[Congenital metatarsus varus].

The authors report on "congenital primitive metatarsus varus". After discussing the incidence, etiology, pathogenesis and classification, the clinical features and associated malformations are evaluated. Then they go into treatment details affirming, according to the literature, that this malformation is susceptible to complete regression if treated bloodlessly during the early months of life, and in any case, before the onset of loss of ambulation, by manipulations followed by a series of plaster casts and finally by corrective braces and shoes. If this treatment is fruitless or in cases seen

Segmental spinal instrumentation in the management of fractures of the thoracic and lumbar spine.

Eleven cases of fracture of the thoracic and lumbar spine were treated with segmental spinal instrumentation (Luque rods). The majority of these injuries occurred in motor vehicular accidents or falls from a height and were usually associated with multisystem trauma. This method of instrumentation provided immediate rigid internal fixation, which obviated the need for postoperative orthoses, allowed easier nursing care and early ambulation, and facilitated rehabilitation of patients with spinal cord injury. At 13 months' average follow-up, there have been no instances of wire breakage, rod def…

Bleeding avoidance strategies during percutaneous coronary interventions.

Bleeding avoidance strategies for percutaneous coronary interventions continue to evolve with the availability of newer antiplatelet and ant … lar access and different anticoagulation regimens. Vascular closure devices have long been used for early ambulation; however, more recent results demonstrating lower bleeding complications from observational registries are encouraging. This review synthesizes this information, taking into account changes in the landscape of interventional practice with respect to …

Osteochondritis dissecans: a historical review and its treatment with cannulated screws.

The etiology of osteochondritis dissecans and the results of treating the early stages with arthroscopic fixation using cannulated screws is discussed. Arthroscopic surgery was performed on 14 patients with osteochondritis dissecans, and the osteochondral fragment was fixed with one or two screws. A second arthroscopic procedure was necessary to assess the lesion and remove the screws. Ambulation without weight bearing is allowed during the first 2 months postoperatively. Full range of motion is encouraged. The results indicate that all patients returned to their previous sport 3-11 months pos

Subtrochanteric fracture: fixation using the AO tibial nail.

We describe an alternative fixation for the pure uncomminuted subtrochanteric fracture by using the AO tibial nail. There were no intraoperative complications in 24 fractures with this method. The fractures healed within 4 months postoperatively in 18 non-pathological fractures that could be followed up. There were minor complications, including 10 degrees external malrotation in one case and varus angulation after nail breakage in another. The nails were removed in seven healed fractures without problems. Two pathological fractures which were stabilized by the AO tibial nail, combined with th

One thousand cases of prostatectomy.

A survey of one thousand transvesical prostatectomies was conducted for studying occasional complications. An improved technique is recommended for the removal of adenomas, 30-40 g in weight or above. The number of complications showed a decline which, together with the possibility to reduce the operating and nursing periods, is attributed to some outstanding features of the method such as the use of continuous running haemostatic synthetic sutures for constricting the edges of the prostatic bed, the use of a three-bore catheter with its balloon located inside the bladder, rinsing the bladder

[Appendicitis in late pregnancy].

Appendicitis in pregnancy is correlated with high morbidity especially for the child. However, the diagnosis is difficult to determine due to subtle signs. This is a case report of a pregnant woman who despite being past due date and having progressive abdominal pain was sent home twice before arriving by ambulance severely affected by a perforated appendix. Abdominal pain and urine leukocytosis were the only early indicators. Foetal asphyxia led to caesarian section disclosing the diagnosis. When pregnant women without signs of ongoing birth have emerging abdominal pains, appendicitis should

Emergency management of acute asthma.

BACKGROUND: Life threatening asthma is not an uncommon emergency and all doctors need to have an ordered approach to managing this problem both in the surgery and the home. OBJECTIVE: To develop a plan for the rapid assessment and management of a patient with asthma. The main focus is on severe life threatening asthma and management is based on the equipment and medication a doctor is likely to have available in the surgery. CONCLUSION: The key to successful treatment of life threatening asthma is a rapid assessment of severity, an early emergency call for an ambulance, oxygen, continuous ne

Total hip arthroplasty to treat congenital musculoskeletal abnormalities in the juvenile Down Syndrome hip: review of literature with case.

Down Syndrome can result in musculoskeletal abnormalities of the hip at an early age. Avascular necrosis of the femoral head can occur as a result of slipped capital femoral epiphysis causing the patient a great deal of pain, limiting the ability to ambulate. Despite the benefits that this patient group can receive from the surgery, surgeons may be apprehensive to operate. It is our experience that these patients benefit greatly from arthroplasty without complication. In this report, we present a total hip replacement to treat avascular necrosis in an adolescent and address the concerns that s

Impact of surgical intervention timing on the case fatality rate for Fournier's gangrene: an analysis of 379 cases.

