OBJECTIVES: Beyond non-biomechanical factors, the trabecular architecture of long bone epiphyses variably underlies functional adaptations to locomotor behavior. We report a male neonate with craniofacial dysmorphic features, multiple congenital anomalies and an unusual form of chondrodysplasia punctata. Radiographic examination revealed punctate epiphyses and coronal clefting of the thoracic spine. The hand radiographs showed some similarities to the brachytelephalangic type of chondrodysplasia punctata. However, the disorder did not fit well with any known entity of chondrodysplasia punctata or other condition characterized by punctate epiphyses.

Dysplasia epiphysealis hemimelica (DEH) is a rare developmental disorder characterized by osteocartilaginous overgrowth in one or more epiphyses. Telomeres are important in chromosome structure and function, protecting against their degradation. However, few studies have examined telomeres in growth plates within articular cartilage during normal development. Slipped proximal humeral epiphyses as a sequela of radiotherapy were identified in two children treated for Ewing's sarcoma in whom the shoulder was included in the field of treatment. The radiation doses were slightly greater than those in children developing slipped capital femoral epiphyses following radiotherapy. The lesser stress about the shoulder compared to the hip is believed to be the reason this is an infrequent complication of radiotherapy.

Unstable slipped capital femoral epiphysis can have disastrous complications including osteonecrosis and chondrolysis. It has been shown that 20% to 80% of patients may develop a contralateral slip ≤18 months after diagnosis. This is a case report of a 35-year-old man with multiple open epiphyses and a slipping of the right capital femoral epiphysis. An endocrine evaluation revealed primary hypothyroidism. The patient was placed on hormonal therapy and an in situ hip pinning was performed to prevent further slip. The operation was successful. The symptoms disappeared and all of the epiphyses were closed within one year. In an adult patient presenting with slipped capital femoral epiphysis, endocrine dysfunction such as hypothyroidism should be suspected.

Slipped capital femoral epiphyses (SCFE) were created in 10 pair of bovine femurs. Each pair was pinned with a single screw on one side and two screws contralaterally. The specimens were reloaded to failure. Double pin fixation yielded only a 33% increase in stiffness as compared with single pin fixation.
