Combined Monteggia and Galeazzi fractures in a child's forearm. A case report.

We report the occurrence of a previously undescribed combined Monteggia and Galeazzi fracture pattern of the left forearm in an 8-year-old boy. Previous reports do not address the combined injury described in this article, which consists of a left radius shaft fracture with dislocation of the distal radioulnar joint, and a left ulna shaft fracture with anterior dislocation of the radiohumeral joint. An excellent result was obtained with closed reduction. Radiographic evaluation is crucial in these disorders for both diagnosis and for treatment decisions. A review of the literature is provided …

The radial forearm flap as a pedicled flap for resurfacing a scarred nose.

The radial forearm flap, applied as a free flap, has been shown to be very versatile, if covering a defect with a thin, flexible flap is of predominant importance. The advantage of this technique compensates for any disadvantages in the donor area. Under certain circumstances it is a better solution to design the radial forearm flap as a distant, vascular pedicled flap than to perform it as a free flap with microvascular anastomoses. In treatment of this patient we used a distally pedicled radial forearm flap to resurface a burned nose and to reconstruct the tip of the nose. In this case a sca…

The radial forearm free flap for head and neck reconstruction: a review.

BACKGROUND: The radial forearm free flap has become a workhorse flap in head and neck reconstruction. Its lack of bulk, ease of dissection, vascularity, and malleability are among its many advantages. PATIENTS AND METHODS: A review of 157 radial forearm free flaps performed in 155 patients between March 1988 and June 1994 at The University of Texas M.D. Anderson Cancer Center was undertaken to determine outcome. Patient age ranged from 22 to 80 years (mean 56). There were 79 men and 76 women. Follow-up ranged from 2 to 75 months. The most prevalent neoplasm was squamous cell carcinoma (n = 12…

[Pathomorphogenesis of radio-ulnar synostosis in fractures of forearm bone shafts].

Pathomorphogenesis of radioulnar synostosis which occurred in 4 out of 174 patients treated because of recent and inveterate fractures as well as pseudoarthroses within shafts of forearm bones has been presented. Fractures of both radius and ulna, located at the same level within proximal half, inveterate or neglected, after severe or multiple trauma are at the highest risk of developing radioulnar synostosis.

The interosseous membrane of the forearm: structure and its role in Galeazzi fractures.

A cadaveric study was performed to anatomically describe and mechanically document the interosseous membrane of the forearm using gross, histologic, scanning electron microscopic, and mechanical testing. The membrane was found to be a complex structure composed of nerves and vessels but mainly of collagen fibers that thicken to form bands coursing from radius to ulna. Strain-gauge studies demonstrated that the load transfer occurs from the radius to the ulna via the membrane and changes with supination or pronation. With specimens under a specific test load, sectioning of the membrane allowed …

Effects of kwashiorkor malnutrition on measured capillary filtration rate in forearm.

This study investigated the effects of kwashiorkor malnutrition on blood tissue fluid exchange by measuring the rate of capillary filtration (CFR) in response to a 60-mmHg increment in venous pressure in the forearms of 1- to 3-yr-old native African children within the Transkei Homeland. They were divided into the following subject groups: kwashiorkor patients (K); kwashiorkor patients who were at various stages of recovery (RK); marasmus patients (M); patients with edema of nonkwashiorkor origin (E); and control children (C). Measurements of CFR were significantly lower in the K subjects comp…

[Use of posterior interosseus artery flap in the repair of forearm and hand].

The posterior interosseous vessels give a significant contribution to the fascial plexus which supplies the skin of the forearm. This vascular arrangement forms the basis for an island fasciocutaneous flap which can be based either proximally or distally. Its principal advantage is that it is a thin flap with excellent circulation and that the donor area can be closed primarily. The design and usage of an island fasciocutaneous flap with possibility of expansion is also described.

[Ischemic contracture of the forearm and hand. Staging and indications for surgical treatment].

Results of long-term follow up of 66 patients with ischemic contracture of the forearm and hand, all surgically treated in the "Unfallkrankenhaus Hamburg)) between 1961 and 1982, are presented. Whereas ischemic contracture of the forearm flexors resulted mainly from fractures, ischemic contracture of the intrinsic muscles of the hand was most often seen after pressure injuries. All patients in this study presented to us with fully established ischemic contractures. Muscle and nerve damage was retrospectively evaluated according to operative notes, and the degree of damage could be classified i…

The predictive value of forearm bone mineral content measurements in men.

