Characteristics of humoral and T-cell immune responses in people living with HIV after breakthrough SARS-CoV-2 Omicron variant infection during December 2022 to January 2023.

Due to inherent immune deficiency, the characteristics of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-specific immune responses in people living with HIV (PLWH) following breakthrough infection with remain incompletely elucidated. A large-sample real-world study was conducted from December 2022 to January 2023, which systematically analyzed immune responses in 1367 PLWH and 219 people without HIV (PWOH) by evaluating serum IgG antibody levels against SARS-CoV-2 wild-type strain and Omicron variants, neutralizing antibody titers, as well as the features of SARS-CoV-2-specific T

Diagnostic value of the urine lipoarabinomannan assay in HIV-positive, ambulatory patients with CD4 below 200 cells/μl in 2 low-resource settings: A prospective observational study.

BACKGROUND: Current guidelines recommend the use of the lateral flow urine lipoarabinomannan assay (LAM) in HIV-positive, ambulatory patients with signs and symptoms of tuberculosis (TB) only if they are seriously ill or have CD4 count ≤ 100 cells/μl. We assessed the diagnostic yield of including LAM in TB diagnostic algorithms in HIV-positive, ambulatory patients with CD4 < 200 cells/μl, as well as the risk of mortality in LAM-positive patients who were not diagnosed using other diagnostic tools and not treated for TB. METHODS AND FINDINGS: We conducted a prospective observational study incl

Systematic Structure-Activity Relationship Investigation on Novel 5‑Alkyl-6-benzyl-2-{[(methylthio/phenylthio)alkyl]thio}-pyrimidin-4(3H)‑ones Endowed with Potent Anti-HIV‑1 Activity.

Four series of 5-alkyl-6-benzyl-2-[(methylthio/phenylthio)-alkyl]-thiopyrimidin-4-(3H)-ones were systematically diversified at C2, C5, and C6 and evaluated as anti-HIV-1 agents. In MT-4 cells, these compounds displayed low-to-moderate cytotoxicity and subnanomolar-to-submicromolar inhibition of WT HIV-1, with potency governed by a balanced SAR. Ortho-dihalogenated C6-benzyl groups, particularly 2,6-F2 and 2,6-Cl2, were generally favored. In the 2,6-dihalobenzyl series, optimal activity required C5 methyl combined with para-substituted phenylthiomethyl groups at C2, whereas 3,5-Me2-benzyl analo

Molecular analysis of critical sequences within the EBNA-2 type 1 gene from Epstein-Barr virus isolates from patients with infectious mononucleosis, tonsillar hyperplasia, and HIV infection.

EBNA-2 is the first protein to be detected after infection of primary B lymphocytes by Epstein-Barr virus (EBV) and plays an essential role as transcriptional activator in EBV-induced lymphocyte transformation. We analysed by PCR and sequencing regions of the EBNA-2 type 1 gene from isolates from 13 children with infectious mononucleosis (IM), 6 children with tonsillar hyperplasia (TH), and 9 patients with HIV infection followed longitudinally. We found in all three groups of patients frequent non-silent point mutations at positions 48990, 48991, 49021, 49057, 49083, 49089, 49091, 49113, 49119

The effect of hypothetical interventions to limit weight gain on incident type 2 diabetes mellitus among people with HIV initiating antiretroviral therapy in the United States, 2012-2021.

Among people with HIV (PWH), initiating antiretroviral therapy (ART) is associated with weight gain that might increase the risk of cardiometabolic outcomes. We estimated the effect of hypothetical interventions to restrict increases in body mass index (BMI) in the two years after ART initiation on the 7-year risk of incident type-2 diabetes mellitus (T2DM) among PWH in the North American AIDS Cohort Collaboration on Research and Design (NA-ACCORD) clinical cohorts using the parametric extended g-formula. Among 13,822 normal weight or overweight PWH (BMI ≥18.5 and ≤29.9 kg/m2) who initiated AR

Safety and immunogenicity of GBS6 in pregnant women living with and without HIV and their infants in Uganda (PREPARE WP4): a double-blind, randomised, placebo-controlled, phase 2 clinical trial.

BACKGROUND: Immunisation in pregnancy against group B streptococcus could reduce severe disease in early infancy; however, there are few data on the safety and immunogenicity of vaccines given to women living with HIV and their infants. We aimed to evaluate the safety, tolerability, and immunogenicity of the hexavalent capsular polysaccharide conjugate group B streptococcus vaccine (GBS6) in pregnant women living with and without HIV, assess infant outcomes, and quantify the placental transfer of vaccine-induced antibodies to their infants. METHODS: We conducted a double-blind, randomised, pl

Development of an Ecological Momentary Assessment Study to Identify Real-Time Predictors of Physical Activity Among Older People With HIV: Protocol for a 2-Phase Mixed Methods Study.

BACKGROUND: People with HIV are aging rapidly and face accelerated aging-related comorbidities, including cardiovascular diseases and cognitive impairment, due to prolonged HIV-associated inflammation. Physical activity (PA) is a well-established intervention to mitigate these risks; yet, most older people with HIV remain sedentary. Despite considerable efforts to understand PA determinants and design interventions for people with HIV, outcomes have been suboptimal. OBJECTIVE: The overarching goal of this project is to use an ecological momentary assessment (EMA) approach to capture ecologica

Detection of SARS-CoV-2 and HHV-8 from a large pericardial effusion in an HIV-positive patient with COVID-19 and clinically diagnosed Kaposi sarcoma: a case report.

BACKGROUND: Pericardial effusion is a late manifestation of HIV more commonly observed in individuals with depressed CD4 counts. Although Mycobacterium tuberculosis remains to be one of the most frequently identified pathogens in the pericardial fluid among people living with HIV, less commonly described etiologies include SARS-CoV-2 that causes coronavirus disease and human herpesvirus-8 which is associated with Kaposi sarcoma. Isolation of more than one pathogen in normally sterile sites remains challenging and rare. We report the first documentation of both SARS-CoV-2 and HHV-8 in the peric

Successful Kidney Transplantation in a Recipient Coinfected with Hepatitis C Genotype 2 and HIV from a Donor Infected with Hepatitis C Genotype 1 in the Direct-Acting Antiviral Era.

Despite significant advances in transplantation of HIV-infected individuals, little is known about HIV coinfected patients with hepatitis C virus (HCV) genotypes other than genotype 1, especially when receiving HCV-infected organs with a different genotype. We describe the first case of kidney transplantation in a man coinfected with hepatitis C and HIV in our state. To our knowledge, this is also the first report of an HIV/HCV/HBV tri-infected patient with non-1 (2a) HCV genotype who received an HCV-infected kidney graft with the discordant genotype (1a), to which he converted after transplan
