Lack of knowledge of the socioeconomic impact of various diseases is generally one of the biggest obstacles to obtaining funds for investment in applied research and for starting to implement control programs that are needed in developing countries. PURPOSE OF REVIEW: Chemotherapy against onchocerciasis and lymphatic filariasis has been discussed mainly within the framework of mass drug administration with diethylcarbamazine, ivermectin and albendazole. The relation between infection and disease in lymphatic filariasis is still imperfectly understood. This paper presents baseline data on microfilaraemia, oedema and acute episodes from a drug trial against bancroftian filariasis in Papua New Guinea. Among 2,187 people with data on these manifestations, 1,144 were microfilaraemic, 78 had leg oedema, 356 had acute episodes and 919 were negative for all three. Geographic information system (GIS) was used to analyze the spatial distribution of filariasis in the Nile Delta. The study involved 201 villages belonging to Giza, Qalubiya, Monoufiya, Gharbiya, and Dakahliya governorates. Components of vectorial capacity (biting density, survival, and host preferences) of a population of Aedes (Finlaya) poicilius, the principal vector of Wuchereria bancrofti in the Philippines, were studied in 1994-1995 in Sorsogon province. A study was conducted to determine the susceptibility of local strains of Culex quinquefasciatus and Aedes aegypti to infection with the strain of Wuchereria bancrofti that occurs in Maceió. State of Alagoas, Brazil. Cx. quinquefasciatus blood fed simultaneously on the same microfilariae carrier ingested more blood and 2-3x more microfilariae than Ae. aegypti. In patients with filarial lymphoedema of the limbs, infestation by maggots is extremely rare. We saw three patients with lymphoedema who harboured Chrysomyia bezziana in leg ulcers and in one of them 128 maggots were recovered. A course of local dressing, antibiotics and anti-inflammatory drugs resulted in healing of the wounds. Ulcers in patients with lymphoedema should be carefully tended with cleaning and dressing otherwise myiasis may supervene.
