Management of frontal sinus fractures. Changing concepts.

Since the turn of the century, surgeons have handled frontal sinus fractures with a variety of different procedures. The optimal management procedure remains controversial. We have presented a graduated anatomic algorithm for treatment of frontal sinus fractures based on the degree of fracture displacement and nasofrontal duct involvement and presence of CSF leak. Nondisplaced fractures are best handled conservatively, without operative intervention. However, the majority of frontal sinus fractures require operative correction. Uncomplicated anterior table displacement with an aesthetic deform

Traumatic tension pneumocephalus: Two case reports.

INTRODUCTION: Traumatic pneumocephalus rarely evolves into tension pneumocephalus. It can be devastating if not recognized and treated promptly. CASE PRESENTATION: We presented two cases of post-traumatic tension pneumocephalus. A 30- year old male pedestrian hit by a car presented with right frontal bone fracture extending to right frontal sinuses. He developed pneumocephalus involving all ventricles and subdural space and extending down to foramen magnum with tight basal cistern. The patient was managed conservatively. During the hospital course, he developed cerebrospinal fluid leak from t…

Frontal sinus pneumatisation: an isolated finding or a sign of concomitant anatomical variation?

OBJECTIVE: To determine the association between frontal sinus pneumatisation and concomitant anatomical variations in paranasal sinuses as seen on computed tomography. METHODS: A total of 403 patients who underwent paranasal sinus computed tomography were allocated to three groups according to the degree of frontal sinus pneumatisation (type 1 - hypoplasia; type 2 - medium size; and type 3 - hyperplasia). In this unique model, the prevalence of ipsilateral variations on paranasal sinuses on each side of the head were analysed separately within each pneumatisation group. RESULTS: The type 3 f

Frontal intradiploic meningioma with progressive intracranial invasion: A rare case report.

RATIONALE: Intradiploic meningiomas are a subset of extradural meningiomas that arise in the skull. They are mostly localized in the frontoparietal and orbital regions, and they are often mistaken for primary bone tumors. PATIENT CONCERNS: The patient was a 48-year-old man with headache and a 12-year history of frontal cranium occupation, which was first discovered in 2005 and enlarged in 2009. The patient had a history of fracture in his frontal cranium 12 years ago. The computed tomography and magnetic resonance imaging revealed an occupation and intracranial invasion in frontal cranium. An

Frontal sinus disease. IV. Cellular response to experimentally-induced infection.

The pathogenesis of chronic frontal sinusitis is poorly understood. The canine frontal sinus model was developed to isolate experimentally the variables involved. We have recently determined that blockage of the naso-frontal duct does not, by itself, produce sinusitis or mucocele experimentally. The purpose of the present investigation is to examine the histological and ultrastructural responses of the sinus mucosa to artificially-induced infection. Bone wax was implanted in the canine frontal sinus via an osteoplastic flap, and subsequently removed three months later. Radiological, histologic

Molecular and clonal evolution in recurrent metastatic gliosarcoma.

We discuss the molecular evolution of gliosarcoma, a mesenchymal type of glioblastoma (GBM), using the case of a 37-yr-old woman who developed two recurrences and an extracranial metastasis. She was initially diagnosed with isocitrate dehydrogenase (IDH) wild-type gliosarcoma in the frontal lobe and treated with surgery followed by concurrent radiotherapy with temozolomide. Five months later the tumor recurred in the left frontal lobe, outside the initially resected area, and was treated with further surgery and radiotherapy. Six months later the patient developed a second left frontal recurre

A new reconstruction of the Le Moustier 1 skull and investigation of internal structures using 3-D-muCT data.

Using the non-destructive technique of 3-D micro computed tomography (3-D-microCT), we present a new, virtual reconstruction of the Le Moustier 1 Neandertal skull. This new reconstruction corrects defects found in earlier reconstruction attempts by repositioning misaligned cranial fragments, addressing the problem of asymmetry caused by pressure during the fossilization process, and placing the basioccipital in its proper anatomical position. Metric comparisons between Le Moustier 1 and juvenile and adult Neandertals demonstrate that facial height proceeded at a faster rate of growth than faci

Total Removal of a Giant Frontal Sinus Osteoma with Orbital Extension - A Case Report.

