Periodic Artifact Removal With Applications to Deep Brain Stimulation.

Deep brain stimulation (DBS) therapies have shown clinical success in the treatment of a number of neurological illnesses, including obsessive-compulsive disorder, epilepsy, and Parkinson's disease. An emerging strategy for increasing the efficacy of DBS therapies is to develop closed-loop, adaptive DBS systems that can sense biomarkers associated with particular symptoms and in response, adjust DBS parameters in real-time. The development of such systems requires extensive analysis of the underlying neural signals while DBS is on, so that candidate biomarkers can be identified and the effects

Fistula-sparing negative pressure wound therapy in a patient with polytrauma: a case report.

Enteroatmospheric fistulas (EAFs) are characterised as an abnormal connection between the gastrointestinal tract and the atmosphere. They pr … ft tissue infection requiring large abdominal wall debridement, pelvic hardware infection requiring hardware removal and external fixation, and fascial dehiscence culminating with the development of an EAF. Several iterations of wound care were attempted before succeeding at achieving a fistula-sparing NPWT dressing. Once this was achieved there was rapid improvem …

Lead migration and postdural puncture headache following spinal cord stimulator placement in patient with cough: a case report.

INTRODUCTION AND IMPORTANCE: Spinal cord stimulator (SCS) implantation has become a well-validated treatment for refractory chronic back pain. While generally safe, complications such as lead migration and postdural puncture headache (PDPH) can arise. Recognizing these hardware and biologic complications is crucial to improving outcomes and guiding management strategies. CASE PRESENTATION: A middle-aged male with chronic nocturnal cough underwent successful percutaneous SCS trial placement under fluoroscopic guidance. Adequate pain coverage was confirmed, and he was discharged home. The follo

Increased electrode impedance as an indicator for early detection of deep brain stimulation (DBS) hardware Infection: Clinical experience and in vitro study.

BACKGROUND: When deep brain stimulation (DBS) infections are identified, they are often too advanced to treat without complete hardware removal. New objective markers to promptly identify DBS infections are needed. We present a patient with GPi (globus pallidus interna) DBS for dystonia, where the electrode impedance unexpectedly increased 3-months post-operatively, followed by serologic and hematologic markers of inflammation at 6-months, prompting explantation surgery. We recreated these conditions in a laboratory environment to analyze the pattern of changing of electrical impedance across …

Tissue reaction to implant corrosion in 38 internal fixation devices.

The corrosion characteristics, metallurgical properties, and clinical performance of 38 retrieved internal fixation devices were correlated with the tissue reaction to these devices. Metallurgical parameters included thin and heavy inclusion content, Rockwell hardness, and grain size. The excised fibrous tissue strip was directly overlying each plate at removal and sectioned between screw-hole sites. The material studied from the 38 plates consisted of 201 screw-hole junctions with associated tissue biopsy sites. Clinical histories were obtained on all 38 patients with hardware removal. The av…

Effects of electrical brainstem stimulation on tinnitus.

The primary aim of the auditory brainstem implant (ABI), an investigational device, is to restore auditory sensations in neurofibromatosis-2 patients deafened by bilateral acoustic neuroma surgery. An electrode is placed on the dorsal cochlear nucleus after the tumor is removed through the translabyrinthine approach. This procedure was performed on 18 patients: 13 were completely deafferentated after bilateral tumor removal and had received the ABI during surgery on the second-sided tumor; 5 were implanted at first-sided tumor removal. Three of these 5 patients had usable hearing remaining on

Complications associated with retained foreign bodies from infected penile implants: proposal for the use of an implant-specific checklist at the time of device removal.

INTRODUCTION: Infection of a penile prosthesis requires that all device hardware and associated foreign materials are removed, irrespective of whether a salvage procedure will be performed. Failure to remove all foreign bodies from the operative field may result in persistent infection, necessitating surgical intervention. AIM: To review our experience with complications arising from retained foreign bodies following removal of an infected penile prosthesis. We highlight the clinical features that should raise suspicion of retained device-associated materials, and also the role of imaging in

Dual-plating is associated with lower implant removal rates compared with single-plate fixation for midshaft clavicle fractures: a meta-analysis.

BACKGROUND: Displaced midshaft clavicle fractures are often managed operatively, with single-plate fixation serving as the standard approach. Dual-plate fixation has emerged as an alternative, particularly for complex fracture patterns, aiming to reduce hardware-related complications and enhance biomechanical stability. The aim of this study was to compare outcomes and complications following single and dual-plating techniques for midshaft clavicle fractures. METHODS: A systematic literature search was performed using PubMed, Scopus, Embase, and Google Scholar through July 20, 2025. Comparati

Surgical treatment of extra-articular or simple intra-articular distal tibial fractures: MIPO versus supercutaneous plating.

Minimally invasive plate osteosynthesis (MIPO) has become a widely accepted technique to treat distal tibial fractures. Recently, the novel application of a locking plate used as an external fixator (supercutaneous plating) was introduced for the management of open fractures and infected nonunions and even as an adjunct in distraction osteogenesis, which is considered another less invasive method. The aim of this study was to compare the results of supercutaneous plating with closed reduction and minimally invasive plating in the treatment of distal tibial fractures. Forty-eight matched patien

Modified Lapidus procedure with a nitinol staple and two screw construct technique.

Many fixation options exist for correction of a hallux valgus deformity with a first tarsometatarsal joint arthrodesis with pros and cons to each. Few have looked at utilizing a shape memory alloy staple made from nitinol for fixation. This study evaluates the effectiveness of a modified Lapidus technique using a nitinol (shape memory alloy) staple combined with a two-screw construct for first tarsometatarsal joint arthrodesis in correcting hallux valgus deformity. The non-union rate, time to weightbearing in both a boot and a shoe, hardware removal of the staples, and the need for revision su…
