PURPOSE: Diverticulitis has been described as a more virulent disease in young patients, necessitating an aggressive surgical approach. Colouterine fistulae secondary to sigmoid diverticulitis are unusual. Methods for diagnosis remain to be established. We report a case with a colouterine fistula in which sonohysterography detected the flow of ultrasound contrast medium between the uterine cavity and the sigmoid colon through the posterior uterine wall, thus confirming the diagnosis. PURPOSE: Colonic diverticula are saccules made of colonic mucosa and submucosa that herniate from the colonic lumen through the muscular layer of the wall where straight vessels penetrate. Congenital fistulae between the biliary tract and the proximal foregut are rare. Acute colonic diverticulitis is usually suspected by typical clinical findings supported by laboratory tests. Investigations of the colon are usually delayed 1 to 2 months until the acute situation is resolved. We studied 53 patients with an initial clinical diagnosis of acute diverticulitis by performing early water-soluble contrast enema of the colon. Small bowel diverticulosis occurs in 0.3-4.4% of all autopsies. Generally the condition is clinically silent; but acute diverticulitis with peritonitis and paralytic ileus may occur in elderly people. This complication has a high mortality and its successful management requires early diagnosis and surgery.

Eighteen patients with abdominal symptoms clinically resembling acute colonic diverticulitis were studied with 99mTc-labelled leucocytes on average 5 days after the onset of symptoms. The diagnosis of diverticular disease was confirmed in all cases by means of barium enema or sigmoidoscopy. The leucocyte scan was positive in 13 patients, nonspecific in four and unclear in one patient. The diagnostic possibilities in acute colonic diverticulitis are discussed.

Hepatic portal venous gas (HPVG) in the adult is usually associated with bowel necrosis. Together these have an 80% mortality. However, HPVG may occur as a result of a variety of other pathologic conditions. We studied what we believe is the second known case resulting from sigmoid diverticulitis. This patient's survival was unexpected, because bowel perforation and pathologically demonstrated septic phlebitis occurred during the patient's long-term corticosteroid therapy.

Diverticular disease may mimic many of the symptoms and signs of Crohn's disease. The presence, however, of perirectal involvement and a longitudinal fistula greater than 10 cm. in the wall of the colon are two unusual features of diverticular disease. Discussed herein is a patient who presented with both of these complications.