THE RATIONALE: Osteomas are benign tumours that are usually asymptomatic. However, giant osteomas can lead to severe complications. We report here the management of a giant frontal sinus osteoma with orbital extension. PATIENT CONCERNS: We present the case of a 17-year-old patient presenting with rapidly progressive isolated right exophthalmos. DIAGNOSIS: A cerebral computed tomography scan was performed and showed a giant osteoma of the right frontal sinus extending to the orbit and massively eroding the anterior wall of the frontal sinus. TREATMENT AND OUTCOMES: The patient underwent surg

Navigation as a tool to visualize bone-covered hidden structures in transfrontal approaches.

A retrospective analysis of 10 patients was performed to evaluate navigation systems in extranasal frontal skull base surgery. When performing a craniotomy following a bicoronal skin incision, the surgeon has to calculate the extent of the frontal sinus to avoid unnecessary damage to the dura or mucoceles later. Due to surgical morbidity including compression of the frontal lobe, many skull base surgeons have refused to use such an approach. Malformation or bone-destruction complicates the identification of the borders and increases the risk of side-effects. Navigation systems can be an altern

Use of tobramycin-impregnated antibiotic beads in frontal sinus osteomyelitis.

OBJECTIVE: Osteomyelitis of the frontal bone is a rare but devastating complication of frontal sinusitis. Treatment involves aggressive surgery to remove all sequestra in combination with long-term antibiotic therapy. However, systemic antibiotics may struggle to penetrate any remaining infection in devascularised areas, and the morbidity associated with surgical resection of some areas of the skull base is too high. In contrast, locally implanted antibiotics provide a reliable, high concentration of treatment to these areas while also minimising potential systemic side effects. The clinical a…

Potts puffy tumour: a rare but important diagnosis.

Potts puffy tumour (PPT) is a subperiosteal abscess arising from frontal bone osteomyelitis. We present a case of a 75-year-old lady with headache and a forehead swelling who was initially treated for sinusitis and giant cell arteritis in a primary care setting, but failed to improve. Following clinical deterioration and further investigation, CT appearances were consistent with a diagnosis of PPT and an extra-axial collection. Needle decompression, frontal trephine and endoscopic frontal sinusotomy were performed and intraoperative swabs cultured Streptococcus constellatus PPT is an unusual c…

Medial femoral condyle morphometrics and subchondral bone density patterns in Thoroughbred racehorses.

OBJECTIVE: To characterize medial femoral condyle (MFC) morphometrics and subchondral bone density patterns in Thoroughbred racehorses and to determine whether these variables differ between left and right limbs. SAMPLE: Stifle joints harvested from 6 Thoroughbred racehorses euthanized for reasons other than hind limb lameness. PROCEDURES: The distal portion of the left and right femurs of each cadaver was scanned via CT. Hounsfield units were converted to dipotassium phosphate equivalent densities through use of a phantom on each specimen. Medial femoral condyle width, length, height, and c

A Painless Retroauricular Mass in A Patient with Myotonic Dystrophy: Computed Tomographic Documentation of the Bone Changes that Occur in the Skull Base.

A 16-year-old male with a history of neonatal myotonic dystrophy had a painless enlarging left retroauricular mass. Computed tomography showed an enlarged frontal sinus, thickened apparently normal bone in the anterior left clinoid process, doresum sellae, and all portions of the temporal bones. In particular, there was bone production in the left retroauricular region and along the margins of the left internal auditory canal. The left temporomandibular joint was also dislocated. This is the first case report to document the dynamic aspect of the bone changes in this dystrophy.

Pott's puffy tumor: A rare complication of sinusitis. A case report.

A rare complication of frontal sinusitis includes Pott's puffy tumor. It manifests as a swelling of the forehead due to the presence of a subperiosteal abscess secondary to osteomyelitis of the frontal bone. A timely diagnosis allows for an early, intensive medical and surgical treatment, which is critical to prevent serious intracranial complications. Here we describe the case of a 12-year-old boy with Pott's puffy tumor as a complication of pansinusitis. This case was a diagnostic challenge; however, a timely treatment allowed for a favorable clinical course.

[The frontomaxillary suture. Theoretical bases and general principles of the application of postero-anterior extraoral forces to the orthopedic mask].

Each maxilla is joined to the frontal bone by a veritable fronto-maxillary sutural joint which, in the young subject, permits sliding movements. During growth, the maxilla orients itself in relation to the mechanical effects of the various structures which surround it. The location, below the frontal sinus, of this joint endows it with special properties of resistance and change in position of orientation. These anatomico-functional peculiarities explain certain results obtained by classical heavy antero-posterior of postero-anterior extra-oral forces.