UNLABELLED: Study Type - Prognosis (outcome) Level of Evidence 2b. What's known on the subject? and What does the study add? Reportedly, Fournier's gangrene has a high mortality rate, ~7.5-40%, and experts recommend early surgical debridement. This study examines 379 patients and shows that an early intervention, i.e. within 2 hospital days could halve the mortality rate compared with later intervention. OBJECTIVE: • To examine how early surgical intervention influenced cases of Fournier's gangrene (FG) fatality. PATIENTS AND METHODS: • Patients with FG (defined as an International Classific

CLN2 Disease: Current Understandings, Challenges, and Future Directions.

Neuronal ceroid lipofuscinosis type 2 (CLN2) disease is a rare neurodegenerative condition that rapidly progresses with language regression, loss of ambulation, blindness, intractable seizures, and premature death in childhood. Enzyme replacement therapy has transformed the clinical trajectory of CLN2 disease, and early genetic testing is crucial because enzyme replacement therapy cannot reverse clinical progression. Lack of clinician awareness of early clinical symptomatology, initially normal language development, and history of provoked or treatment-responsive seizures may contribute to dia

Graves' Disease With Initial Presentation of Thyrotoxic Periodic Paralysis.

Graves' disease is a common cause of hyperthyroidism. However, thyrotoxic periodic paralysis (TPP) is a rare complication of Graves' disease and is characterized by episodes of muscle weakness and hypokalemia in the setting of thyrotoxicosis. Episodic weakness and paralysis can be the first manifestation of Graves' disease with TPP despite lacking classic symptoms of hyperthyroidism and can be precipitated by risk factors such as a high carbohydrate diet and strenuous exercise. Although TPP is reversible with correction of hypokalemia and thyrotoxicosis, its uncommon presentation can lead to d

Robust immune cell infiltration and macrophage senescence occur within a week of recovery after limb immobilization in older adult skeletal muscle.

Immune cells are critical for modulating inflammation and extracellular matrix remodelling for effective muscle regrowth following disuse atrophy. However, disrupted macrophage function and accumulation of cellular senescence may limit muscle recovery in ageing. The present study aimed to compare changes in the cellular dynamics of muscle macrophages, cellular senescence and collagen content during early recovery following 14 days of unilateral limb immobilization in young (n = 18; ∼24 years) and older male and female adults (n = 18; ∼69 years). Participants underwent 14 days of immobilization

Pan-immune-inflammation value as a predictor of loss of ambulation in duchenne muscular dystrophy: a retrospective cohort study.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a progressive X-linked neuromuscular disorder marked by early functional decline and considerable variability in the timing of loss of ambulation (LOA). Readily accessible biomarkers to predict this decline remain limited. The pan-immune-inflammation value (PIV), derived from routine blood counts, has been studied in various inflammatory conditions, but its relevance in DMD is not yet well defined. METHODS: This retrospective cohort study included 86 children and adolescents with genetically or biopsy-confirmed DMD followed between 2010 and 2025

Effect of Bilateral Ultrasound-Guided Deep Parasternal Intercostal Plane Block Before Extubation on Early Recovery After Cardiac Surgery via Median Sternotomy: A Randomized Controlled Trial.

INTRODUCTION: Postoperative pain remains a major concern after cardiac surgery via median sternotomy and may impair early recovery. Deep parasternal intercostal plane (DPIP) block is a regional analgesic technique targeting the parasternal intercostal nerves, but the effect of DPIP block performed before extubation on postoperative recovery has not been well defined. We therefore evaluated whether bilateral ultrasound-guided DPIP block administered before extubation could improve early recovery after median sternotomy cardiac surgery. METHODS: In this single-center, prospective, randomized, s

Effects of early photobiomodulation therapy on motor recovery in patients with incomplete acute cervical spinal cord injury: A multicenter randomized controlled trial.

BACKGROUND: Early neurological recovery following acute incomplete cervical spinal cord injury (SCI) is associated with better long-term function. Photobiomodulation (PBM) has shown neuroprotective effects in preclinical studies, but multicenter clinical evidence remains limited. METHODS: In this multicenter randomized controlled trial, adults (18-70 years) with American Spinal Injury Association (ASIA) grade C cervical SCI presenting within 24 h were enrolled at three tertiary hospitals and randomized to PBM or control. All patients underwent posterior decompression within 24 h. The PBM grou

Continuous Wearable-Sensor Monitoring After Colorectal Surgery: A Systematic Review of Clinical Outcomes and Predictive Analytics.

Background and Objectives: Early ambulation and timely detection of postoperative complications are cornerstones of colorectal Enhanced Recovery After Surgery (ERAS) programmes, yet the traditional bedside checks performed every 4-8 h may miss clinically relevant deterioration. The consumer wearables boom has spawned a new generation of wrist- or waistband-mounted sensors that stream step count, heart-rate and temperature data continuously, creating an opportunity for data-driven early-warning strategies. No previous systematic review has focused exclusively on colorectal surgery. Methods: Thr…
