An international study on emerging arboviral infections and blood safety.

BACKGROUND: Emerging and re-emerging arboviral infections are a risk to blood safety. We conducted an international survey on how blood establishments respond to current and future arbovirus threats. STUDY DESIGN AND METHODS: A questionnaire on arbovirus donor deferral strategies, pathogen reduction, and donation screening was distributed to members of the International Society of Blood Transfusion working party on transfusion-transmitted infectious diseases. Data from 2024 were gathered and analyzed. RESULTS: A total of 23 survey responses were received from 21 countries. This covered a pop

Liver Discard Rate Attributable to Conservative Estimations of Steatosis: An Inference-based Approach.

BACKGROUND: On-site conservative estimations of steatosis could result in the unnecessary discard of donor livers. This study applied the body mass index as an independent statistical indicator to determine the extent of this problem. We aimed to quantitatively evaluate if decisions based nonbiopsy donor liver assessments are more conservative (inclined to reject marginal fatty livers) than biopsy-based evaluations. METHODS: The study processed intradatabase comparisons using 177 081 datasets from Organ Procurement and Transplantation Network spanning 2004 to 2022 September in the United Stat

Evaluating outcomes of preemptive kidney transplant in patients over 65 using high-KDPI kidneys compared to non-preemptive recipients.

BACKGROUND: Utilizing kidneys with a Kidney Donor Profile Index (KDPI) score above 85 could be beneficial, especially in End-Stage Kidney Disease (ESKD) patients older than 65. Our study aimed to assess the outcomes of preemptive kidney transplants (KT) in these patients over 65 receiving high KDPI kidneys (≥ 85) and compare the outcomes to the non-preemptive kidney recipients. METHODS AND MATERIALS: This retrospective cohort analyzed the SRTR database for KTs that were performed in the United States from January 2000 to May 2023. This study includes patients older than 65 years old who recei

Attitudes Toward Use of an APOL1 Genetic Testing Chatbot in Living Kidney Donor Evaluation: A Focus Group Study.

BACKGROUND: Living kidney donor (LKD) candidates of African ancestry are increasingly asked to undergo Apolipoprotein L1 (APOL1) genetic testing during the donor evaluation process to better understand their risk of kidney disease. LKD candidates' attitudes about using a clinical chatbot on APOL1 remain unknown. This study builds on prior work to culturally adapt the Gia (Genetic Information Assistant) chatbot on APOL1 by assessing donor, recipient, and community member attitudes about the Gia chatbot for enhancing the integration of APOL1 testing into the LKD clinical evaluation workflow. Triaging donor lungs based on a microaspiration signature that predicts adverse recipient outcome.

BACKGROUND: Aspiration is a relative contraindication to accepting donor lungs for transplant and is currently assessed by visual inspection of the airways via bronchoscopy. However, this method is limited as it does not assess for microaspiration. Bile acids measured in large airway bronchial wash (LABW) samples have been shown to be a marker of aspiration in lung transplant recipients. Herein, we investigate the utility of measuring total bile acids (TBA) in donor LABW to predict performance of donor lungs and recipient outcomes. METHODS: TBA was measured in 605 consecutive lung donors at t

The Benefits of Preemptive Transplantation Using High-Kidney Donor Profile Index Kidneys.

BACKGROUND: The Kidney Donor Profile Index (KDPI) is a percentile score summarizing the likelihood of allograft failure: A KDPI ≥85% is associated with shorter allograft survival, and 50% of these donated kidneys are not currently used for transplantation. Preemptive transplantation (transplantation without prior maintenance dialysis) is associated with longer allograft survival than transplantation after dialysis; however, it is unknown whether this benefit extends to high-KDPI transplants. The objective of this analysis was to determine whether the benefit of preemptive transplantation exten

Significance of ambulatory blood pressure monitoring in assessment of potential living kidney donors.

The most recent British Transplant Society (BTS) guidelines recommend that office blood pressure (BP) monitoring in living donors is sufficient for the assessment of hypertension (HTN) and those with BP >140/90 should be further assessed using ambulatory BP monitoring (ABPM). ABPM can detect diurnal and nocturnal variation in BP, thus it can identify masked HTN. The aim of the current study is to assess reliability of ABPM vesus office BP monitoring for assessment in living kidney donors. Office and ABPM of all potential kidney donors at a single center from April 2009 to March 2017 were retro

Socio-epidemiological characterisation of blood donors with asymptomatic Leishmania infantum infection from three Brazilian endemic regions and analysis of the transfusional transmission risk of visceral leishmaniasis.

OBJECTIVES: This study assessed the prevalence of visceral leishmaniasis in blood donors from three endemic regions in Brazil and evaluated the risk of transmission by transfusion. BACKGROUND: Despite strong evidence of the transmission of visceral leishmaniasis through blood transfusion, the real risk, an essential condition for taking effective measures to control this serious disease, has not been determined. METHODS: A multicentre study was performed in highly endemic areas. Candidates eligible for their first blood donation underwent a socio-epidemiological interview, and blood samples

Diagnostic evaluation of military blood donors screening positive for Trypanosoma cruzi infection.

