Facility-Based Assessment of Emergency Obstetric and Neonatal Care in Vanga Health Zone, Kwilu Province, Democratic Republic of Congo.

BACKGROUND: Maternal and neonatal morbidity and mortality rates in the Democratic Republic of Congo (DRC) remain unacceptably high. The lack of empirical evidence on the capacity and performance of health facilities offering emergency obstetric and neonatal care (EmONC) is a central problem. METHODS: This study assessed the quality of EmONC provision across 63 healthcare facilities in Vanga Health Zone, Kwilu province, DRC. RESULTS: We identified widespread infrastructural deficiencies, including no water sources in 61/63 facilities, no emergency transfer capability, and critically low bed c

Multidisciplinary research priorities for artificial intelligence in mental health: a call to action.

The use of artificial intelligence (AI) is anticipated to transform mental health care. However, the rapid research growth in this field has outpaced coordinated frameworks, leaving research efforts fragmented, standards inconsistent, and safeguards for safety and ethics largely absent. This Position Paper outlines a coordinated roadmap to guide the responsible evaluation and implementation of AI in mental health, structured across four overarching priority domains that define near-term actions and longer-term strategic goals. Domain 1 (Strengthen safety and evidence standards) addresses defic

Confined to a lunatic asylum for eleven years: the case of Haaken Bergh.

BACKGROUND: Haaken Bergh spent eleven years as a patient in psychiatric institutions in Norway and Sweden in the early twentieth century. Following his discharge from Dikemark Asylum in 1921, he devoted the rest of his life to seeking redress and campaigning for reform of the mental health service. Bergh published two books, brought legal proceedings against Oslo local authority, petitioned the Norwegian parliament (the Storting) for ex-gratia compensation and held lectures throughout Norway. He attracted considerable public attention during his lifetime but was largely forgotten after his dea

The impact of U.S. foreign aid reduction on global health.

A reduction in U.S. foreign aid under the "America First" policy of President Donald Trump, who took office in 2025, has significantly impacted global health. As the world's largest provider of foreign aid, the U.S. has frozen development aid to evaluate its alignment with national interests. This has led to the termination of numerous international health programs, including those addressing malaria, HIV, tuberculosis, and polio, and has caused funding shortages for non-profit and international organizations like GAVI and the World Bank. Projections indicate dire consequences. According to US

Surveillance without silos: a One Health data imperative for avian influenza in West Africa.

Highly pathogenic avian influenza (HPAI) has circulated in West Africa since Nigeria's first human case in 2006, making the region a key entry point and dispersal hub for multiple virus lineages in sub-Saharan Africa. The 2.3.4.4b virus clade driving the global spread of avian influenza is now infecting mammals more often, a troubling sign that raises the risk of a future pandemic. At the same time, West Africa's ability to control HPAI is held back by fragmented surveillance systems. Animal, human and environmental health services operate in isolation, rely on incompatible data formats and ar

The healthcare system in Sweden.

The Swedish population is characterized by high life expectancy and low avoidable mortality rates. This review outlines the Swedish healthcare system, which offers universal access to all residents and has a long tradition of reforms for social equity. Responsibility for healthcare is shared between the state, the regions, and the municipalities. The Ministry of Health and Social Affairs provides the overall healthcare framework; additionally, several governmental agencies are directly involved in healthcare and public health initiatives. The 21 regions organize, finance, and provide most prim

[Structural resilience and robustness of German hospitals: results form a nationwide survey of emergency departments].

BACKGROUND: Hospital disaster response planning (KAEP) is a core component of health security and civil protection. In the context of escalating security threats and climate-related extreme events, its practical implementation is gaining importance. Adequate medical care requires structurally and organizationally resilient healthcare facilities; however, the structural and infrastructural prerequisites in Germany have scarcely been examined systematically. OBJECTIVE: To analyze the current status of structural and infrastructural conditions that affect the effectiveness and feasibility of KAE

Strategies and challenges for maintaining the continuity of essential health services during a pandemic: a scoping review.

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic has had a significant impact on health systems worldwide, resulting in disruptions to essential health service delivery, such as routine immunizations, maternal and child health, and treatment for communicable and noncommunicable diseases. These services have been disrupted due to the diversion of resources towards the COVID-19 response. Therefore, the objective of this scoping review was to identify the strategies and challenges associated with maintaining the continuity of essential health services during a pandemic. METHODS: This

Food Loss and Waste Reduction in Specific Fruit and Vegetable Value Chains in Eastern Africa.

The United Nations Sustainable Development Goal (SDG) 12.3 and the Malabo Declaration both address the critical issue of food loss and waste (FLW), but they differ in scope, timelines and regional focus. While SDG 12.3 provides a global framework and target of 2030, the Malabo Declaration reflects Africa's pressing need to reduce FLW by 2025. Despite these targets and focus on FLW reduction by the global community, high FLW of fruits and vegetables continues to persist in many parts of Africa due to systemic constraints related to limitations in governance, financing and knowledge. It is estim

Closing the gap between implementation science and policy in Nigeria: lessons from the Nigeria implementation science alliance using a nominal group technique.

INTRODUCTION: Knowledge translation in healthcare has been of keen interest to researchers, practitioners, policymakers and administrators as it seeks to confront complex health issues within communities by closing the gap between knowledge generation through research and knowledge application. A paucity of information exists regarding nature of the relationship between Nigerian implementation science researchers and policymakers in the sphere of knowledge translation. This study aimed to identify and discuss barriers to successful engagement between implementation researchers and policymakers

Remote Didactic Education Effectively Increases Cardiac Critical Care Nursing Staff Knowledge in Low-Resource Settings.

