This paper describes the clinical management of patients with malignant cells in their sputum and a normal chest roentgenogram and those with asymptomatic peripheral pulmonary masses. The source of malignant cells in the sputum of patients with no roentgenographic abnormalities can be localized by tantalum bronchography and fiberoptic bronchoscopy. Peripheral pulmonary masses can be diagnosed preoperatively by needle biopsy or transbronchial fiberoptic bronchoscopy with little morbidity and no mortality. These procedures are not necessary, however, if there is firm clinical and roentgenographi

After a short review of the development of endoscopic investigations, the technique of gastroscopy is described and the indications for this procedure are pointed out. This procedure is of equal importance as radio-diagnostic measures in the routine investigation of stomach diseases and is of particular value when used as an emergency procedure and in the follow-up of patients after gastric operations. It also offers great advantages in the diagnosis of stomach tumours since biopsy material can be obtained from the lesion. Stomach haemorrhage due to gastric erosion, oesophagitis and the Mallor

Fifty consecutive patients presenting in a general practice with dyspepsia were studied. Each patient was referred for a combined barium-meal examination and cholecystogram, followed by fibreoptic endoscopy of the oesophagus, stomach, and duodenum. Thirty patients had a specific lesion of the upper digestive tract and a further four had gall-bladder disease; of these, 16 required surgical treatment. Endoscopy in the remaining 16 patients showed nine with mucosal abnormalities, leaving only seven patients to be classified as normal. The advantage of having an endoscopy service to supplement radi

PURPOSE: To use novel confocal scanning ophthalmoscopy technology to test hypothesis that HIV-seropositive patients without history of retinitis with a history of a low CD4 count are more likely to have damage to their retinal nerve fiber layer (RNFL) when compared to patients with high CD4 count. In addition, we compared optic disc morphologic changes with glaucoma.
DESIGN: Cross-sectional study.
PARTICIPANTS AND CONTROLS: 171 patients were divided into four groups. The control group consisted of 40 eyes of 20 HIV-seronegative patients. The second group consisted of 80 eyes of 41 HIV-positi

Glaucoma is an optic neuropathy and the leading cause of irreversible blindness worldwide. Imaging of the ganglion cell complex and retinal nerve fiber layer with optical coherence tomography (OCT) is a noninvasive, high-resolution means of diagnosing and quantitatively monitoring glaucoma. In the anterior segment, OCT can also be used to assess the anterior chamber angle and identify angle closure, a risk factor for glaucoma. The interpretation of OCT images for accurate diagnosis requires expert-level knowledge of both the technology and glaucoma. Deep learning (DL) is a subfield of artifici

Cervical cancer arises from uncontrolled proliferation of epithelial cells in the lower uterine cervix, primarily triggered by persistent infection with high-risk human papillomavirus strains. Timely and reliable detection of malignant cellular transformations is therefore essential for effective therapeutic intervention. Here, we report a highly sensitive plasmonic U-bent fibre-optic biosensor (FOB) integrated with Au and Ag nanocoatings for rapid identification of cervical cancer cells. This work is innovative in combining a U-bent fibre geometry, which enables robust waveguide propagation a

We present a case of an infant patient with Robin sequence (Pierre Robin sequence; PRS) who underwent general anesthesia for a glossopexy procedure. Pediatric patients with PRS are prone to upper airway obstruction during general anesthesia induction and intubation difficulties due to micrognathia and glossoptosis. In this case, we facilitated mask ventilation by inserting a nasopharyngeal airway before induction and successfully intubated the patient using a 2-person technique that combined the use of a video laryngoscope and a flexible fiber-optic scope. This experience suggests that the use

BACKGROUND The flexibility of the long flexometallic tube makes insertion of the flexible reinforced laryngeal mask airway (f-LMA) difficult. We compared the usefulness of rigid lightwand-guided f-LMA insertion with standard digital manipulation. MATERIAL AND METHODS Fifty-four patients (aged 19-70 years) were randomly divided into a control group (digital manipulation technique) or the lightwand group (lightwand-guided insertion technique). The insertion profiles, oropharyngeal leak pressure (OLP), peak inspiratory pressure (PIP), expiratory tidal volume, and ventilatory score were measured i

BACKGROUND: Sedation during elective fiberoptic intubation for difficult airway can cause respiratory depression, apnea and periods of desaturation. During apneic episodes, hypoxemia can be prevented by insufflation of oxygen in the deep laryngeal space. The aim of this study was to evaluate an oropharyngeal oxygenation device (OOD) designed for deep laryngeal insufflation during fiberoptic intubation.
METHODS: The OOD is split in the front to form a path for the bronchoscope. An external lumen delivers oxygen in the deep laryngeal space. In this experimental study, air application (as contro

Bacteria in the genus Brucella are the cause of brucellosis in humans and many domestic and wild animals. A rapid and culture-free detection assay to detect Brucella in clinical samples would be highly valuable. Nanomaterial optical fiber biosensors (NOFS) are capable of recognizing DNA hybridization events or other analyte interactions with high specificity and sensitivity. Therefore, a NOFS assay was developed to detect Brucella DNA from cultures and in tissue samples from infected mice. An ionic self-assembled multilayer (ISAM) film was coupled to a long-period grating optical fiber, and a

We report a child with vocal cord adhesion encountered during induction of anesthesia. A 4-month-old girl was scheduled for bilateral lip plasty. She was intubated for one week due to pneumonia at the age of 3 days. Hoarseness and stridor appeared just after extubation. Although laryngo-fiberoptic examination had been tried several times, otorhinologists could not find any abnormality. We once decided to postpone the operation because of severe stridor. However, laryngofiberoptic examination could not reveal any abnormality, and we rescheduled the operation. Tracheal intubation using laryngosc

Supraglottic airway devices (SADs) are used to keep the upper airway open to provide unobstructed ventilation. Early (first-generation) SADs rapidly replaced endotracheal intubation and face masks in > 40% of general anesthesia cases due to their versatility and ease of use. Second-generation devices have further improved efficacy and utility by incorporating design changes. Individual second-generation SADs have allowed more dependable positive-pressure ventilation, are made of disposable materials, have integrated bite blocks, are better able to act as conduits for tracheal tube placement, a

Fiberoptic intubation (FOI) is an effective technique for establishing airway access in patients with both anticipated and unanticipated difficult airways. First described in the late 1960s, this approach can facilitate airway management in a variety of clinical scenarios given proper patient preparation and technique. This paper seeks to review the pertinent technology, clinical techniques, and indications for and complications of its use. The role of FOI in airway management algorithms is discussed. Evidence is presented comparing FOI to other techniques with regard to difficult airway manag
