Distal femoral fractures are increasingly observed in the elderly population, often presenting with osteoporotic bone characteristics. Surgical fixation remains the gold standard, with lateral locking plates being the most commonly used method. To prevent hardware failure and varus collapse in osteoporotic bone, various surgical techniques and augmentation strategies have been proposed. We describe a surgical technique for managing osteoporotic, comminuted distal femur fractures using a fibular strut allograft in combination with a lateral-only locking plate, along with outcomes in patients tr

Open forearm fractures involving both the radius and ulna are challenging orthopedic injuries, typically resulting from high-energy impacts. These injuries require complex surgical interventions, such as open reduction and internal fixation (ORIF) or intramedullary nailing, and often necessitate multiple approaches due to the proximity of neurovascular structures. Severe cases can lead to complications such as infection, nonunion, and functional impairment of the limb. Here, we present the case of a 42-year-old male who sustained multiple injuries in a motor vehicle accident, including an open

A critical analysis of the results reveals the following major factors in determining the outcome of an individual acetabular disruption: The damage to the acetabular articular surface, which is reflected by the pattern of fracture and its degree of comminution, impaction, and osteoporosis, and associated damage of the femoral head The adequacy of the reduction, which is reflected by intraoperative inspection and postoperative radiographs The associated complications of the fracture and treatment (e.g., avascular necrosis, sciatic palsy, and heterotopic ossification) We recommend principles of

The technique of intrafocal pinning was originally described by Kapandji. This technique, along with the addition of supplemental external or internal bridging fixation, can be performed quickly with minimal soft tissue damage. This technique is indicated for the treatment of unstable extra-articular or minimally displaced intra-articular fractures of the distal radius. Studies demonstrate that young patients with displaced extra-articular distal radius fractures and minimal comminution can be treated by percutaneous intrafocal pin fixation alone, whereas, patients over 55 years of age and you

Fractures of the tibial shaft from skiing accidents were compared with fractures of other etiology. Skiing fractures more often affected the lower third of the tibia and they seldom were open. Many were comminuted or transverse but displacement tended to be slight. The incidence of complications was low and the periods of hospitalization, of healing, and of inability to work were shorter than for the other fractures. The highly favorable prognosis was evident whatever the method used for treatment. Methods of treatment of tibial fractures should not be compared on the basis of experience with

STUDY DESIGN: Retrospective database analysis.
OBJECTIVE: To determine C2 fracture patterns associated with vertebral artery injury (VAI) as assessed by computed tomography angiography.
SUMMARY OF BACKGROUND DATA: The incidence of C2 fracture hospitalizations has increased significantly in the last decade. The vertebral arteries are susceptible to injury as each courses through the C2 transverse foramen. Early screening for VAI to institute antithrombotic treatment is critical to prevent ischemic neurological sequelae. Imaging-based fracture classification schemes to determine which patterns

Champy miniplates have been used in the treatment of craniofacial fractures and in osteotomies for correction of posttraumatic deformities, congenital craniofacial deformities, and secondary bony deformities due to pathologies such as hemangioma and neurofibroma. An additional use has been to stabilize free and vascularized bone grafts. The total number of cases reviewed were 50 (25 acute trauma, 4 vascularized mandible reconstruction, and 21 osteotomies of varied types). There were three infections, two in mandibular fractures that were comminuted and compound into the mouth and one in a comp

BACKGROUND: Elderly acetabular fractures are one of the more difficult types of fractures to resolve. For patients at this age, the more common type of fracture is comminuted. How to better fix this type of fracture has always been an issue of concern. This study was performed to observe the mechanical properties of different internal fixation methods used in treating elderly acetabular fracture patients.
METHODS: A model of a comminuted acetabular fracture in osteoporosis was established, consisting of an anterior column-posterior hemitransverse fracture with disruption of the quadrilateral

From an experience of 120 fractures of the base of the first metacarpal bone, the authors evaluated the functional results of in 40 patients followed-up. Most of these patients had been treated by Bunden's method of longitudinal pinning. If one eliminates the comminuted fractures and the Bennett's fractures with a single large fragment treated by screw fixation the method of longitudinal pinning restores normal function to the first interdigitalspace Furthermore, the development of secondary arthrosis of the trapezo-metacarpal joint is rare.

