Co-existence of lip and epiglottis Kimura's disease.

Kimura's disease is a rare, chronic inflammatory condition of unknown etiology; with a predilection in the head and neck region. However, the involvement of the lip and epiglottis is extremely rare, and poses a diagnostic challenge. Here, we report a case of a 32-year-old Saudi male presenting with lip mass and epiglottic swelling without any history of hoarse voice or airway compromise. Serology showed elevated immunoglobulin E levels, and histopathological examination of biopsied lesions revealed well-developed lymphoid follicles with eosinophilic infiltration confirming Kimura's disease. The primate hyolaryngeal apparatus and herbivorous modifications.

The predominantly herbivorous diet of the howler and baboon is responsible for modification in the configuration of the hyolaryngeal apparatus. In a herbivorous primate the air sac located in the concavity of the body of the hyoid bone takes on a resonating function. There is definite correlation between : (1) the presence of a large epiglottis with a convex superior surface, (2) close apposition of the soft palate with the epiglottis, (3) presence of high aryepiglottic and lateral epiglottic folds, (4) presence of air sacs and (5) concavity of the posterior surface of the body of the hyoid bo

DISE and Rhinomanometry Correlates of Response to Head-of-Bed Elevation in Obstructive Sleep Apnea.

OBJECTIVES: To identify endoscopic and rhinomanometric correlation of response to 30° head-of-bed elevation (HOBE) in patients with obstructive sleep apnea (OSA), and to explore whether upper airway (UA) collapse patterns and nasal resistance are associated with positional improvement. METHODS: We assessed the associations between drug-induced sleep endoscopy (DISE) findings and rhinomanometric parameters with response to HOBE in OSA patients. Polysomnography was performed overnight in standardized supine, 30° HOBE, and lateral positions. HOBE response was evaluated based on percent change in

Experience in reconstruction of esophagus, epiglottis, and upper trachea due to caustic injuries in pediatric patients and establishment of algorithm.

BACKGROUND: Unintentional swallowing of corrosive agents cause problems in the pediatric population. Swallowing dysfunction can be seen after injuring the pharynx and/or epiglottis which leads to the obstruction of esophagus. An algorithm was established taking into account the injury to the epiglottis and restoring gastrointestinal continuity with isolated or combine free and or supercharged jejunum flap, or supercharged colon transposition flap. METHODS: Seventeen patients between the ages of 3 and 16 (mean age: 7.7) were treated between 1985 and 2019. Three different procedures were perfor

High-speed treadmill videoendoscopic examination of the upper respiratory tract in the horse: the results of 291 clinical cases.

The purpose of the study was to describe the prevalence of upper airway abnormalities and establish if any significant associations existed between study variables and the two most frequently identified disorders; axial deviation of the aryepiglottic folds and dorsal displacement of the soft palate. The clinical records and video-recordings of all horses referred for upper respiratory tract evaluation during high-speed treadmill videoendoscopy between November 1997 and September 2003 were reviewed. Of 291 horses included in the study, 265 underwent resting endoscopy and 42% (112/265) had a rec

[Primary HIV infection].

Up to 70% of individuals with primary HIV infection will develop symptoms of an acute illness. The most common symptoms reported are fever, generalized lymphadenopathy, arthralgia and myalgia, headache, pharyngitis, enanthema, skin rash, diarrhoea, and mucocutaneous ulcerations. More rarely, oesophageal candidiasis, meningoencephalitis, rhabdomyolysis and epiglottitis have been reported. The diagnosis of the acute HIV infection syndrome can be established by demonstrating antibodies to HIV or by demonstration of HIV antigen positivity. Detection of virus through culture or PCR may prove to be …

Antibody response to capsular polysaccharides of groups A and C neisseria meningitidis and Haemophilus influenzae type b during bacteremic disease.

