Hypoplastic left heart syndrome.

HLHS is a complex developmental malformation of the left ventricle, aorta, and associated valves. Survival beyond neonatal life is the exception in untreated infants. The prognosis, however, is no longer hopeless. Newer surgical strategies of staged palliative reconstruction and heart replacement by transplantation appear promising. These options on occasion will become complementary in the management of individual patients. Those children surviving Fontan operations are frequently able to lead NYHA Class I or II lives without drugs, while transplant recipients will require one or more immunor…

Heart transplantation in children.

Heart transplantation in children is being performed with increasing frequency. As experience has accrued, problems of rejection, graft atherosclerosis, and growth have been noted. Seventeen children (seven boys and 10 girls) between the ages of 5 months and 14 years have undergone heart transplantation since 1981. The preoperative diagnosis was cardiomyopathy in 13 children, congenital heart disease in two, and endocardial fibroelastosis in two. Immunosuppressive therapy has included a tapering schedule of cyclosporine, azathioprine, and prednisone. There are 13 children alive, with four hosp…

[Heart transplantation in childhood].

In nine patients between the ages of 3 months and 18 years with endstage heart failure, orthotopic heart transplantation was performed. Seven of these patients suffered from dilative cardiomyopathy. Additional diagnoses were tricuspid atresia in one case, and hypoplastic left-heart syndrome in another case. Seven of these children (77.7%) were catecholamine-dependent before the operation. The postoperative immunosuppressive treatment consisted of a combination therapy of cyclosporine A, azathioprine, and prednisolone, similar to the treatment in adults. Diagnosis of rejection was based on both…

Neuropathology of heart transplantation.

The neuropathology of 18 cardiac transplant recipients was reviewed with the clinical findings. Pathological changes were noted in the central nervous system (CNS) in 94% of the patients, the most frequent being cerebral vascular in origin (72%). Eight patients (44%) had multiple cerebral infarcts and morphologically, a large number of these antedated the transplantation. In addition 4 patients had acute focal ischemic changes which occurred after transplantation. Intracranial hemorrhage was noted in 5 patients (28%), including one case of fatal intracerebral hemorrhage following an acute hype

Fetal complete heart block.

In a series of over 6000 patients referred for fetal echocardiography during an eight year period, 37 fetuses were found to have complete heart block. There were 16 cases of isolated heart block and 21 cases associated with structural heart disease. All mothers of fetuses with isolated complete heart block had evidence of circulating syndrome Sjögren A antibody (Ro). Only one mother had clinical evidence of connective tissue disease. In the 21 cases associated with structural heart disease there were 17 cases of atrioventricular septal defect, one case of secundum atrial and perimembranous ven…

Echocardiography. Acquired heart disease.

Acquired disease of the cardiac valves, myocardium, and pericardium may be recognized through echocardiography. Quantitation of atrial and ventricular dimensions is a key aspect in the echocardiographic evaluation of acquired heart diseases. Subjective interpretation permits identification of pericardial effusion, dilated cardiomyopathy, valvular lesions, cardiac masses, and abnormal blood flow.

The third heart sound.

A unitary concept is proposed to explain the genesis of the third heart sound and associated "rapid filling wave" of the apexcardiogram in physiological and pathological states including constrictive pericarditis. This theory not only clarifies the hitherto unexplained phenomenon such as presence of S3 in significant mitral stenosis, but also places in proper perspective the hemodynamic significance of S3 in physiological and various pathological states.

Echocardiography. Congenital heart disease.

Echocardiography can be used to identify malformations of the heart and to assess the degree of cardiac compensation that occurs with congenital heart disease. Integration of clinical and ultrasound data will generally allow a diagnosis to be made noninvasively. Contrast echocardiography may delineate intracardiac shunting.

Congenital complete heart block.

Recent studies have demonstrated immunologic abnormalities in the mothers of infants with congenital complete heart block. The disorder should be suspected when a slow pulse rate is detected before or after birth. The block most often occurs in the AV node. There are several specific indications for pacemaker therapy. Most patients who survive beyond infancy are able to lead relatively normal lives.

Heart transplantation in Arizona.

From January 1, 1979 through October 31, 1984, 58 heart transplantations have been performed in 57 patients. Conventional immunosuppression was utilized in the first 32 patients. Since January 1, 1983 cyclosporine and a low dose of prednisone have been the immunosuppressive agents of choice. The annual survival rates were 75% in 1979, 67% in 1980, 75% in 1981, 57% in 1982 and 60% in 1983. Patients whose age ranged from 50 to 57 years had similar survival to the younger recipients. Early deaths resulted from infection in 3 patients, acute rejection in 4, acute failure of the donor heart in 2 an…

[Heart transplantation in cardiomyopathy].

Within the spectrum of presently accepted candidates for heart transplantation, end-stage heart failure in dilated cardiomyopathy has become the principle indication. Although several indicators of poor prognosis have been specified, the decision for heart transplantation is primarily made on clinical grounds. Expected long-term survival after transplantation is 60 to 80% at one year and more than 50% at five years. Since July, 1983, 50 patients underwent orthotopic heart transplantation, 38 of whom had been suffering from dilated cardiomyopathy. Ages ranged from nine to 54 years with a mean o…

Pregnancy and heart disease.

The maternal cardiovascular system undergoes extensive changes during pregnancy. These changes become especially important in women with underlying cardiac disease. In order to provide a basis for understanding cardiac disease in pregnancy, the authors provide a discussion of the normal cardiovascular physiology of pregnancy, followed by a review of the types of heart disease in women of childbearing age.

Total artificial heart implantation.

This report details the implantation of a total artificial heart into a human being who survived for 112 days. Included are the criteria for candidate selection, the preoperative psychiatric evaluation, and the description of operative and postoperative complications in the context of the research subject's mental status over the period of his survival. Postmortem pathologic findings are reviewed. We summarize pertinent literature, including disordered behavior, postcardiotomy, and neuropsychiatric syndromes in hepatic, renal, and cardiac transplant patients.

[Univentricular heart; angiocardiographic study].

Authors present angiocardiographic study of 19 patients with univentricular heart. It was realized during the neonatal period in 13 cases and between the six first six months in the rest. To determine the existence of an only ventricle contrast was injected in the right and left atrium and the ventricle through the right and left atrioventricular valve. The bulbo-ventricular foramen was orientated in all cases from back-front wards. Those of left ventricle type had a right anterior, anterior-superior or left-anterior rudimentary chamber. Depending on its' morphology they were classified in thr…
