OBJECTIVE: Bilateral Ménière's disease is classified according to the time of appearance of symptoms in each ear into synchronous and metachronous types. A descriptive longitudinal study, involving 59 bilateral Ménière's disease patients, was carried out to assess the two forms of bilateral Ménière's disease.
METHOD: Data on symptomatic chronology in each ear, auditory evolution and evolution of vertiginous crisis, among other aspects, were obtained, analysed and compared. Possible risk factors for Ménière's disease becoming bilateral were analysed after conducting nested case-control studies

By idiopathic sudden deafness is meant solely sudden sensory loss of hearing, the pathological-anatomical substrate of which is thought to be acute, vascular endolymphatic hydrops of unknown cause. Sudden deafness always affects one ear only, tinnitus and temporary vertigo may be accompanying symptoms. Sudden loss of hearing reflecting an inner ear involvement of general diseases must be distinguished from sudden deafness as defined above, as also from psychogenic deafness, central-neural deafness during the course of multiple sclerosis, sensory hearing loss in Cogan's disease or syphilis. The…

The membranous labyrinth of the inner ear is a complex network of endolymph-filled structures critical for auditory and vestibular function. Pathological distension of these spaces, termed endolymphatic hydrops (EH), is associated with disorders such as Ménière's disease (MD). However, diagnosing inner ear pathologies remains challenging due to limitations in traditional imaging techniques, which lack the spatial resolution required to assess these intricate structures. Advances in 7-Tesla (7T) magnetic resonance imaging (MRI) now allow for high-resolution visualization of the inner ear. In th…

OBJECTIVES: Acute low-tone sensorineural hearing loss (ALHL) is thought to have a different etiology from that of idiopathic sudden sensorineural hearing loss. We hypothesized that endolymphatic hydrops (EH) in the inner ear organ contributes to ALHL, even in patients without vertigo. This study investigated the presence of EH in ALHL and compared the clinical characteristics of patients with or without EH.
METHODS: We retrospectively reviewed 38 patients diagnosed with ALHL without vertigo from January 2017 to March 2022. EH was measured in all patients using inner ear magnetic resonance ima…

OBJECTIVE: To study differences between electrocochleography (ECochG) and cochlear hydrops analysis masking procedure (CHAMP) in diagnosis o … 082) results.
CONCLUSIONS: ECochG is a more sensitive test than CHAMP in detecting the presence of endolymphatic hydrops in Ménière's patients. CHAMP covaried interaurally in unilateral Ménière's patients whereas ECochG did not.

Luetic hydrops can occur in both congenital and acquired lues and otologic manifestations may be indistinguishable from Meniere's disease. Eight endolymphatic sac decompression procedures were performed on 6 patients with refractory luetic vertigo. Of those, 3 had congenital and 3 acquired lues. All cases had received extensive anti-luetic medical therapy without any discernible improvement in the vertiginous attacks. In our 6 patients with luetic hydrops the main complaint was severe refractory vertigo, indeed, disabling in most cases. Postoperatively, all cases remained free from these sever

A multicentre study of the inner ears of an 88-year-old patient with vertiginous spells and severe hearing loss in the left ear was performed, employing regular and block surface preparations, light and electron microscopy with qualitative and quantitative evaluation of the cochlear and vestibular nerves. There was severe hydrops of the left cochlea and saccule. Reissner's membrane extended into the vestibule and herniated into the perilymphatic space of the non-ampullated end of the horizontal canal. Furthermore, the short canal connecting the posterior ampulla with the utricle had a small, e

PURPOSE OF REVIEW: Meniere disease has long been postulated to be a disorder of intralabyrinthine fluid dynamics.
RECENT FINDINGS: More recent developments in this field indicate that the control of fluid movement may be at a cellular level and that hormonal influence may be important.
SUMMARY: The control of fluid and ion movements through aquaporins and gap junctions in the cell membranes are creating new perspectives in the mechanism of development of endolymphatic hydrops as well as potential methods for treatment. Intralabyrinthine fluid dynamics also play a role in the ability to local…

In a previous report it was suggested that intravenous administration of furosemid followed by a comparative caloric test (the furosemid test) could be utilized for detection of endolymphatic hydrops (Kitahara et al., 1973). As a result of the further application of the furosemid test upon 161 patients with vertigo, positive results were observed in 80% of patients with typical Menière's disease, 6% of atypical Menière's disease, 42% with labyrinthine syphilis and 27% with sudden deafness. In patients with labyrinthitis, results were negative. It is thus concluded that the furosemid test provi…

Most patients with suspicion of hydrops do not have access to MRI with 3D reconstruction of the endolymphatic space. Our main objective was to show that measurements of the saccule on a non-enhanced 3D-T2 MRI could show hydrops and help diagnose Menière disease. We conducted a prospective study from 2015 to 2016 to compare consecutive patients consulting for Menière's disease to a control group (patients with unilateral non-hydrops disorders and contralateral healthy ears). They all received full auditory and vestibular testing. They also underwent a 3-Tesla 3D-T2 MRI using CISS sequence (0.4

PURPOSE: To compare the clinical characteristics of patients with early and late-onset Ménière's disease (MD) and to investigate the impact … f a vertigo episode were statistically different between the two groups. The severity of vestibular endolymphatic hydrops, abnormal rate of canal paresis (CP) value of the caloric test, total DHI score, and VAS score were all higher in the late-onset group. PHQ-9 and GAD-7 scores were significantly correlated with total DHI score in the early-onset group.
CONCLUSION: …

INTRODUCTION: Endolymphatic hydrops is the pathophysiological substrate of Ménière's disease. The changes in the inner ear, transmitted to the middle ear through changes in the ossicular chain mobility, can be quantified by wideband tympanometry, through the measurement of the acoustic absorbance at multiple frequencies, represented by the sound energy absorbed by the middle ear, even at its early stages. Studying the behavior of the middle ear through the absorbance in patients with endolymphatic hydrops under ambient pressure and under peak pressure can be useful for detecting Ménière's dise…

OBJECTIVES: A three-dimensional (3D) inversion-recovery (IR) sequence with real reconstruction (3D-real IR) sequence 4 hours after intravenous (IV) gadolinium injection has been used to visualize the endolymphatic hydrops (ELH) in Ménière's disease (MD). This study was designed to investigate the ELH characteristics in clinically diagnosed definite MD and to explore the pathology of MD with magnetic resonance imaging (MRI).
METHODS: One hundred fifty-four patients with definite MD were included in this study. All patients underwent the IV method. The grades of ELH were analyzed on each image,…

OBJECTIVE: Auditory disorders of the contralateral ear in patients with unilateral acoustic neurinoma have previously been reported using th … ings. One possible cause may be dysfunction of the olivocochlear efferent system and another may be endolymphatic hydrops (e.g. delayed endolymphatic hydrops). Currently, however, the causes of -SP/AP ratio increase are still unknown and further investigations are required.