Levels of antibodies to the capsular polysaccharides of groups A and C Neisseria meningitidis and Haemophilus influenzae type b were determined by radioimmunoassay in acute- and convalescent-phase sera of 377 patients with meningitis, septicemia, or epiglottitis. Diagnostic criteria, based on a fourfold or greater rise in antibody level and/or a high specific antibody level, correctly identified 85% of group A meningococcal diseases, 90% of group C meningococcal diseases, and 78% of H. influenzae type b diseases in children older than but only 15% in infants younger than 1.5 years of age. [A case of a patient with Forestier's disease--intubation by Airway Scope and bronchofiberscope].

Forestier's disease accompanies difficult airway management with the ossification of the anterior longitudinal ligament of the spine. The abnormalities include limited cervical mobility, constrained epiglottic elevation, exclusion of the part of aryepiglottic, recurrent nerve paralysis and thinning of respiratory tract mucosa. We planned endotracheal intubation using both AWS and bronchofiberscope in a patient with Forestier's disease. We could easily confirm his glottis by using AWS and evaluate his respiratory tract below the glottis by bronchofiberscope. We could intubate an enforced tube (

Obstructing mass lesion of epiglottis: it can be tubercular.

We report a case of 60-year old male who had difficulty in breathing as well as in swallowing. On examination, he was found to be having proliferative growth of epiglottis and right aryepiglottic fold mimicking neoplasm. So emergency tracheostomy was performed and biopsy taken. He was found to be having asymptomatic miliary mottling on routine x-ray chest PA view. Further on HRCT, it turned out to be lesion suggesting tubercular etiology. Histopathology (epiglottic biopsy) report confirmed the whole process as tubercular. The patient recovered promptly in due course with anti-tubercular treatm

The Great Leap Forward: the anatomic basis for the acquisition of speech and obstructive sleep apnea.

Obstructive sleep apnea is an anatomic illness caused by evolutionary changes in the human upper respiratory tract. These changes include shortening of the maxillary, ethmoid, palatal and mandibular bones, acute oral cavity-skull base angulation, pharyngeal collapse with anterior migration of the foramen magnum, posterior migration of the tongue into the pharynx, descent of the larynx and shortening of the soft palate with loss of the epiglottic-soft palate lock-up. While it is commonly believed that some of these changes had positive selection pressures for bipedalism, binocular vision and lo

Laryngeal mask insertion with a laryngoscope in paediatric patients.

PURPOSE: To assess epiglottic position after laryngeal mask airway (LMA) insertion with or without the use of a laryngoscope. METHODS: A double-blind randomized study. In 48 children an LMA (#2 for 6-20 kg, #2.5 for 20-30 kg) was inserted either blindly or with the help of a laryngoscope and its position assessed using fibreoptic endoscopy. RESULTS: An unobstructed view of the glottis, as assessed by fibrescope, was observed in 10 of 25 patients in the laryngoscope group, but only in 1 of 22 patients in the blind insertion group (P = 0.005). CONCLUSION: This technique offers an alternative

Cervical osteophyte induced dysphagia.

Although cervical spondylosis is a common disorder, dysphagia induced by osteophyte formation is uncommon. Fewer than one hundred cases of cervical osteophyte induced dysphagia have been reported, with little attention to the diagnosis by barium swallow. The radiological features of two cases treated surgically with good results are described. Both cases complained of dysphagia while one had associated respiratory obstruction on forward flexion of his neck. The features on barium study of cervical osteophytes causing dysphagia include deformity at the level of osteophyte formation, in both AP

Laryngospasm from the anesthesiologist's viewpoint.

Laryngeal spasm is a problem constantly confronting the anesthetist. It can be serious and may produce fatal cerebral or cardiac complications. Etiologic agents include primary vagal hypertonicity, anoxemia, and painful stimulation of whatever source. Laryngeal spasm must be differentiated from simple obstruction by the tongue or foreign bodies, epiglottic impaction, laryngeal edema, tracheal spasm and collapse, and bronchial spasm. Proper checking of the patient before anesthesia and adequate premedication with atropine or scopolamine are preventive measures of great value. Once spasm has dev
