Low-voltage direct current as a fungicidal agent for treating onychomycosis.

Onychomycosis, most commonly caused by two species of dermatophyte fungi--Trichophyton rubrum and Trichophyton mentagrophytes--is primarily treated with regimens of topical and systemic antifungal medications. This study was undertaken to evaluate in vitro the efficacy of low-voltage direct current as an antifungal agent for treating onychomycosis. Agar plate cultures of T rubrum and T mentagrophytes were subjected to low-voltage direct current electrostimulation, and antifungal effects were observed as zones in the agar around the electrodes lacking fungal growth. Zones devoid of fungal growt

Eccrine syringofibroadenoma with diffuse plantar hyperkeratosis.

In 1999, Serum Institute of India indigenously developed an adsorbed human diploid cell rabies vaccine (Rabivax). During 2000-04, this new vaccine was subjected to a series of immunogenecity and safety studies. Initially, an experimental batch of Rabivax (adsorbed) was assessed on ten healthy adult volunteers and its response was comparable with that of Merieux inactivated rabies vaccine (MIRV, lyophilized) which was used as a control. Subsequently, Rabivax (adsorbed) was assessed on forty-five suspect rabid dog bite cases with MIRV as control. The vaccine was found to be equally safe and immu

Successful treatment of onychomycosis with ciclopirox nail lacquer: a case report.

Onychomycosis commonly causes patients to seek medical care. Its unpleasant cosmetic appearance and negative impact on a patient's quality of life often prompt these visits. Onychomycosis rarely resolves spontaneously and has a high rate of recurrence. Although a variety of treatment options are available, inconsistent results and poor patient compliance have often hindered their success. We report the case of a 75-year-old man with moderate-to-severe onychomycosis who was treated successfully with ciclopirox nail lacquer solution 8%, the first topical antifungal agent approved by the Food and

Eumycetoma due to Madurella mycetomatis acquired in Jamaica.

We report a case of eumycetoma due to Madurella mycetomatis affecting the left dorsal forefoot in a 35-year-old woman. She had spent her childhood in Jamaica, and had been resident in the U.K. for 20 years prior to her presentation. She gave a highly suggestive history for a mycetoma, having observed the intermittent discharge of black grains from the lesion. The diagnosis was confirmed by histological and mycological analysis of the grains, and a magnetic resonance imaging scan excluded osteomyelitis. She has responded very favourably to treatment with itraconazole. Mycetomas due to M. myceto

Tungiasis in a 3-year-old child.

Tungiasis is an infestation of the skin caused by the flea Tunga penetrans. It is not rare in western Europe because of tourism to tropical and subtropical countries. However, tungiasis has been very rarely reported in children. We describe a case of tungiasis in a 3-year-old child. The infestation was localized to the second toe of the left foot and was characterized by an atypical clinical presentation.

Tinea nigra: treatment with topical ketoconazole.

Tinea nigra is a relatively uncommon fungal infection presenting as a pigmented macule of the palms or soles. Since the lesion can easily mimic a melanocytic process, it is important to recognize the infection before recommending unnecessary surgical procedures. A case of tinea nigra that responded to treatment with topical ketoconazole is presented.

Two post-operative epidermoid cysts following realignment of the hallux nail.

We present a case in which two implantation epidermoid cysts appeared following surgical realignment of the hallux nail. Atypical features of this case were the very young age of the patient, the unusual direction of malalignment of the nail and the site and number of lesions.

Acro-angiodermatitis. A simulant of Kaposi's sarcoma.

Acro-angiodermatitis, usually related to venous insufficiency of the lower limbs, may simulate some of the features of Kaposi's sarcoma, both to the clinician and the dermatopathologist. With the increasing incidence of Kaposi's sarcoma related to the acquired immune deficiency syndrome (AIDS), there is a greater awareness of Kaposi's sarcoma than the benign simulant. Against this background, there is a danger that acro-angiodermatitis will be misdiagnosed as Kaposi's sarcoma.

[Mycetoma caused by Petriellidium boydii: treatment with ketoconazole].

A case of mycetoma of the lower leg (Madura foot) with bone involvement caused by Petriellidium boydii is presented. After an initially favourable therapeutic response to ketoconazole (2 X 200 mg/die) the process recurred despite proven sensitivity of the fungus in vitro. It could not be controlled with conservative treatment and ultimately necessitated amputation of the lower leg. A striking, hitherto unreported feature of mycetoma was the episodic occurrence of circulating immune complexes with febrile monoarthritis of the adjacent ankle.
