Expanding the scope of quality measurement in surgery to include nonoperative care: Results from the American College of Surgeons National Surgical Quality Improvement Program emergency general surgery pilot.

BACKGROUND: Patients managed nonoperatively have been excluded from risk-adjusted benchmarking programs, including the American College of Surgeons (ACS) National Surgical Quality Improvement Program (NSQIP). Consequently, optimal performance evaluation is not possible for specialties like emergency general surgery (EGS) where nonoperative management is common. We developed a multi-institutional EGS clinical data registry within ACS NSQIP that includes patients managed nonoperatively to evaluate variability in nonoperative care across hospitals and identify gaps in performance assessment that …

What Should We Write on End of Rotation Evaluations? A Mixed-methods Exploration of General Surgery Resident and Faculty Perspectives.

BACKGROUND: Narrative comments on resident end-of-rotation (EOR) evaluations are essential to competency-based assessment in surgical training, yet residents often perceive them as poor quality and variably useful. We explored resident and faculty perspectives on satisfaction with EOR comments, what constitutes high-quality comments and barriers to producing them. METHODS: This is a sequential mixed-methods study at a single academic general surgery residency. Semi-structured interviews with residents and faculty were thematically analyzed using directed content analysis guided by the Relatio…

Safety of Argon Plasma Coagulation for Minute Synchronous Neoplasms Detected under General Anesthesia during Endoscopic Laryngopharyngeal Surgery: A Case Series.

INTRODUCTION: Despite the use of narrow-band imaging, comprehensive endoscopic evaluation of the laryngopharynx in the endoscopy suite remains challenging because of its complex anatomical structure, limited working space, and reactions such as gagging and coughing. Consequently, minute synchronous neoplasms may occasionally be overlooked during the initial endoscopic assessment. We hypothesized that argon-plasma coagulation (APC) could serve as a minimally invasive treatment for minute synchronous neoplasms of the head and neck detected during endoscopic laryngopharyngeal surgery combined wit

SGLT-2 Inhibitor-Induced Euglycemic Diabetic Ketoacidosis Masked by Concurrent Pneumoperitoneum Following Spinal Surgery Under General Anesthesia: A Case Report.

Background: Sodium-glucose co-transporter-2 (SGLT-2) inhibitors are widely prescribed for type 2 diabetes mellitus (T2DM) because of their cardiovascular and renoprotective benefits. However, their use is associated with euglycemic diabetic ketoacidosis (EDKA), a rare but potentially life-threatening complication characterized by severe ketoacidosis despite relatively normal blood glucose levels. Failure to discontinue SGLT-2 inhibitors before surgery, as recommended in current guidelines, together with perioperative fasting and surgical stress, increases the risk of EDKA. Diagnostic complexit

Patient and Public Involvement in randomised controlled trials in general and abdominal SURGery: a protocol for the PPISurg systematic review.

INTRODUCTION: Randomised controlled trials (RCTs) are central to generating high-quality evidence in the surgical field but face unique methodological and practical challenges, including recruitment, follow-up, blinding and ensuring patient-centred outcomes. Patient and public involvement (PPI) has emerged as a promising strategy to enhance the relevance, quality and impact of surgical research by actively involving patients throughout the whole research process. Despite growing recognition of its value, the integration and reporting of PPI in surgical RCTs remain inconsistent, and no systemat

Impact of general anesthesia on postoperative complications in orthognathic surgery: a retrospective comparison of total intravenous anesthesia versus volatile anesthesia.

Orthognathic surgery has a high incidence of postoperative nausea (PON) and vomiting (POV), delaying mobility initiation and postoperative recovery. Bleeding is another risk associated with this surgical procedure. We aimed to compare total intravenous anesthesia (TIVA) and volatile anesthesia in patients undergoing orthognathic surgery in terms of postoperative nausea and vomiting (PONV) incidence and hemodynamic changes. This retrospective study included 82 patients who underwent bilateral sagittal split ramus osteotomies at Saga University Hospital between April 2016 and April 2021. We comp
