Focal Staphylococcus Aureus Septic Arthritis Elicits Age and TLR2-Dependent Periarticular Bone Loss.

INTRODUCTION: Septic arthritis, primarily caused by Staphylococcus aureus (S. aureus), is a severe joint infection that leads to joint and bone damage. S. aureus lipoproteins (LPPs) bind to Toll-like Receptor 2 (TLR2), inducing arthritis and localized bone loss. Aging affects TLR2 immune response to pathogens. While intra-articular injections of S. aureus LPPs induces local bone resorption in mice, the influence of aging and TLR2 expression on bone mineral density (BMD) after S. aureus bacteremia remains unclear. METHODS: We analyzed distal femoral BMD in young and old TLR2 knock-out and wild

Management of a Severe Cervicofacial Odontogenic Infection.

INTRODUCTION: Odontogenic infections originate from a tooth or from its supporting structures, generally secondary to a pulp necrosis, periodontal disease, pericoronitis, apical lesions, or complications of dental procedures, which can be restricted to the alveolus or can reach the jaws and face through maxillofacial spaces. These are the most common conditions which affect the head and neck regions. AIM: The aim of this study is to report a severe case of a cervicofacial odontogenic infection while discussing the most relevant aspects. CASE REPORT: A 47-year-old female patient with a backgr

Aperture width of the osteomeatal complex as a predictor of successful treatment of odontogenic maxillary sinusitis.

Odontogenic maxillary sinusitis (OMS) is an inflammatory disease caused by the spread of dental inflammation into the sinus. The long-term administration of antibiotic medicine and/or treatment of the causative tooth are the usual initial treatments. These initial treatments are not always effective, and the reason is not well understood. The purpose of this study was to identify factors of significance that may contribute to the results of the initial treatment of OMS. Thirty-nine patients were studied, divided into two groups according to the results of initial treatment: effective or non-ef

Effectiveness of 1 alpha-hydroxyvitamin D3 in children with renal osteodystrophy associated with hemodialysis.

Two children who were receiving maintenance hemodialysis were treated with 1 to 4 microng/day of 1 alpha-hydroxyvitamin D3 for 500 to 700 days, respectively. Sequential parathyroid hormone levels and radiologic evaluations showed considerable improvement in one patient. The second patient initially responded with healing of the bone disease. Subsequent deterioration may be related either to medical noncomplicance and/or interference by diphenylhydantoin in the subsequent hepatic 25-hydroxylation of 1 alpha-OH-D3.

An Unusual case of neurocysticercosis leading to blindness in a young Indian male.

Neurocysticercosis is an endemic disease in India. It is caused by infection from the larval form of Taenia solium and humans serve as the intermediate host. Its diverse manifestations include seizures, headaches, focal neurological signs. Obstructive hydrocephalus, with its potential for death is the most disastrous complication. We hereby report a case of 24 year old boy presenting with blindness without hydrocephalus.

Replacement lipomatosis: CT and MRI findings of a rare renal mass.

Replacement lipomatosis of the kidney (RLK) is an advanced form of renal sinus lipomatosis, in which infection, renal calculi and long-standing hydronephrosis are accompanied by renal parenchymal atrophy. The kidneys are usually poor or non-functioning. We present CT and MRI findings of an unusual focal RLK of a 52-year-old male, who was examined with the suspicion of renal malignancy.

Progressive multifocal leukoencephalopathy occurring with the acquired immune deficiency syndrome.

A 33-year-old homosexual man with symptoms and signs of a focal brain process was subsequently found to have an acquired immune deficiency syndrome (AIDS) with biopsy-proven progressive multifocal leukoencephalopathy. This report reemphasizes the association of progressive multifocal leukoencephalopathy with AIDS and probably is best viewed as another example of an opportunistic CNS infection complicating deficient cell-mediated immunity.

Identification of Rickettsia rickettsii in a guinea pig model by immunofluorescent and electron microscopic techniques.

Moribund guinea pigs infected with Richettsia rickettsii were examined by necropsy, histology, immunofluorescence, electron microscopy, and serology. Untreated animals died at 9 and 10 days after inoculation. Animals given saline subcutaneously survived from 1 to 4 days longer. Prolonged survival was accompanied by more severe lesions: scrotal necrosis; infarction of ears; and swollen, hemorrhagic footpads, epididymis, and cremaster muscle. Histopathologic examination demonstrated that acute, necrotizing vasculitis, perivascular hemorrhage, and focal necrosis were more extensive. Direct immuno

Effectiveness of an Integrated One Health Intervention on Schistosoma japonicum Infection in Wild Rodents - Anhui Province, China, 2022-2024.

WHAT IS ALREADY KNOWN ABOUT THIS TOPIC?: Schistosomiasis represents a natural focal disease in which wild rodents function as critical reservoir hosts for Schistosoma japonicum transmission within specific endemic regions of China. WHAT IS ADDED BY THIS REPORT?: Implementation of a comprehensive 2-year One Health intervention package demonstrated a significant reduction in S. japonicum infection rates among wild rodents, declining from 69.15% to 22.09%, with intervention villages showing an 88.46% decrease in infection odds compared to control villages. WHAT ARE THE IMPLICATIONS FOR PUBLIC H

Herpes zoster polyradiculopathy.

Herpes zoster infection, resulting from reactivation of the dormant varicella zoster virus in the dorsal root ganglia, usually causes a painful dermatomal vesicular rash. Rarely, associated peripheral motor weakness is present, the mechanism of which is unclear. Three patients are reported who had focal limb muscle weakness associated with zoster infection. Physical and occupational therapy played a key role in motor function recovery of the patients, yet emphasis on the rehabilitation of postherpetic motor weakness is lacking in the literature. Physiatrists evaluating patients with limb muscl

Utility of multimodal sampling and testing during advanced bronchoscopy for diagnosing atypical respiratory infections in a Coccidioides-endemic region.

BACKGROUND: The role of advanced diagnostic bronchoscopy (ADB) for assessing atypical respiratory infections is unclear. The purpose of this study was to ascertain: (I) the diagnostic utility of ADB-tissue sampling in patients with focal thoracic lesions due to atypical respiratory infections; (II) how multimodal bronchoscopic sampling and testing enhance diagnosis in a Coccidioides-endemic region. METHODS: A retrospective observational cohort study analyzing all ADBs performed over a 10-year period in patients with focal thoracic lesions diagnosed with a non-malignant disorder. Only cases wh

Severe West Nile virus disease in healthy adults.

The inpatient records of 44 case patients with West Nile virus infection hospitalized in 2002 were reviewed. Sixty-five percent of the case patients had encephalitis, and 35% had aseptic meningitis. There was no significant difference in the distribution of aseptic meningitis versus encephalitis among adults aged < or =50 years compared with adults aged > or =65 years. Focal weakness, likely due to anterior horn cell involvement, was present in 10 case patients (overall rate, 23%; rate among patients with encephalitis, 34%). Case patients with focal weakness who were aged < or =50 years had mo
