Adalimumab-Induced Psoriasis with Severe Alopecia.

A 52-year-old woman with a 10-pack-year smoking history presented with serious hair loss. She had a 10-year history of Crohn's disease associated with arthritis, for which she had been taking adalimumab. After 2 months of this treatment, she developed pustular lesions on her palms, soles, and scalp. Her gastroenterologist discontinued adalimumab injections, ordered skin biopsy, and switched her to ustekinumab 90-mg subcutaneous injections every 8 weeks. She then presented to our dermatology clinic, 2 months following the initiation of anti-IL12/23 medications, for her alopecia. She had no pers

Comorbidities in patients with spondyloarthritis.

Chronic inflammatory spondyloarthritis involves axial symptoms of the spine and sacroiliac joints, or peripheral arthritis. Many patients suffer from extra-articular manifestations. With acute anterior uveitis, rapid treatment prevents synechiae. Other organs can be involved. Treatment includes exercise, nonsteroidal antiinflammatory drugs (if insufficient response, tumor necrosis factor blockers), and (with peripheral arthritis) sulfasalazine. Patients with ankylosing spondylitis have comorbidities and increased cardiovascular risk. For uveitis or inflammatory bowel disease, patients should b

Non-IBD colitides: clinically useful histopathological clues.

Apart from inflammatory bowel diseases (IBD), there are several other form of colitis that may resemble macroscopically IBD, entering the differential diagnosis. These forms are represented by infectious colitis, ischemic colitis, pseudomembranous colitis, colitis related to diverticular disease, colitis related to mucosal prolapse, drug colitis, allergic colitis, and microscopic colitis. However, to distinguish between these forms is not always easy, and it frequently requires a strict interrelationship between the pathologist and the gastroenterologist. Here we discuss the more frequent form

Chronic gastritis in former times and now.

The roots of gastritis research stretch far back into the 19th century, and into early decades of the 20th century. Modern aspects in classification of gastritis and knowledge of the biologic course of chronic gastric inflammation, and of the links of chronic gastritis with many important gastric disorders were well known even at time of discovery of Helicobacter pylori in 1982. This discovery, however, significantly changed the field and raised the interest of gastroenterologists in gastritis. Chronic gastritis became a curable disease with known etiology and well-established links to main ga…

Diagnosis and management of cholecystitis and cholangitis.

Cholelithiasis is a prevalent condition in Western populations. Most cases are asymptomatic but complications can occur. Acute cholangitis, cholecystitis, and gallstone pancreatitis are the most common biliary tract emergencies and are usually caused by biliary calculi. Whenever possible, acute cholecystitis should be treated with early LC. AAC is an uncommon condition usually affecting patients with significant comorbidities. Treatment is usually with percutaneous cholecystostomy, which often is also the only required therapy. Endoscopic drainage is the preferred form of biliary decompression

[Complex diagnosis of congenital cranial dysostosis in children].

Ten patients (aged 3-15 years) with congenital cranial dysostosis were examined by a pediatrician, geneticist, gastroenterologist, neuropathologist, ophthalmologist, endocrinologist, and orthopaedist. In addition to the clinical signs characteristic of hereditary multiple developmental defects, the study revealed changes in the jaws and temporomandibular joint and local factors promoting the progress of deformations of the jaws. Manifest and inapparent pathological changes and dysfunctions in gastrointestinal organs were paralleled by dysfunctions of the central and autonomic nervous systems,

[Infections of the upper gastrointestinal tract--more than Helicobacter pylori?].

Helicobacter pylori infection is a problem frequently handled by the gastroenterologist, but many other infections may cause symptoms and pathological findings which is of great significance for the endoscopist to recognize. Candida infection is the most common infection in the oesophagus. It may signal an underlying disease like diabetes. When the patient enters Norway from an area where tuberculosis is endemic it is necessary to be aware of possible abdominal tuberculosis. Immuno-compromised patients, and especially AIDS-patients, may have several dangerous gastrointestinal infections. The m

[Etiology of recurrent ulcers after selective proximal vagotomy].

The work analyses the results of late-term follow-up of 525 patients who underwent selective proximal vagotomy (SPV) in different variants. The results were gained on the basis of long-standing follow-up with the use of the complex dispensary method. The frequency of peptic ulcer recurrences after SPV was 10%. The main causes of the recurrences were: incomplete vagotomy of the parietal cell zone and inadequate selection of patients for this intervention. Finally, the authors demonstrate the clinico-pathogenetical variants of the course of peptic ulcer when SPV cannot correct the existing disor

Acute severe ulcerative colitis: defining the precise moment for colectomy.

INTRODUCTION: Acute severe ulcerative colitis (ASUC) is a critical manifestation of ulcerative colitis (UC), often necessitating colectomy when medical management fails. Despite advancements in therapeutic interventions such as corticosteroids, biologics, and JAK inhibitors, a significant proportion of patients require surgery, with colectomy rates ranging from 10% to 15%. AREAS COVERED: This paper reviews the factors influencing the timing and necessity of colectomy in ASUC management, emphasizing the importance of multidisciplinary decision-making involving gastroenterologists and surgeons.…

Benign Anorectal Disorder Management in Low-Resource Settings.

There are many benign anorectal disorders, which can make patients seek care. In low-resource settings, the incidence of those pathologies is not different from the industrialized and western world. However, an interesting difference colorectal surgeons and gastroenterologists can face is the fact that many patients do not seek help or are not aware and have little opportunities to be helped. Latin America population is estimated to be around 8% of the world population, with Brazil having the largest percentage. Infectious diseases, which were previously under control or were steadily declinin…

Current Trends in the Management of Gastroesophageal Reflux Disease.

Gastroesophageal reflux disease (GERD) characterized by heartburn and/or regurgitation symptoms is one of the most common gastrointestinal disorders managed by gastroenterologists and primary care physicians. There has been an increase in GERD prevalence, particularly in North America and East Asia. Over the past three decades proton pump inhibitors (PPIs) have been the mainstay of medical therapy for GERD. However, recently there has been an increasing awareness amongst physicians and patients regarding the side effects of the PPI class of drugs. In addition, there has been a marked decline i…
