The role of the nurse in meeting the educational needs for self-care in cachectic cancer patients and their family caregivers: A scoping review.

OBJECTIVE: To give an overview of what is known about the nurse contribution to education in self-care by people with cancer cachexia and their family caregivers. Nurse-led patient education can help patients and their family caregivers to manage cancer symptoms, cancer treatments, and treatment side effects. METHODS: This scoping review explored the extent to which nurse-led education has become part of the multimodal management of cancer cachexia. It is based on a systematic search of Medline, Embase, CINAHL, APA PsycINFO, and the Cochrane Library. Search limits were English language, date

Nurse Practitioner Roles and Contributions in a Mobile Health Clinic for People Experiencing Homelessness.

AIM: To present a descriptive, practice-based account of a nurse practitioner-led mobile health clinic serving individuals experiencing homelessness, guided by the PRIME-NP framework. BACKGROUND: Homelessness is associated with health inequities and limited access to care. Individuals face barriers including transportation, stigma, and displacement. Although mobile outreach may help address these barriers, little work has examined competencies for nurse practitioners in unstable, resource-limited environments. In this clinic, nurse practitioners provide field-based care, including wound manag

Nursing Care for the Initial Resuscitation of Severe Sepsis Patients.

Sepsis is recognized as a major health care problem worldwide. In the United States, mortality from severe sepsis and septic shock remains a serious health problem; yet, the early recognition of sepsis by nurses reduces can reduce mortality, morbidity, and long-term consequences of sepsis for patients. Improving the knowledge of nurses to first recognize the early signs of sepsis and then how to apply the most up-to-date evidence-based treatments can improve outcomes. Enhanced monitoring includes the use of computerized early warning systems to alert nurses of worrisome clinical patterns and p

Highlights From the U.S. Selected Practice Recommendations for Contraceptive Use.

The U.S. Centers for Disease Control and Prevention recently updated the U.S. Selected Practice Recommendations for Contraceptive Use to foster a reduction in unplanned pregnancy rates and to provide clinicians an evidence-based guide for contraception management. Nurses play an important role in helping women and families with reproductive life planning. By bridging knowledge gaps and removing access barriers with regard to contraception, nurses can contribute to reducing rates of unintended pregnancy. Nurses and other clinicians are encouraged to use the U.S. Selected Practice Recommendation

Advance-practice nursing in heart-failure management: an integrative review.

The number of patients with heart failure (HF) is predicted to escalate into the next decade, whereas the number of cardiac specialists who are skilled in evidence-based recommendations in HF practice will struggle to provide available, quality care. The advance-practice nurse, whose focus is HF management, may be an important key to improving access to this growing aggregate. This integrative review indicates the positive cost-to-benefit ratio for the advance-practice nurse's collaboration in HF management. Other measurable nursing outcomes have yet to scratch the surface, thereby inviting st

The nursing contribution to nutritional care in cancer cachexia.

Cancer cachexia is a complex syndrome. Its defining feature is involuntary weight loss, which arises, in part, because of muscle atrophy and is accompanied by functional decline. International expert consensus recommends that nutritional support and counselling is a component of multimodal therapy for cancer cachexia, as poor nutritional intake can contribute to progression of the syndrome. The present paper focuses on what is presently known about the nursing contribution to nutritional care in cancer cachexia. There is potential for nurses to play an important role. However, obstacles to thi

Why board of nursing disciplinary actions do not always yield the expected results.

One of the ways boards of nursing serve to protect the public health, safety, and welfare is by removing from practice those nurses who fail to meet recognized standards of care or otherwise pose a public threat. Self-reporting and discovery through criminal background checks and reports from the court system or other regulatory bodies represent only small numbers of those disciplined. Most complaints investigated by a board are reported by nursing administrators, either chief nursing officers or nurse managers. Frequently, the reporting nurses provide the board with information from their own

Dependency in older people recently admitted to care homes.

OBJECTIVE: to investigate dependency and general health status of a cohort of older people admitted to residential or nursing homes for long-term care. METHOD: we assessed 308 people aged over 65 years within 2 weeks of admission for long-term care to one of 30 nursing or residential homes in north-west England. Dependency was assessed using the Barthel activities of daily living index and the Crichton Royal Behaviour Rating Scale. We collected information from the homes' records on diagnosed conditions and current medication. RESULTS: 50% of the cohort were in a 'low dependency' band (Barth

Evaluation of a Patient-Centered Fall-Prevention Tool Kit to Reduce Falls and Injuries: A Nonrandomized Controlled Trial.

IMPORTANCE: Falls represent a leading cause of preventable injury in hospitals and a frequently reported serious adverse event. Hospitalization is associated with an increased risk for falls and serious injuries including hip fractures, subdural hematomas, or even death. Multifactorial strategies have been shown to reduce falls in acute care hospitals, but evidence for fall-related injury prevention in hospitals is lacking. OBJECTIVE: To assess whether a fall-prevention tool kit that engages patients and families in the fall-prevention process throughout hospitalization is associated with red

The indwelling urinary catheter: principles for best practice.

Each year, millions of Americans diagnosed with bladder dysfunction, primarily older adults in acute-care hospitals, long-term care settings, or their homes, use a urinary drainage system (UDS) to drain and collect urine. A UDS can be an integral part of managing urinary retention and urinary incontinence in certain patients, but the actual number of patients who use a catheter indefinitely to manage urinary incontinence or because of chronic urinary retention has not been well documented in the medical or nursing research. The care of UDS has traditionally been the domain of nursing staff, wh

Development of a psychometrically valid training needs analysis instrument for use with primary health care teams.

The growing demand for professional updating and training within the health service has created a proliferation of post-registration courses, many of which fail to reach the appropriate personnel or the real training objectives of the participants and their managers. One reason behind this problem relates to the fact that many such courses are constructed and delivered in a haphazard way, without systematic reference to the direct and indirect consumers of the educational programmes. A more rational approach to post-experience provision of this sort might involve the methodical collection of i

Improving quality of life in venous leg ulceration: a case study.

It is not uncommon to find that patients with venous leg ulcers have a reduced quality of life, but to prevent inappropriate management and thus contribute to the chronicity of these wounds, clinical practice must be based on sound evidence and application of research. The choice of compression options is fairly wide, but occasionally problems are encountered with tailoring a particular product to a patient. In this case study, a number of challenges were presented to the nursing team in implementing evidence-based practice and accommodating patient choice, but these were overcome and the pati

Unpacking the Role of Financial Incentives in the Motivation and Individual Performance of Nurses in LMICs: A Scoping Review Protocol.

BACKGROUND: Health systems in low- and middle-income countries (LMICs) continue to face persistent challenges related to health worker performance and quality of care, exacerbated by a global shortage of nurses. Financial incentive strategies (FIS), which include performance-based financing, bonuses and allowances, are extensively implemented as mechanisms to enhance the motivation and individual performance of health workers, including nurses. Nevertheless, the effects of these strategies exhibit considerable variability and are highly contingent upon specific contextual factors. OBJECTIVE:
