply. The aim of this study is to present an uncommon anatomical variation of the artery and discuss the clinical implications of its presence. CASE REPORT: This case report depicts a rare variant of compression and the kinking of the subscapular artery by the radial nerve on the posterior wall of the axilla that was encountered during dissection of a male cadaver of Greek orig