Six hundred and fifty-four men had their bone mineral content (BMC) measured with single photon absorptiometry during 1970-1976. All roentgenograms 1975-1985 (11 years) were registered and thus all falls requiring roentgen examinations. BMC was found to be of predictive value in ages below 70-80 with regard to future fragility fractures. Other characteristics were more falls, less grip strength, and weight. A history of previous vertebral fracture and cerebral disorder were also predictive. Men with only nonfragility fractures showed no differences compared to the nonfracture population.

[Stable flexible nailing of fractures of both bones of the forearm in children].

Flexible medullary nailing is a method of choice for fractures of the forearm in children. This technique is indicated when orthopaedic treatment has fallen, or for instable fracture. We reported forty four fractures of the forearm concerning children with an age ranged from 2.5 to 15 years. We obtained 38 good results (83.3%) and 6 complications (14.7%) which were related to growth disturbance, leading to limitation of pronation; in one case we obtained a rupture of extensor pollicis longus.

Effects of nicorandil on arterial and venous vessels of the forearm in systemic hypertension.

The effects of a single oral dose of 20 mg of nicorandil were evaluated in 12 untreated patients with mild to moderate essential hypertension. Serial measurements of arterial pressures were obtained by means of an automatic device (Dynamap) up to 120 minutes after drug administration. Forearm hemodynamics were determined with a pulsed Doppler velocimeter, or strain gauge mecanography and plethysmography enabling measurement of the diameter, velocity and flow of the brachial artery as well as the arterial pulse wave velocity and forearm venous tone. In addition, local vascular resistance, compl…

Uptake of beta-hydroxybutyrate, acetoacetate and glucose by the forearm of the newborn infant.

Forearm muscle metabolism was studied in twelve appropriate for gestational age premature infants suffering from respiratory distress. Arterial blood was obtained by puncture of the radial artery as clinically indicated for measuring oxygen tension, and venous blood samples were taken from the same arm's deep brachial vein. This arrangement allowed to study the arterial-deep venous differences of beta-hydroxybutyrate, acetoacetate and glucose in simultaneously taken blood samples. Net muscular uptakes of beta-hydroxybutyrate and glucose were observed, however, in four studies a virtual net pro…

Surgical significance of the motor fascicular group of the ulnar nerve in the forearm.

The topography of the fascicular group of the ulnar nerve at the wrist and forearm was studied focusing on the motor (muscle) fascicular group. In 109 of 111 specimens studied (98%), the motor fascicular group of the ulnar nerve is located at the ulnar dorsal or straight dorsal position at the wrist and the distal forearm. This motor fascicular group may be identified as a distinct entity up to 90 mm proximal from the level of the radial styloid. The relatively constant location of the motor fascicular group is significant since most lacerations of the major peripheral nerves of the upper extr…

Microvascular reconstruction for close-range gunshot injuries to the distal forearm.

Three patients with compound defects in the distal forearm resulting from close-range gunshot injuries were treated with a free microvascular composite groin flap. The composite flap, which consisted of groin skin, the iliac crest, and abdominal muscles, was used to cover the exposed nerves, tendons, and vessels in one stage. The iliac crest replaced the segmental radial and ulnar defects, and the attached muscles were placed deep to the tendons to provide a gliding surface and to prevent adhesions to the bone. "Sandwiching" the damaged nerves and tendons between highly vascular soft tissue ha…

[Ischemic necrosis of the deep extensors of the forearm: a rare compartment syndrome].

The ischemic compartment syndrome is thought to develop following an unphysiological pressure increase in one of the numerous muscular compartments of the extremities, confined by bone surface and strong fascia. Pressure-induced ischemia may cause partial or total necrosis of the vulnerable structures in the compartment. This ischemic compression syndrome was found in the deep extensor compartment of the distal forearm in a young female patient following osteotomy of the ulna. The syndrome at this rather unusual site was not diagnosed and treated in the acute phase. Resection of fibrotic, cont…
