Chrysomya bezziana oral myiasis.

Myiasis is an opportunistic infestation of human and vertebrate animals with dipterous larvae. Oral myiasis is a rare condition associated with poor oral hygiene, mental disability, halitosis and other conditions. We present a case report of an adult mentally challenged woman with extensive necrotic oral lesion burrowing into the hard palate through which three live maggots (larvae) were seen emerging out. The larvae were removed using forceps and the patient was treated with oral ivermectin. The maggots were identified as larvae of the Chrysomya bezziana fly.

Should upper gastrointestinal endoscopy be part of the evaluation for supraesophageal symptoms of GERD?

The past decade has witnessed increasing interest in the supraesophageal (ear, nose, and throat (ENT)) symptoms potentially caused by gastroesophageal reflux disease (GERD). Hoarseness, halitosis, problems with vocal presentations, excessive phlegm, frequent throat clearing, globus sensation, sore throat, cough, aspiration, laryngospasm, and laryngeal carcinoma are some, but not all of the ENT symptoms that have been attributed to GERD (1). The availability of highly effective medical and surgical therapies allows for control of these symptoms in many patients adding to the importance of accur…

Novel approach to management of a posterior tracheal tear complicating percutaneous tracheostomy.

We treated a patient who developed a posterior tracheal wall perforation and severe respiratory compromise following percutaneous tracheostomy, using a covered expandable metallic stent. The stent was deployed under direct vision using rigid and fibreoptic bronchoscopy. The defect was sealed and the right lung, which had been collapsed, was re-expanded. The patient was subsequently weaned from mechanical ventilation. Late complications included halitosis, which was treated with nebulized colistin sulphate, and the development of intratracheal granulation tissue, which was cleared using low pow…

Killian Jamieson Diverticulum: A Rare Cause of Dysphagia.

Killian-Jamieson diverticula (KJD) and Zenker's diverticula (more common) share similar pathophysiology but are considered to be different types of phrenoesophageal diverticula. A 55-year-old female presented to the clinic with chronic dysphagia, halitosis, and regurgitation. Imaging modalities confirmed a Killian-Jamieson diverticulum, explaining her symptoms. She was offered different treatment options and decided to proceed with a less invasive endoscopic approach. Physicians should be aware of the variable presentations of KJD and the different available treatments as newer techniques are …

The relevance of alcohol to dental practice.

Alcohol is a very widely used drug which can cause dental disease and influence dental treatment. It is therefore important that dental professionals take an alcohol history from all patients and are aware of how to give alcohol reduction advice. This article discusses the impact of alcohol on oral health, including oral cancer, trauma, halitosis, tooth wear, periodontal disease and caries, with reference to the available literature. It also outlines current alcohol risk level advice and guidance regarding giving alcohol reduction advice in a dental setting.

[Tonsilloliths on a panoramic radiograph].

Tonsilloliths are calcifications within the tonsillar crypts. Affected are especially the palatine tonsils. The prevalence is ranging between 16 and 46.1%. Tonsilloliths can be the reason for chronic halitosis, irritating cough, dysphagia, otalgia, foreign body sensation or foul taste in the mouth. They are often asymptomatic. Tonsilloliths can be discovered incidentally in orthopantomography (OPT) as singular or multiple radioopacities in the area of the ramus mandibulae. The aim of this case report is to present two cases with asymptomatic tonsilloliths detected based on an OPT and confirmed…

[Megaesophagus in the dog and cat].

Megaesophagus is a disorder of the esophagus characterized by diffuse dilation and decreased peristalsis. It is classified into congenital and acquired forms. Gastrointestinal, endocrine, immune-mediated, neuromuscular, paraneoplastic, and toxic disorders have been associated with acquired megaesophagus. Common clinical signs of megaesophagus are regurgitation, weight loss, coughing, and halitosis. Most cases of megaesophagus can be diagnosed using thoracic radiography; however, diagnosing the underlying cause requires a thorough history and additional diagnostics. The treatment, management, a…

[Osteonecrosis of the jaw secondary to oral alendronate: Report of three cases].

Osteonecrosis of the jaw associated to biphosphonate use is more common in cancer patients with bone metastases, that are using intravenous diphosphonates. When these drugs are used orally the risk of the complication is lower. We report 3 diabetic women aged 69, 76 and 82 years, receiving alendronate 70 mg every one week. The unveiling event was the extraction of several teeth without the use of antibiotics. All had bone pain, purulent discharge, loss of bone and halitosis. All improved five months after discontinuing alendronate.
