Long-term survival of patients with partial atrioventricular septal defect (PAVSD) without operative therapy is very rare. The case is reported of a 75-year-old man who presented with dyspnea and decline of general condition. A heart defect was known but not investigated as the patient refused to undergo invasive cardiologic diagnosis. Hence, treatment was conservative. At autopsy, a huge (3 cm diameter) ostium primum atrial septal defect (ASD) was found, together with deformation of the right and left atrioventricular valves and extreme dilatation of the heart. Pulmonary arteriosclerosis indi

A prospective study of 3-dimensional (3-D) transthoracic echocardiographic definition of atrioventricular septal defect (AVSD) morphology and its dynamic changes during the cardiac cycle was performed. The information obtained from 2-D and 3-D transthoracic echocardiography (TTE) was compared with intraoperative findings in an unselected group of 15 patients with AVSD (median age 22 months). In all study patients, 3-D reconstructions provided anatomic views of the atrioventricular valve(s) en face from either atrial or ventricular perspectives that allowed comprehensive assessment of dynamic v

BACKGROUND: Most non-chromosomal congenital heart defects are thought to be caused by the interaction of genetic factors involving multiple genes and environmental factors. Families that have several affected members have been reported, however, which suggests that a single autosomal dominant or recessive gene may cause the cardiac defects. A family in which atrioventricular septal defect seemed to be a single gene disorder is reported.
OBSERVATIONS: A family in which the mother and her two daughters from different fathers had atrioventricular septal defect not associated with trisomy 21 is r

Subaortic stenosis has been described with increasing frequency as an ominous feature of atrioventricular septal defect (AVSD), especially following surgical correction of the anomaly in non-Down's syndrome patients. In order to study the surgical anatomy of the left ventricular outflow tract in this malformation, 48 hearts featuring AVSD were examined. Obstructive lesions were classified into unequivocal forms (class A, 13.5%) and potential ones (class B, 10.8%). In the remaining hearts (class C, 75.7%) no obstruction was noted. In class A, subaortic stenosis was due to exaggeration of the an

Isolated ostium secundum atrial septal defect (OS-ASD) may rarely lead to severe symptoms in infancy. Over a period of four years, 12 infants admitted to our hospital with severe congestive heart failure had an isolated OS-ASD. Their echocardiographic findings, clinical course, and outcome were outlined. All 12 infants had atypical findings including a holosystolic murmur parasternally and absence of wide fixed splitting of the second heart sound, and none were clinically suspected to have an ASD. M-mode echocardiography in all infants showed a greater than normal diastolic right ventricular i

Hypoplastic left heart syndrome (HLHS) is a complex combination of cardiac malformations that probably results from multiple developmental errors in the early stages of cardiogenesis and that, if left untreated, invariably proves fatal. A variety of chest radiographic findings are seen in patients with HLHS, including an enlarged cardiac silhouette (notably a prominent right atrium), pulmonary venous hypertension, an atrial septal defect, and valvular stenosis or atresia. The recent evolution of palliative surgical procedures (modified Norwood procedure, bidirectional cavopulmonary shunt, modi

BACKGROUND: Patients with atrial septal defects and pulmonary hypertension would benefit from early identification and treatment of pulmonary hypertension prior to closure. Noninvasive screening tools for pulmonary hypertension are inadequate. Electrocardiography, though readily available, has so far been of limited screening utility in patients with pulmonary hypertension. We hypothesize that derived vectorcardiographic parameters will aid in identifying increased right ventricular afterload and thus provide an additional screening tool for pulmonary hypertension in patients with secundum atr

Congenital heart disease (CHD) represents one of the most common birth defects, but the genetic causes remain largely unknown. Mutations in GATA4, encoding a zinc finger transcription factor with a pivotal role in heart development, have been associated with CHD in several familial cases and a small subset of sporadic patients. To estimate the pathogenetic role of GATA4 in CHD, we screened for mutations in 357 unrelated patients with different congenital heart malformations. In addition to nine synonymous changes, we identified two known (A411V and D425N) and two novel putative mutations (G69D

