The genetic architecture of alopecia areata.

A major impetus to initiating the Human Genome Project was the belief that information encoded in the human genome would "accelerate progress in understanding disease pathogenesis and in developing new approaches to diagnosis, treatment, and prevention in many areas of medicine". Alopecia areata (AA) is a notable example of how understanding the genetic basis of a disease can have an impact on the care of patients in a relatively short time. Our first genome-wide association study in AA identified an initial set of common variants that increase risk of AA, some of which are shared with other a

Genetics of hypertrophic cardiomyopathy: one, two, or more diseases?

PURPOSE OF REVIEW: Hypertrophic cardiomyopathy is the most common identifiable cause of sudden death in the young. This review details the history of hypertrophic cardiomyopathy, recent discoveries in its genetic underpinnings and important genotype-phenotype relationships described in recent studies. RECENT FINDINGS: Since the discovery of the genetic underpinnings of hypertrophic cardiomyopathy in 1989 hundreds of mutations scattered among at least 10 sarcomeric genes confer the pathogenetic substrate for this 'disease of the sarcomere/myofilament'. More recently, the genetic spectrum of hy

Translation of MicroRNA-Based Huntingtin-Lowering Therapies from Preclinical Studies to the Clinic.

The single mutation underlying the fatal neuropathology of Huntington's disease (HD) is a CAG triplet expansion in exon 1 of the huntingtin (HTT) gene, which gives rise to a toxic mutant HTT protein. There have been a number of not yet successful therapeutic advances in the treatment of HD. The current excitement in the HD field is due to the recent development of therapies targeting the culprit of HD either at the DNA or RNA level to reduce the overall mutant HTT protein. In this review, we briefly describe short-term and long-term HTT-lowering strategies targeting HTT transcripts. One of the

Genetic Disorders of the Thoracic Aorta and Indications for Surgery.

Genetic disorders of the aorta are rare but can lead to life-threatening thoracic aortic aneurysms. Although the genetic causes of many of these connective tissue diseases are well defined, others such as familial thoracic aortic aneurysm and bicuspid aortic valve aortopathy are not. The natural history of genetic thoracic aortic aneurysms is not well understood or predictable, and surgical guidelines for treatment remain imprecise. Future research should strive to provide in-depth and detailed genetic profiling to drive clinical management, including medical and surgical therapies.

Genetic aspects of Alzheimer's disease, Pick's disease, and other dementias.

Although genetic testing is available for some degenerative diseases, in most types of dementia, both genetic and environmental factors are involved. Overall, dementing diseases can be either sporadic or inherited, and in general, the earlier the onset, the more likely a disease is to be inherited. Before genetic testing is performed, the ethical issues, such as the effect the tests might have on asymptomatic children, should be considered. The ethical use of DNA samples in research is another genetic testing issue to be considered.

Deciphering Facioscapulohumeral Dystrophy in the clinical trials era: where are we now?

OBJECTIVES: Facioscapulohumeral muscular dystrophy (FSHD) is a common genetic disorder characterized by progressive muscle weakness, especially in the face, shoulders, and upper limbs. Despite extensive research, the underlying pathogenesis and clinical variability remain incompletely understood. This review aims to summarize recent advances in FSHD research, focusing on genetic and epigenetic factors and the potential for precision medicine. METHODS: A comprehensive review of recent literature was conducted, examining molecular mechanisms such as mutations in the D4Z4 region, DUX4 expression

[Molecular genetics of human hypertension].

A genetic influence on blood pressure was demonstrated more than 100 years ago and a simple Mendelian inheritance was initially presumed. Platt and Pickering conducted a lively debate on this topic. Platt favored the idea that a single gene or only a few genes were responsible for high blood pressure. Pickering presented research results, which supported the assumption that many genes exerted an influence on blood pressure. This was all in a period when it was not even known what genes were. Genome-wide association studies (GWAS) according to the Pickering model have identified > 500 blood pre

Epidemiology of neural tube defects.

