Antiarrhythmic properties of ranolazine: A review of the current evidence.

Ranolazine was developed as an antianginal agent and was approved by the Food and Drug Administration in 2006 for use in chronic stable angina pectoris. Experimental and clinical studies have shown that it also has antiarrhythmic properties based on the frequency-dependent blockade of peak sodium channel current (peak INa) and rapidly activating delayed rectifier potassium current (IKr) in the atria and blockade of late phase of the inward sodium current (late INa) in the ventricles. Recent clinical studies have revealed the efficacy of ranolazine in prevention of atrial fibrillation in patien

Arrhythmia-related workup in hereditary myopathies.

BACKGROUND: Arrhythmias determine life expectancy in patients with hereditary myopathies. AIMS: The aim of this study was to summarize recent advances in the diagnosis and management of arrhythmias in hereditary myopathies. METHODS: Literature search via PubMed and inclusion of own experiences were performed. RESULTS: All types of arrhythmias can be found in patients with hereditary myopathies, but some are more prevalent than others. Arrhythmias reported in myopathies include atrial fibrillation, atrial flutter, sick-sinus syndrome, preexcitation syndromes, atrioventricular conduction dela

Electrovectorcardiographic diagnosis of left septal fascicular block: anatomic and clinical considerations.

Several publications considering anatomical, histological, pathological, electrocardiographic, vectorcardiographic, and electrophysiologic studies have shown that the left bundle branch splits into three fascicles or in a "fan-like interconnected network" in the vast majority of human hearts. The left His system is trifascicular with a left anterior, a left posterior, and a left septal fascicle (LSF). Consequently, the classic term "hemiblock," to describe the block of one of the fascicles, established several decades ago by the Rosembaum's school, should be updated. Electrovectorcardiographic

Intraventricular conduction abnormalities.

There are multiple types of intraventricular conduction abnormalities, each with its own unique clinical significance. It is useful to categorize conduction abnormalities, or blocks, by the number of fascicles involved. This article reviews intraventricular conduction abnormalities.

Acquired forms of the Brugada syndrome.

The Brugada syndrome is characterized by ST-segment elevation in the right precordial leads (V1 through V3) and an episode of ventricular fibrillation in the absence of structural heart disease. SCN5A, the gene encoding the alpha subunit of the sodium channel, is the only gene thus far linked to the Brugada syndrome but is identified in only 18% to 30% of patients with clinically diagnosed Brugada syndrome. On the other hand, experimental studies have suggested that an intrinsically prominent transient outward current-mediated action potential (AP) notch and a subsequent loss of the AP dome in

['Uninterrupted' anterograde atrioventricular excitation conduction in patients with paroxysmal reciprocal atrioventricular nodal tachycardias].

Different variants of anterograde atrioventricular conduction of excitation were distinguished basing on data obtained from 150 patients with paroxysmal reciprocal atrioventricular nodal tachycardias. Excitation conduction during programmed and accelerating atrial transesophageal pacing allowed to assess characteristics of excitation conduction along fast and slow conduction pathways in atrioventricular node. Some parameters were suggested that allowed to predict possibility of detection of different variants of anterograde atrioventricular conduction of excitation. Electrophysiological mechan

Mechanisms of antiarrhythmic drug action on termination of atrial flutter.

Atrial flutter is an important arrhythmia in clinical practice. Although the reentrant circuit of human typical atrial flutter is well characterized, the action of antiarrhythmic drugs on this tachycardia is less understood. Based on the recent clinical trials, pure Class III drugs like ibutilide or dofetilide are more effective in acute termination of human atrial flutter than Class I drugs like procainamide or flecainide. The mechanisms of drug induced termination of atrial flutter include refractory block due to cycle length oscillation, fixed block due to a reduced safety factor for conduc

Catheter ablation of atrial flutter.

Typical atrial flutter in humans is the consequence of a stable macro-reentrant circuit produced by the unique right atrial architecture providing anatomic barriers and functional blocks to conduction. Mapping studies have indicated that the so-called isthmus between the inferior aspect of the tricuspid annulus and the ostium of the inferior caval vein is a critical zone for maintenance of atrial flutter. An anatomically guided approach with placement of a transmural and contiguous lesion line throughout the isthmus has established as curative treatment of typical atrial flutter. Electrophysic

Activation-repolarization coupling in the normal swine endocardium.

BACKGROUND: While abnormalities of activation and repolarization play an important role in arrhythmogenesis, little information is available on the interaction between their spatial dispersions in the heart. This study examined the effects of activation spread on the spatial distribution of the repolarization properties during different depolarization patterns. METHODS AND RESULTS: Left ventricular (LV) endocardial activation and repolarization patterns were mapped in 13 healthy pigs. LV local activation, repolarization, and activation-recovery interval (ARI) times were determined from the in

Electrophysiology of atrial fibrillation and its prevention by coronary sinus pacing.

Atrial fibrillation (AF) is a re-entrant rhythm, and patients with AF have intra-atrial conduction abnormalities as evidenced by prolonged P-wave duration, abnormal SAECG of P wave, fragmented atrial electrograms and greater intra-atrial conduction delays in response to APDs. Our previous work has proposed that intra-atrial conduction delays and dispersion of refractoriness during extrastimulus testing are site dependent; high right atrial (HRA) stimulation results in marked prolongation of intra-atrial conduction times and AF, whereas distal coronary sinus (CS) stimulation is associated with

Epicardial electric shock ablation of the left lateral accessory pathway.

Fifty patients with drug-resistant, recurrent tachyarrhythmias causing Wolff-Parkinson-White syndrome underwent surgery between 1990 and 1992. All recognized surgical methods for accessory pathway destruction were performed. Epicardial electric shock ablation was first used as a method of surgically destroying an accessory atrioventricular pathway in 1983. This technique avoids the need for cardioplegia and hypothermia during operation. The procedure is based on the application of a series of two to five electrical shocks (50-150 J) to the region of the atrioventricular groove where the access

Advances in the diagnosis and therapy of syncope and palpitations in children.

Although advances in diagnosis and therapy of congenital heart disease have received most of the attention, in the past few years advances in the more common troublesome problems of adolescents with syncope and palpitations have been as dramatic. Syncope may be due to a cardiac arrhythmia, but is more commonly due to an autonomic dysfunction of blood pressure control. In cases where the diagnosis is equivocal by history alone, autonomic testing, including head-up tilt table testing, may be useful. Treatment with a salt-enriched diet and a mineralocorticoid is usually effective in reducing or e