Minimally invasive techniques in congenital heart surgery have evolved steadily over the past 5 years. Initially, instrumentation and techniques were adopted from other subspecialties, and efforts were directed at simple extracardiac repairs. These efforts established the safety and efficacy of video-assisted endoscopic techniques for pediatric cardiac surgery. As instruments and techniques evolved, intraoperative cardioscopy became feasible and showed the utility of these new imaging techniques in facilitating open cardiac repairs by exposing remote areas within the heart. This experience has

PURPOSE: The purpose of this paper is to analyze the feasibility of the full sternotomy with minimal skin incision and its related complications and risks.
METHODS: A total of 405 patients with simple congenital heart disease underwent open heart surgery exclusively under full sternotomy with minimal skin incision. We reviewed the available medical records of the patients retrospectively. Bypass time, aorta cross clamp time, and period of hospital stay were compared with the control group (223 patients with standard long skin incision).
RESULTS: Full sternotomy with minimal skin incision pro

BACKGROUND: Previously, we explored the role of Sox9 in the second heart field (SHF) in atrioventricular septation. For that study, we created a SHF-specific Sox9 knockout mouse. In addition to the presence of primary atrial septal defects in half of the offspring, we found that virtually all specimens also developed a ventricular septal defect. Histological analysis suggested that the ventricular septal defects resulted from developmental perturbation of the mesenchymal structures within the outflow tract. In the current study, we investigated the role of Sox9 in the SHF in the development of

OBJECTIVE: Amplatzer (AGA Medical Corporation, Plymouth, Minn) septal and vascular occluder devices have significantly altered the care of patients with congenital heart disease. The relative frequency and consequence of complications resulting from the attempted placement of such devices, however, have not been well assessed. The purpose of this study is to use large databases to assess the frequency and severity of such complications and compare them with those of surgical atrial septal defect closure.
METHODS: The US Food and Drug Administration Manufacturer and User Facility Device Experi

BACKGROUND: The occurrence of congenital heart disease (CHD) with congenital biliary disease is uncommon. Our aim is to present our experience in living donor liver transplantation (LDLT) as treatment for end-stage liver disease (ESLD) in children with CHD.
METHODS: A review of transplant records from June 1994 to December 2004 was performed. Twenty-three LDLT (13 males, 10 females) recipients were diagnosed to have both CHD and ESLD.
RESULTS: CHD diagnoses were made preoperatively using transthoracic two-dimensional color flow Doppler echocardiography. The mean age was 22.3 months. There we

The dawn of open-heart surgery occurred at the University of Minnesota on September 2, 1952, when Dr John Lewis and his surgical team closed an atrial septal defect using total-body hypothermia and inflow stasis. Eighteen months later on March 26, 1954, Dr C. Walton Lillehei and his surgical team used a parent as a biologic oxygenator to repair a ventricular septal defect in a 1-year-old boy. This technique of cross circulation was then used in a total of 45 children during the next 16 months to successfully repair tetralogy of Fallot, atrioventricular canal, and ventricular septal defect. The authors report the occurrence of infective endocarditis in a 32-year-old man with a ventricular septal defect and a left ventricular-to-right-atrial shunt who adhered to the revised 2007 American Heart Association guidelines for infective endocarditis. The patient had received antibiotic prophylaxis prior to multiple previous dental procedures. At a recent dental evaluation for fillings, he was informed that he no longer needed prophylaxis. Fatigue and fevers developed 1 week later, and he was treated with an oral course of ciprofloxacin. The symptoms recurred, and blood cultures grew Stre

BACKGROUND: Little attention has been paid to whether the interventricular communication in complete atrioventricular septal defect is different beneath the superior and inferior bridging leaflets, a feature of obvious surgical significance.
METHODS: We searched for a defect under the bridging leaflets and examined the valve morphologies in 98 autopsied and 86 surgical patients. Of the overall specimens, 27 were associated with Fallot's tetralogy, and a further 20 had subaortic stenosis, aortic coarctation, or both. In the autopsied specimens, we also measured the degree of deficiency of the