The epidemiological investigation of the common open neural tube defects (NTDs), anencephaly, and spina bifida, has a long history. The most significant finding from these past studies of NTDs was the identification of the protective effect of maternal, periconceptional supplementation with folic acid. Fortuitously, the association between folic acid and NTDs became widely accepted in the early 1990s, at a time when genetic association studies of complex traits were becoming increasingly feasible. The confluence of these events has had a major impact on the direction of epidemiological, NTD re

Epigenetic biomarkers in inflammatory bowel diseases-computational challenges and opportunities.

Inflammatory bowel diseases (IBD) remain a therapeutic challenge due to their heterogeneous nature and the absence of clinically actionable biomarkers to guide precision treatment. While multi-omics studies have advanced our understanding of the disease, meaningful translation into practice has been hindered by lack of validation and a need to move from association to a more generalizable signal. Epigenetics, particularly DNA methylation, offers a promising lens to capture disease-relevant regulatory states that reflect both genetic predisposition and environmental influences. However, the pat

Liver regenerative medicine approaches: Translational strategies for pediatric surgery.

Pediatric hepatobiliary diseases can progress to liver failure and transplantation, yet donor scarcity, lifelong immunosuppression, and cumulative graft-related morbidity remain major limitations. Regenerative medicine offers complementary strategies for disease modelling, therapeutic discovery, tissue repair, and clinical decision-making. Patient-derived organoids and microphysiological systems can reproduce disease-specific epithelial phenotypes, support drug screening, and provide mechanistic or prognostic information in conditions such as biliary atresia and inherited cholestatic disorders

A Qualitative Analysis of Parent-AYA Communication About Cancer Predisposition Diagnosed During Pediatric Cancer Care.

BACKGROUND: Cancer-predisposing germline variants are increasingly identified and disclosed during pediatric oncology clinical care. To understand whether and how parents communicate about identified cancer predisposition syndromes (CPS) with their adolescent and young adult (AYA) children, this study qualitatively characterized the content and approach (e.g., timing) of parent-AYA communication about AYAs' CPS. PROCEDURE: AYAs with a CPS identified during clinical cancer care and their parents independently completed semi-structured interviews regarding their CPS-related communication. Inter

Epigenetic regulation of neutrophil dysfunction in sepsis: mechanisms, biomarkers, and translational challenges.

BACKGROUND: Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection and remains a major challenge in critical care medicine. Neutrophils are central components of innate immunity, but their functions are markedly altered during sepsis. These alterations include disturbed granulopoiesis, prolonged survival, impaired migration, dysregulated inflammatory responses, imbalanced reactive oxygen species (ROS) production, and excessive formation of neutrophil extracellular traps (NETs). Epigenetic regulation may help explain how neutrophils acquire distinct f

Trust As the Foundation for Informed Decision-Making in Prenatal Genetic Screening and Diagnostic Testing: A Qualitative Study.

BACKGROUND: Prenatal genetic screening and diagnostic testing (PS&D) requires pregnant patients to make time-sensitive, high-stakes decisions that often involve multifaceted risk assessments and deeply personal values. These decisions are becoming increasingly complex as the clinical integration of new genomic science and technologies advances alongside parallel changes in maternal-foetal medicine, neonatology, and reproductive health services. Despite these changes, there is limited guidance on how best to prepare patients to formulate PS&D decisions with significant implications for obstetri

Well-being and self-care strategies for cardiovascular genetic counselors: A qualitative study.

Repeated exposure to challenging clinical situations impacts healthcare professionals' well-being. Genetic counselors assist individuals to understand and adapt to difficult medical information, which often has implications for close family members. In the cardiac setting, managing families with profound grief and trauma following sudden cardiac death can be difficult, with potential for burnout and compassion fatigue. The aim of this study was to explore the impact of cardiovascular genetic counseling practice on genetic counselor well-being and describe self-care practices. Participants were

Identifying potential LGBTQIA+ competencies for genetic counseling student training.

The LGBTQIA+ community faces considerable health disparities. Developing and integrating LGBTQIA+ competencies into healthcare provider training programs is one way to promote inclusive high-quality care to potentially improve this community's health. Currently, there are no established LGBTQIA+-specific competencies for genetic counseling graduate programs (GCPs), so training across GCPs likely varies. This qualitative focus group-based study aimed to explore current topics related to genetic counseling (GC) for LGBTQIA+ patients covered in North American GCPs, their learning objectives, and