Routine blood donor screening for Trypanosoma cruzi, the causative parasitic agent of Chagas disease, began in the U.S. in 2007. Results of follow-up testing and evaluation after a positive screen have not been studied in the armed forces. Among first-time donors at the Joint Base San Antonio- Lackland Blood Donor Center between January 2014 and December 2016 (N=43,402), a total of 23 (0.05%) screened positive for T. cruzi. This descriptive study highlights demographic and follow-up information for all 22 active duty service members who screened positive; a non-active duty member was excluded

Donor biopsy in living donor liver transplantation: is it still relevant in a developing country?

Liver transplantation is an important modality of treatment for end-stage liver disease. Liver biopsy evaluation has been an important aspect of the donor evaluation protocol. With the advent of newer modalities of donor evaluation such as high resolution CT scan, fibroscan and NMR spectroscopy, the relevance of the liver biopsy appears to be diminishing. We investigated the usefulness of donor liver biopsy evaluation in patients who had been cleared by radiological investigations. We evaluated 184 donor liver biopsies performed over a one-year period and found that 18% showed >5% steatosis an

Evaluation of Potential Donors in Living Donor Liver Transplantation.

INTRODUCTION: Correct donor selection in living donor liver transplantation (LDLT) is essential not only to decrease the risks of complications for the donors but also to increase the survival of both the graft and the recipient. Knowing their most frequent reasons of donor elimination is so important for transplantation centers to gain time. In this study we evaluated the effectiveness of potential donors in LDLT and studied the reasons for nonmaturation of potential liver donors at our transplantation center. PATIENTS AND METHODS: We studied the outcomes of 342 potential living donor candid…

Resolution of HLA-B*44:02:01G, -DRB1*14:01:01G and -DQB1*03:01:01G reveals a high allelic variability among 12 European populations.

Within the framework of the EU-funded HLA-NET action, an analysis of three G-group alleles, HLA-B*44:02:01G, DRB1*14:01:01G and DQB1*03:01:01G, was undertaken in 12 European populations. Ambiguities were resolved by polymerase chain reaction-sequence-specific amplification (PCR-SSP) or PCR-sequence-based typing (PCR-SBT) in a total of 5095 individuals. The results of the DRB1*14:01/14:54 ambiguity showed high relative ratios (24-53%) of DRB1*14:01 in Bulgarians, Croatians, Greeks, Italians and Slovenians, contrasting with low ratios (6-13%) in Austrians, Finnish, French, Hungarians, Norwegians

Screening pigs for xenotransplantation: prevalence and expression of porcine endogenous retroviruses in Göttingen minipigs.

BACKGROUND: To establish the safety of xenotransplantation when cells, tissues, or organs of pigs are used, an effective screening for potential zoonotic microorganisms has to be performed. In doing so, special attendance has to be paid to porcine endogenous retroviruses (PERVs) that are widely distributed as proviruses in the genome of pigs. PERV-A and PERV-B are present in all pigs, they infect human cells in vitro and therefore represent a direct risk. PERV-C infects only pig cells; however, recombinant PERV-A/C infecting human cells and replicating at a higher rate were found in pigs indic

Pathology consultation on transfusion-related acute lung injury (TRALI).

Transfusion-related acute lung injury (TRALI) is a serious condition characterized by respiratory distress, hypoxia, and bilateral pulmonary infiltrates, which occur within 6 hours of transfusion. Several theories have been proposed to explain the underlying pathologic mechanisms of TRALI. Immune-mediated TRALI accounts for over 80% of reported cases and is mediated by donor antibodies to HLAs and/or human neutrophil antigens (HNA). Immune-mediated TRALI is most commonly associated with donor plasma transfusion or other blood products from multiparous women, which has led many countries to red

The outcome of donor screening for human T-cell lymphotropic virus infection in The Netherlands.

BACKGROUND AND OBJECTIVES: Blood donor screening reduces the infectious hazards related to blood transfusion, but the range of agents to be screened for is debatable. In 1993, the screening of all blood donations for Human T-Cell Lymphotropic virus (HTLV) was introduced in The Netherlands. We analysed the outcome and costs of HTLV donor screening. METHODS: For the years 2001-2010, the number of HTLV infections among new and regular donors was used to estimate the prevented number of HTLV-infected donors in the donor pool and the amount of morbidity prevented among recipients. RESULTS: Human

Profunda Artery Perforator Flap for Autologous Breast Reconstruction: A Review of the Evidence and the Linkou Chang Gung Memorial Hospital Experience.

This study aimed to summarize and highlight institutional experience on the profunda artery perforator (PAP) flap for autologous breast reconstruction. Contemporary series demonstrate high flap success and patient satisfaction with PAP reconstruction. Key technical evolutions include perforator-targeted skin paddle designs, strategies to augment venous outflow when needed, and standardized donor site management. Comparative studies and systematic reviews suggest reconstructive reliability comparable to other autologous options, while donor site wound morbidity remains the main area for optimiz