Previous works had successfully demonstrated the clinical effectiveness of unilateral external fixator in treating various types of fracture, ranging from the simple type, such as oblique and transverse fractures, to complex fractures. However, literature that investigated its biomechanical analyses to further justify its efficacy is limited. Therefore, this paper aimed to analyse the stability of unilateral external fixator for treating different types of fracture, including the simple oblique, AO32C3 comminuted, and 20 mm gap transverse fracture. These fractures were reconstructed at the dis

BACKGROUND: Three cannulated cancellous screws (CCS) inverted triangle configuration is considered to be the gold standard for management of displaced intracapsular neck femur (ICNF) fractures in young adults. However, some authors have recommended four CCS in diamond configuration in the presence of comminution. However, there are no comparative studies to assess the superiority of one over the other. Therefore, the aim of the present study was to compare the radiological and functional outcomes of displaced, comminuted, ICNF fracture in young adults managed with three screw inverted triangle

Dual-plating of the distal femur is required in some cases to achieve stable fixation. The indications of a medial plate in addition to the lateral plate are medial supracondylar bone loss, low trans-condylar bicondylar fractures, medial Hoffa fracture, peri-prosthetic distal femur fractures, non-union after failed fixation with single lateral plate, poor bone quality and comminuted distal femur fractures (AO type C3). We recommend orthogonal plate configuration with locked plates by a single incision or dual incision approach as per surgeon choice.

A cooperative study between the Department of Otorhinolaryngology and the Highway Safety Research institute of the University of Michigan was designed to study temporal bone fracture produced in cadavers subjected to realistic automotive impact situations. Utilizing sled and piston impact configurations frontal and parietal impacts were noted to produce ipsilateral and contralateral fractures of nine temporal bones in seven cadavers. The impact velocities varied between 18.1 and 25.0 mph. Using standard otologic microsurgical techniques, the temporal bones were dissected and numerous gross and

In recent years biological surgical solutions have been recommended in cases of distal tibial fractures, with the aim of reducing damage to soft tissues and to bone vascular supply. Between the years 1991-1995, 52 patients suffering from fractures of the distal tibial metaphysis were treated in our department with an unreamed tibial nail (UTN). Fractures were categorized in accordance with the AO Classification. 32 fractures had no articular involvement (43A1, 43A2, 43A3) and 20 included intra-articular extension (43C1, 43C2). 32 fractures had significant metaphyseal comminution (43A2, 43A3, 4

This study analyzed the fate of plates used to correct maxillofacial injuries and defined risk factors that eventually resulted in plate removal. The outpatient clinic files of 108 patients treated with rigid internal fixation after maxillofacial trauma were reviewed. Study variables included age, sex, trauma circumstances, diagnosis, type of fracture, approach to the facial skeleton, presence of teeth in the line of fracture, plate material, site of plates, and reasons for plate removal. Of 204 plates used for fixation, 44 plates (22 percent) were removed. When all factors were considered tog

Patella fractures comprise 1% of all fractures. Treatment options vary based on fracture displacement, classification, and patient factors. Traditionally, nonoperative treatment has been reserved for nondisplaced fractures. Many operative treatments are available with differing indications and levels of success. Tension band constructs have been the most commonly employed approach to fixation, with cerclage wiring for comminuted fractures. Recently, plate fixation of patella fractures has become more popular. Plating constructs offer a low-profile design with stable fixation, allowing for earl

Precontoured olecranon plates are frequently used in the management of proximal ulna fractures. Occasionally, in comminuted proximal ulna fractures or segmental ulna fractures, available precontoured olecranon plates are too short for the management of these fractures. The authors have utilized posterolateral distal humerus plates in these instances. The coronal bend in some posterolateral distal humerus plates anecdotally fits well to the proximal ulna, despite being designed for the distal humerus. We sought to measure the coronal angulation of precontoured posterolateral distal humerus plat