Background: While progress has been made to decrease mortality in children under age five, there continues to be a need for improvement in the treatment of children with congenital heart disease. Many of these patients require surgical correction and live in areas without the expertise of surgical teams. Research has shown that appropriate training is critical to ensure the best clinical outcomes. The Ethiopian government has identified the need for increased training of health care professionals as a method to improve hospital outcomes. Methods: Twenty-five cardiac critical nurses participate

Leading improvement at scale in healthcare: Sir John Oldham.

BACKGROUND: When I became senior partner at my general practice I undertook a part time MBA at Manchester Business School in the late 90s to learn the things medical school hadn't taught me; organizational design and behaviour, finance, strategic management and human resources (as it was then called). My dissertation was on quality methods in other industries that could be applied in healthcare. JOURNEY: We tried out total quality management methods in my practice and began to demonstrably improve care, services and the use of resources. I titled this in presentations in the early noughties t

A global investment framework for the elimination of hepatitis B.

BACKGROUND & AIMS: More than 292 million people are living with hepatitis B worldwide and are at risk of death from cirrhosis and liver cancer. The World Health Organization (WHO) has set global targets for the elimination of viral hepatitis as a public health threat by 2030. However, current levels of global investment in viral hepatitis elimination programmes are insufficient to achieve these goals. METHODS: To catalyse political commitment and to encourage domestic and international financing, we used published modelling data and key stakeholder interviews to develop an investment framewor

Assessment of the performance of community-directed treatment with ivermectin strategy for the control and elimination of onchocerciasis in Edo State, Nigeria.

BACKGROUND: Community-directed treatment with ivermectin (CDTI) was developed in the mid 1990's as a solution for the control and elimination of onchocerciasis. It requires that ivermectin be administered continuously over a period of at least 14 years with community involvement before elimination can be achieved. OBJECTIVES: The objective of this study is to assess the performance of CDTI strategy for control and elimination of onchocerciasis in endemic Local Government areas of Edo State. METHODS: A descriptive evaluation in a cross-sectional, descriptive study design was conducted among 7

The trajectory of methadone maintenance treatment in Nepal.

There are about 28,500 people who inject drugs (PWID) in Nepal and HIV prevalence among this group is high. Nepal introduced harm reduction services for PWID much earlier than other countries in South Asia. Methadone maintenance treatment (MMT) was first introduced in Nepal in 1994. This initial small scale MMT programme was closed in 2002 but reopened in 2007 as an emergency HIV prevention response. It has since been scaled up to include three MMT clinics and continuation of MMT is supported by the Ministry of Home Affairs (MOHA; the nodal ministry for drug supply reduction activities) and ha

Depression in Mongolian women over the first 2 months after childbirth: prevalence and risk factors.

BACKGROUND: Social, political and economic changes in Mongolia have followed post-Soviet style government policies and contributed to both increased liberalisation and reduced security in employment and family finances. This is the first study to attempt to assess the prevalence of depression in a population of Mongolian women in the post-partum period and assess risk factors, including financial position, associated with the condition. METHODS: A total of 1044 women who had delivered healthy babies in Ulaanbaatar between October and December 2002 were screened for depression using the WHO Se

Community-directed treatment with ivermectin in two Nigerian communities: an analysis of first year start-up processes, costs and consequences.

OBJECTIVES: To determine the start-up processes, costs and consequences of community-directed treatment with ivermectin (CDTI) in two onchocerciasis endemic rural towns of Southeast Nigeria; namely Achi and Nike. The other objectives were to discover the community-financing mechanisms, local ivermectin distribution strategies and communities' organisational capacity to handle the programme. METHODS: Structured questionnaires, informal interviews, observations, discussions with community members at general village assemblies and community outreach lectures were used at different stages of the

Safe Surgery Starts With Safe Anesthesia: Positioning Pediatric Anesthesia as a Core Health-System Function Within Global Children's Surgical Systems.

BACKGROUND: Children's surgery is increasingly recognized as a global health priority, yet pediatric anesthesia remains comparatively under-recognized despite being essential for safe surgical and perioperative care. THE PROBLEM: Children require specialized expertise, age-appropriate equipment, reliable monitoring, oxygen, medicines, recovery, and critical care. In many low- and middle-income countries, deficiencies across these domains limit access and contribute to preventable perioperative morbidity and mortality. THE CENTRAL CLAIM: We posit that children's surgical systems cannot be str

Birth rates and migration: an AI and One Health approach within the Italian National Health System.

This study examines the future stability of Italy's health and social systems in the context of demographic decline. The Italian healthcare system, grounded in a universalistic and public model, founded on the principle of equity, is undergoing a phase of instability. The COVID-19 pandemic represented a critical rupture, exposing systemic weaknesses, territorial disparities, infrastructural inadequacies, the rigidity of financing mechanisms and the lack of reform regarding citizenship rights for foreign residents. An inductive, interpretive approach was adopted, starting with data analysis fro

Preparing Physicians for Modern Health Systems: Embedding Leadership and Hospital Administration Training in Graduate Medical Education With Lessons From Psychiatry.

Physicians are entering practice in healthcare systems shaped by consolidation, corporatization, administrative complexity, and rising burnout. These pressures are especially relevant in psychiatry and child and adolescent psychiatry, where clinicians must navigate fragmented systems, multidisciplinary teams, reimbursement constraints, and organizational decisions that directly affect patient care. However, most graduate medical education programs provide limited formal training in leadership, hospital administration, finance, governance, or systems-level decision-making. This editorial argues
